Objective To evaluate the perinatal and maternal outcomes of pregnancies in SARS-CoV-2 infected women, comparing spontaneous and In Vitro Fertilization (IVF) pregnancies (with either own or donor oocytes). Design Multicentre, prospective, observational study. Setting 78 centres participating in the Spanish COVID19 Registry. Patients 1,347 SARS-CoV-2 positive pregnant women registered consecutively between February 26 th and November 5 th , 2020. Interventions Patient´s information was collected from their medical records, and multivariable regression analyses were performed, controlling for maternal age and the clinical presentation of infection. Main outcome measures Obstetrics and neonatal outcomes, pregnancy comorbidities, intensive care unit admission, mechanical ventilation need and medical conditions. Results The IVF group was composed of 74 (5.5%) women whereas the spontaneous group included 1,275 (94.5%) women. Operative delivery rate was high in all patients, especially in the IVF group, where C-section became the most frequent method of delivery (55.4%, compared to 26.1% of spontaneous). The reason for C-section was induction failure in 56.1% of IVF patients. IVF women had more gestational hypertensive disorders [16.2% vs 4.5% among spontaneous, adjusted Odds Ratio (aOR) 5.31, 95% Confidence Interval (CI) 2.45-10.93) irrespective of oocyte origin. The higher rate of ICU admittance observed in the IVF group (8.1% vs 2.4% spontaneous) was attributed to pre-eclampsia (aOR 11.82, 95% CI 5.25-25.87), not to the type of conception, Conclusions High rate of operative delivery has been observed in SARS-CoV-2 infected women, especially in IVF pregnancies; method of conception does not affect foetal or maternal outcomes, except for pre-eclampsia.
Análisis de la mortalidad perinatal en nacidos con peso menor de 1000 gramos en el Hospital San Cecilio de Granada (España) durante el período 1991-2010Perinatal mortality analysis in newborns with weight under 1000 grams at San Cecilio´s Hospital (Granada, Spain) during the 1991-2010 period RESUMEN Introducción. La mortalidad perinatal ha disminuido sustancialmente en las últimas décadas. La prematuridad y el bajo peso al nacer son los factores predictivos más fuertemente asociados a esta mortalidad. El objetivo es analizar la evolución de la mortalidad perinatal en los nacidos con peso menor de 1000 g en los últimos 20 años (1991-2010) y sus causas. Población y métodos. Estudio observacional-descriptivo de tipo ecológico longitudinal, sobre 264 nacidos con peso menor de 1000 g de un total de 56 024 nacidos durante el período estudiado. Se calculan las diferentes tasas de mortalidad perinatal específicas por peso. Se aplica el coeficiente de correlación Rho de Spearman para evaluar la relación entre las tasas de mortalidad y los años de estudio, y las pruebas ANOVA y de MannWhitney para comparación de quinquenios y decenios, respectivamente. Resultados. Se han producido 131 muertes perinatales, 82 de ellas muertes fetales y 49 neonatales precoces. El 64,1% sucede antes de la semana de gestación 27. Sólo la tasa de mortalidad fetal presenta una disminución estadísticamente significativa, aunque la mortalidad perinatal presenta una tendencia al descenso, pero sin alcanzar la significación. Las principales causas inmediatas de óbito son la inmadurez extrema, la hipoxia intrauterina y la infección. Las causas fundamentales relacionadas con la muerte de este grupo de nacidos son la infección por rotura prematura de membranas, la hipertensión materna, la amenaza de parto pretérmino incontrolable y la gemelaridad. Conclusiones. La disminución de las tasas de mortalidad en este grupo de nacidos está sufriendo un enlentecimiento. Palabras clave: nacidos con peso extremadamente bajo, mortalidad perinatal.
RESUMENLa dehiscencia de la cúpula vaginal, con o sin evisceración, es una complicación rara de la cirugía ginecológica. El principal factor de riesgo es el antecedente de una histerectomía, a lo que se añaden otros factores desencadenantes, entre los que destaca la atrofia vaginal. El diagnóstico de evisceración es fácil, al comprobar salida de material intestinal a través de la vagina, siendo más dificultoso el diagnóstico de dehiscencia, ya que puede acontecer con síntomas banales. El tratamiento supone una urgencia quirúrgica al existir riesgo de lesión intestinal y peritonitis, pudiéndose realizar diferentes vías de abordaje teniendo en cuenta sobre todo la clínica de la paciente y tras comprobar el estado del contenido eviscerado. Presentamos el caso de una paciente con varios factores de riesgo, que presentó dehiscencia de la cúpula vaginal con evisceración de intestino de manera reiterada, y a la que se le practicó tratamiento quirúrgico por diferentes vías de abordaje así como tratamiento médico.PALABRAS CLAVE: Dehiscencia cúpula vaginal, evisceración intestinal, histerectomía previa, atrofia vaginal, urgencia quirúrgica SUMMARYDehiscence of the vaginal vault with or without evisceration is a rare complication after gynaecological surgery. The main risk factor is a previous hysterectomy, with the addition of other trigger conditions, the most important is vaginal atrophy. The diagnosis of evisceration is easy by confirmation the loosing of intestinal contents through the vagina. Diagnosis of dehiscence is more difficult because its symptoms could be more generic. The treatment is an emergency surgery as there is a risk of intestinal injury and peritonitis. There could be different surgical approaches taking into consideration the patient's clinic symptoms and after checking the state of eviscerated tissues. Seguridad del alta a las 48 horas en las mujeres intervenidas de cesarea. 30 RESUMENIntroducción: La estancia media de las mujeres a las que se les realiza una cesárea, ha disminuido drásticamente en los últimos decenios, actualmente más del 75% de las mujeres se dan de alta antes del cuarto día del postoperatorio. El objetivo de este trabajo es comparar el porcentaje de reingresos maternos, en las mujeres dadas de alta a las 48 horas de realizar la cesárea y en las dadas de alta posteriormente, ya que existe una considerable controversia en el tiempo de hospitalización recomendado después de la cesárea.Método: Estudio observacional-descriptivo de tipo ecológico longitudinal. Se valoran las cesáreas realizadas entre el 1 de enero del 2005 y el 31 de agosto del 2014. Se excluyen las mujeres con alta voluntaria, traslados a otro servicio y traslados a hospital de referencia. La población final está constituida por 2.031 mujeres. Se ha realizado un análisis descriptivo de los datos, un análisis de tendencia y un análisis bivariante realizando tablas de contingencia mediante test Chi Cuadrado para variables cualitativas y test de comparación de medias para muestras independientes mediante T de Student para variables cuantitativas.Resultados: El porcentaje de altas a las 48 horas y a las 72 horas o más ha sido respectivamente del 41% y del 59%.Entre el 2005-2011, las altas a las 48 horas fueron del 30% y entre el 2012-2014 fueron del 67% (Test de tendencia: E.S. p=0,000). El porcentaje de reingresos fue del 0,72% (alta a las 48 horas) y del 2,92% (alta a las 72 horas o más), diferencias estadísticamente significativas (Chi-cuadrado 12,04, p=0,0005) Las causas más importantes de reingreso fueron: sangrado genital (10), infección puerperal (9) y legrado (8), que constituyen el 70% de los reingresos.Conclusiones: En nuestro estudio se evidencia que el alta precoz (a las 48 horas de la intervención) en gestantes de bajo riesgo, que se ha realizado cesárea es como poco tan segura como el alta más tardía (72 horas o más), ya que hay menor número de reingresos en el primer grupo, diferencias que son estadísticamente significativas.
