MethodsMethods Methods Methods Methods: we retrospectively studied fifty-four cases of GIST, assessing the main prognostic factors of this neoplasis: risk levels, topography, size, mitotic index, necrosis, histological subtype and immunophenotype. We also verified their association and the reduction of overall survival. Results Results ResultsResults Results: Univariate analysis showed that tumors with mitoses number greater than 5 per 50CGA (high-power fields), the presence of necrosis and a high risk for both the systems proposed by Fletcher and Miettinen had a significant association with reduced survival (p = 0.00001, 0.0056, 0.03 and 0.009, respectively). The remaining analyzed factors (size, histological subtype, topography and immunophenotype) had no such association. Multivariate analysis (Jacard index) showed that the Miettinen degree of risk was the one that best correlated with prognosis. Conclusion Conclusion Conclusion Conclusion Conclusion: the risk criteria of Fletcher and Miettinen are important in assessing the prognosis of patients with gastrointestinal stromal tumors, especially the latter, which adds to the mitotic index and the presence of tumor necrosis. G astrointestinal stromal tumors (GIST) are the most common mesenchymal neoplasms of the gastrointestinal tract 1 , their incidence being estimated at 14 to 20 cases per million population 2 ; they are more frequent in male patients older than 50 years old 3 . The pathogenesis is related to mutational changes in two tyrosine kinase receptors: KIT and PDGFR-alpha (plateletderived growth factor receptor alpha) on the surface of the interstitial cells of Cajal, the former being the most common (85% of cases) 4,5 . Gastrointestinal stromal tumors can develop in any topography, from the esophagus to the rectum. However, they are more common in the stomach (50% to 60%), followed by small intestine (20% to 30%), colon (10%), rectum or esophagus (5%) 6 . Macroscopically, the tumor lesions often have a nodular form, transmural involvement and submucosal growth, with ulceration of the mucosa or not. In light microscopy, histology reveals three types: the most common spindle (70%), epidermoid (20%) and the mixed type (10%), when there is combination of epithelioid and spindle ones 7 . RESUMO: Objetivo: estudar os critérios morfológicos e imunoistoquímicos relacionados ao prognóstico dos tumores estromais gastrointestinais. Métodos: o estudo foi retrospectivo de 42 casos de tumor estromal gastrointestinal (GIST). Vinte e cinco casos foram obtidos no arquivo do Serviço de Anatomia Patológica do Hospital Universitário Gaffrée e Guinle e os outros dezessete, do Serviço de Anatomia Patológica do Hospital Universitário Clementino Fraga Filho. Resultados: de acordo com a análise univariada os tumores maiores que 5 cm, com número de mitoses maior que 5/50 CGA, presença de necrose, de alto risco, revelaram significância em relação a redução da sobrevida (p= 0,017, 0,010, 0,001 e 0,016, respectivamente). Os outros fatores analisados (subtipo histológico, topografia e imunofenótipo) não mostraram significância. Conclusão: os resultados confirmam a utilidade do grau de risco, do tamanho tumoral, do índice mitótico e da necrose como fatores preditores do comportamento biológico dos tumores estromais gastrointestinais (Rev. A maioria dos GIST resulta da ativação oncogênica do receptor Kit tirosina-quinase ou de seu homólogo, o receptor alfa do fator de crescimento derivado de plaqueta (PDGFRα) 3,4 . Este tumor tem sob o ponto de vista de sua histogênese a mesma linhagem das Células Intersticiais de Cajal 5,6 . No passado, este tipo de neoplasia mesenquimal era confundido com outras histologicamente semelhantes, como aquelas derivadas da musculatura lisa ou dos nervos autônomos do tubo digestório. No entanto, com o apuro do exame histopatológico a partir do advento dos marcadores imunohistoquímicos, tanto para o Kit (CD 117), o CD34, como para a actina músculo-específico, a vimentina, a proteína S-100, entre outros, tornou-se mais fácil dirimir as controvérsias quanto ao diagnóstico 7,8 . O GIST ocorre em praticamente todo o tubo digestório, da metade distal do esôfago até a região ano-retal, com uma incidência estimada em 10 a 20 casos/milhão de habitantes 8 . Contudo, a localização mais freqüente é no estôma-go com cerca de 60% dos casos, seguido pelo intestino delgado (30%), cólon e reto (5%) e esôfago (5%) 9 . É possível ainda que o GIST se desenvolva fora do trato gastrointestinal, como por exemplo no mesentério, no omento ou no retroperitônio, embora de forma muito menos freqüente 10,11 . Quando localizados no trato digestório variam de pequenos tumores subserosos até grandes massas com áreas sólido-císticas; à luz da microscopia podem apresentar três subtipos histológicos: os mais comuns de células fusiformes que representam aproximadamente 70% dos casos, os epitelióides (20%) e os mistos (10%) 12 . Introdução: os tumores estromais gastrointestinais possuem amplo espectro biológico, variando desde lesões de comportamento benigno até aquelas de caráter maligno, capazes de ampla disseminação e frequentes metástases viscerais. Atualmente, o prognóstico está baseado num escore, denominado grau de risco. Entretanto, este sistema apresenta falhas, com tumores classificados como de riscos intermediário e baixo associados ao desenvolvimento de metástases. Desta forma, são necessários estudos que visem ao aprimoramento desse sistema de classificação, destacando-se nesse campo, nos últimos anos, o índice de proliferação celular que tem mostrado valor prognóstico na predição da agressividade tumoral. Objetivos: analisar critérios morfológicos (tamanho macroscópico, topografia do tumor, índice mitótico, necrose, subtipo histológico), verificar o grau de risco e pesquisar a aplicabilidade dos marcadores imunohistoquímicos (actina músculo-específico, proteína S-100, Ki67 e p16ink4a) como fatores prognósticos do tumor estromal gastrointestinal (GIST). Resultados: a análise univariada mostrou significância com tumores maiores que 5 cm, número de mitoses maior que 5/50 CGA, presença de necrose, de grau de risco alto e índice de proliferação celular (Ki67) maior que 5% com relação à redução da sobrevida global dos pacientes (p = 0,017; 0,01; 0,001; 0,016; < 0,001 respectivamente). Os outros fatores analisados (subtipo histológico, imunofenótipo e p16 ink4a ) não mostraram significância. Conclusão: o grau de risco, o tamanho tumoral, o índice mitótico e a presença de necrose corroboram evidências prévias da sua utilização, como fatores morfológicos prognósticos e o emprego do índice de proliferação celular (Ki67) associado ao grau de risco para melhor esclarecimento do comportamento biológico dos GIST. abstract resumo Introduction: Gastrointestinal stromal tumors (GIST) have a wide biological spectrum, ranging from benign to malignant lesions, which are prone to wide spread and frequent visceral metastasis. Currently, the prognosis is based on a score system known as risk level. However, this system has some drawbacks. For instance, tumors classified as low or intermediate risk may be associated with the development of metastasis. Therefore, studies are required to improve this classification system and incorporate recent developments such as cellular proliferation index, which has shown prognostic value in the prediction of tumor aggressiveness. The AIDS pandemic has made diseases such as tuberculosis, CMV disease, and other opportunistic infections more prevalent; these diseases may even be found to be associated among themselves, and the natural history of each disease may present in an unusual manner. We report the case of a 41-year-old man with HIV (CD4 of 144 cells/dL) and HCV with hematochezia due to tuberculosis in the ileocecal valve and descending colon and CMV tissue invasive disease in the esophagus and descending colon. Coinfection among tuberculosis and cytomegalovirus in the gastrointestinal tract was described only once in a patient with a recent diagnosis of HIV that affected the distal ileum and ascending colon. We will discuss the peculiarities of the case and the behavior of the immune system in the face of simultaneous opportunistic infections. This is a challenging scenario that has scarce publications and is of great clinical importance.
