Introdução:O teste de toxicidade in vivo, conhecido como Teste de Toxicidade Inespecífica (TTI), é sugerido para avaliar a segurança de produtos biológicos e derivados de biotecnologia. O princípio do teste é a administração do produto em animais, seguido por um período de observação. O Instituto Nacional de Controle de Qualidade em Saúde (INCQS) é o laboratório de controle nacional brasileiro (LCN) que controla os produtos biológicos, especialmente vacinas e soros hiperimunes, para o Programa Nacional de Imunização (PNI) antes da liberação para o mercado. São preconizados ensaios químicos, microbiológicos e toxicológicos, inclusive o TTI. Objetivo:Analisar retrospectivamente o TTI realizado no INCQS e verificar as monografias da Farmacopeia Brasileiraonde ainda é preconizado o teste. Método:Foi realizado um levantamento no sistema de dados Harpya do INCQS, para verificar os resultados satisfatórios e insatisfatórios do TTI. Resultados:No período de 1999 a 2012, foram realizados 3.453 TTI nos produtos biológicos, resultando em 100% de resultados satisfatórios. Com base nestes resultados e seguindo a tendência mundial, o INCQS vem desde 2002 reduzindo gradualmente a quantidade de produtos biológicos submetidos ao TTI. Atualmente, o risco de contaminação de produtos farmacêuticos com substâncias não específicas é significativamente baixo, pois as indústrias são reguladas para o cumprimento das boas práticas de fabricação (BPF). Embora o TTI ainda permaneça nos Testes Gerais da Farmacopeia Brasileira, ele não é requerido nas monografias específicas de soros hiperimunes e vacinas para uso humano. Conclusões: O TTI não parece ser eficiente para detectar toxicidade inespecífica em biológicos e o Brasil deve seguir a tendência mundial de interromper a sua execução.
Introduction: The COVID-19 vaccines in use (inactivaded virus, encapsulated m-RNA, non-replicating adenovirus-vectored DNA) were clinically tested in randomized placebocontrolled phase-3 studies. Objective: To address certainties and uncertainties about safety and effectiveness of COVID-19 vaccines that were approved for use in various countries. Method: The evidence provided by clinical studies on the efficacy and safety of COVID-19 vaccines was critically appraised. Results: COVID-19 vaccines proved to be efficacious and safe in clinical trials. Adverse events were mostly those of minor severity commonly noted with other vaccines such as injection site pain, mild flu-like symptoms, headache and asthenia. Although being very rare, anaphylaxis-like reactions were noted with mRNA vaccines. Uncertainties regarding vaccine effectiveness refer mainly to the (long-term) duration of immunity provided by vaccination, the degree of protection conferred to elderly people, and how effective vaccines are against emerging SARS-CoV-2 variants. There are few uncertainties about vaccine safety including the absence of clinical trial data in pregnant women (and the impact on the unborn child), children and adolescents. Conclusions: Notwithstanding the knowledge gaps about effectiveness and safety of COVID-19 vaccines (to be further addressed by observational studies), there is overwhelming evidence that public health benefits of vaccination by far outweigh any foreseeable risk.
Introdução: O teste de ganho de peso em camundongos (TGPC) para a determinação da toxicidade da vacina pertussis de células inteiras (VPCI) é essencial para aprovar o emprego da vacina adsorvida contra a difteria, o tétano e a pertussis (DTP) nos programas de vacinação infantil. Objetivo: Avaliar a influência do sexo e das linhagens de camundongo (NIH e BALB/c An) no TGPC da VPCI de referência, da toxina pertussis (TP) e de 36 amostras de vacinas DTP. Método: Dez animais de cada sexo e linhagem foram pesados e, em seguida, inoculados intraperitonealmente com 0,5 mL/camundongo da VPCI de referência, de TP (0,25-2,0 μg/animal) ou de vacinas DTP. Grupos-controle para VPCI e TP foram inoculados com PBS e para vacinas DTP empregou-se o cloreto de sódio 0,9% com 100 ppm de timerosal. O teste é satisfatório se não ocorrerem mortes, se o ganho de peso médio (GPM) dos animais inoculados no 3° dia for superior ao inicial e no 7° dia o GPM for igual ou maior que 60% do controle. Os resultados do TGPC foram avaliados pelos critérios em vigência e pelo proposto no 7° dia, no qual o limite inferior do intervalo de confiança de 95% do GPM seja igual ou maior que 60% do controle. Resultados: A VPCI de referência foi satisfatória em fêmeas NIH e BALB/cAn e insatisfatória em machos de ambas as linhagens considerando-se ambos os critérios. O TGPC em ambas as linhagens e sexos revelou baixa sensibilidade na detecção do efeito da TP isolada (0,25-2,0μg/camundongo). As vacinas DTP foram satisfatórias em camundongos NIH de ambos os sexos e em fêmeas BALB/cAn, porém o resultado foi insatisfatório para 14,0% das vacinas DTP em machos BALB/cAn por ambos os critérios. Conclusões: O TGPC de vacinas DTP em camundongos fêmeas de ambas as linhagens deve ser considerado, pois evidenciou-se que fêmeas NIH e BALB/cAn são adequadas para a realização do TGPC. A adoção do critério proposto no 7° dia deve ser estimulado por aumentar a sensibilidade e precisão do TGPC.
A manipulação de medicamentos oncológicos injetáveis é uma atividade do segmento farmacêutico de grande relevância e complexidade. Requer ações de biossegurança para minimizar a contaminação ambiental e ocupacional e condições ambientais associadas à técnica asséptica para manutenção da esterilidade. São preparações extemporâneas, isentas de teste de esterilidade, sendo, portanto, necessário um controle rigoroso do processo de preparo. O monitoramento ambiental, exigência da normativa nacional de boas práticas de manipulação, é uma ferramenta utilizada para demonstrar que o ambiente de produção atende às exigências de qualidade. No entanto, a normativa não estabelece a forma de realizá-lo e nem o padrão de conformidade aceitável. Essa ausência de informação propicia a realização de ensaios sem padrão de referência, podendo o produto final ficar aquém das exigências quanto à segurança, integridade e confiabilidade. Nesse sentido, esse trabalho traz à luz as exigências das normativas internacionais quanto ao monitoramento ambiental na manipulação de medicamentos injetáveis traçando um contraponto com as principais normas e guidelines industriais nacionaise internacionais. Embora os produtos industrializados e manipulados por processo asséptico tenham que manter a mesma característica de esterilidade, observam-se divergências de métodos e limites aceitáveis de contaminação, questionando-se se é possível a flexibilização quanto as exigências de qualidade. Ressalta-se ainda a necessidade de a agência regulatória brasileira atualizar a normativa voltada para farmácia de manipulação de injetáveis, para que auxilie na efetiva implantação de um Programa de Monitoramento Ambiental de forma a contribuir para o fortalecimento do Sistema de Gestão da Qualidade em Serviços de Saúde.
