RESUMOO presente estudo de revisão visa uniformizar a nomenclatura e os conceitos classificatórios existentes na literatura, relativos aos desenhos ou modelos de estudo científico mais comumente utilizados em pesquisas na área clínico-cirúrgica geral. Procurou-se facilitar a elaboração do delineamento de um projeto de pesquisa científica, visando sua aplicabilidade. Esta revisão também tem o intuito de auxiliar o cirurgião-pesquisador a identificar qual foi o delineamento proposto em um artigo, assim como desenvolver um senso de crítica metodológica mais apurada sobre o mesmo, além de desmistificar a miscelânea conceitual existente na literatura. ABSTRACTThe purpose of this study is to analyze the classifications and concepts found at the literature regarding the research models on scientific studies commonly used in medical articles. This paper provides elements to facilitate the creation of a research model. It also helps the researcher to identify which is the research model of an article. Epidemiologic Research Design. Clinical Trials. Cohort Studies. IntroduçãoO ramo da Medicina que trata da metodologia das pesquisas é conhecido como Epidemiologia Clínica. Esse ramo, considerado uma área básica, constitui-se num importante alicerce no qual a Medicina moderna é praticada. Essa denominação é oriunda de duas disciplinas médicas: Epidemiologia e Medicina Clínica (ou Geral). A contribuição da Epidemiologia é representada pelo fato que vários dos métodos utilizados, para responder as questões das pesquisas, foram desenvolvidos por epidemiologistas, e pelo cuidado individual dos pacientes ser considerado num contexto da população maior, da qual o paciente faz apenas parte. A contribuição da Medicina Clínica reside na procura em responder a questões clínicas, assim como conduzir decisões clínicas baseadas nas melhores evidências disponíveis. 1 Porém, um fator considerado essencial ao progresso da pesquisa médica veio da área da Matemática. A Bioestatística introduziu a análise estatística para a confirmação de achados, assim como para a avaliação de tratamentos médicos, fornecendo-lhes maior solidez, confiabilidade e credibilidade.
The main purpose of a study is its publication on a scientific journal. Research financing agencies are important institutions so that studies can be developed and published. The most important research financing agencies that are discussed in this article are: "Coordenação de Aperfeiçoamento de Pessoal de Nível Superior" (CAPES), "Conselho Nacional de Desenvolvimento Científico e Tecnológico" (CNPq) and "Fundação de Amparo à Pesquisa do Estado de São Paulo" (FAPESP). CAPES' activities can be grouped in four different strategy lines: a) it evaluates the stricto sensu, at the post-graduation level; b) it provides access and development of scientific research; c) it provides investment on the development of high qualified human resources in Brazil and abroad, and d) it promotes international scientific cooperation. Although CAPES does not support directly scientific publications, almost all actions of this agency contribute to the development of scientific research and publication. CNPq has two main purposes: financing researches and development of human resources. It provides the researchers with financial aid to scientific publication. The grants for editing were specifically created for supporting the national scientific and technical publications edited by Brazilians institutions or societies. CNPq can also support Congresses, Symposiums and similar short-term courses. The Plataforma Lattes is also a branch of CNPq on which the Curriculum Lattes is available. This site has the curriculum vitae of the scientific community and is of great value for researchers. FAPESP also finances journal publications, articles and books that bring up original results of studies made by researchers from the state of São Paulo. It finances, partially, the travel expenses of innovative papers authors in meetings within the country or abroad. Brazilian authors are increasing the number of international publications. Universities, research institutes, financing agencies and private companies are more and more concerned with knowledge property. Researchers must understand the need of knowledge property and the financing agencies have to consider the patents achieved as a criteria of evaluation of scientific production.
Objective To compare the level of vitamin D3 in cutaneous melanoma patients, with or without disease activity, with reference values and with patients from a general hospital.Methods The serum levels of vitamin D3 were measured in cutaneous melanoma patients, aged 20 to 88 years, both genders, from January 2010 to December 2013. The samples from the general group were processed at Hospital Israelita Albert Einstein (control group). Data analysis was performed using the Statistics software.Results A total of 100 patients were studied, 54 of them men, with mean age of 54.67 years, and 95 Caucasian. Out of these 100 patients, 17 had active disease. The average levels of vitamin D3 in the melanoma patients were lower than the level considered sufficient, but above the average of the control group. Both groups (with or without active disease) of patients showed a similar distribution of vitamin D3 deficiency.Conclusion Vitamin D3 levels in melanoma patients were higher than those of general patients and lower than the reference level. If the reference values are appropriate, a large part of the population had insufficient levels of vitamin D, including those with melanoma, or else, this standard needs to be reevaluated. No difference in vitamin D3 levels was found among melanoma patients with or without active disease. More comprehensive research is needed to assess the relation between vitamin D and melanoma.
Cutaneous melanoma is the solid neoplasia with the highest growing incidence among all tumors. It spreads predictably to the lymphatic vessels and sentinel lymph node, and when the latter is affected the prognosis worsens dramatically. Sentinel lymph node biopsy is considered when thickness of the primary tumor exceeds 1mm and/or when there are adverse features in thinner melanomas. When there is nodal metastasis, current evidence in the literature recommends complete lymphadenectomy, although this procedure has its intrinsic risks (i.e., lymphedema and cellulitis), and there are no published clinical trials proving additional overall survival benefits. The current in-depth literature review thus aims to identify patients that will benefit most from the procedure, including those with the highest likelihood of presenting additional affected lymph nodes in the same nodal basin. The authors also discuss techniques for identification of the sentinel lymph node, false-negative rates, and predictive models for lymph node involvement. In conclusion, complete elective lymphadenectomy should always be discussed on a case-by-case basis when metastases are detected in the sentinel lymph node.
The results are suggesting that particle size and symmetry may interfere with LN radiopharmaceutical uptake.
