Desde los primeros meses de la pandemia de Covid-19 en México, personal de salud denunció ante los medios de comunicación diversos problemas en la gestión de las autoridades hospitalarias y sanitarias, que afectaban a la adecuación y calidad de la atención proporcionada y a su percepción de riesgo ante una potencial infección nosocomial. A partir de entrevistas semiestructuradas realizadas a médicas y médicos de hospitales “Covid” e “híbridos” de la Ciudad de México, en este artículo se propone la categoría cultura organizacional epidémica de atención en salud, con el objetivo de analizar cómo se produce esta nueva reorganización de la atención hospitalaria desde las representaciones sociales y prácticas de estos actores sociales. Para ello se analizan sus representaciones respecto a ciertas dimensiones que consideran significativas en la gestión de la pandemia, a nivel gubernamental y hospitalario, y del comportamiento de la población; así como sus prácticas autoorganizadas para enfrentar la ausencia de protocolos y de medios materiales para una atención médica adecuada, y mitigar el riesgo de infecciones hospitalarias. Recuperar el punto de vista y las prácticas de los actores implicados en la atención médica, permite visibilizar las contradicciones del sistema de salud y mejorar las respuestas, actuales y futuras, ante emergencias sanitarias.
The paper analyzes, from a qualitative perspective, how the COVID-19 pandemic has significantly reduced medical care for chronic patients at the three levels of the Mexico City health sector due to community mitigation strategies, the perceived risk of infection of health personnel and patients, the focus on pandemic care, and how these processes are articulated with the previous underfunding conditions of the public health system. Thus, we adopted the Grounded Theory approach based on the description and analysis of the experiences of 42 doctors interviewed at the three care levels. The pandemic iatrogenesis concept is proposed to address the interrelation between the material and symbolic organization of the dominant model of medical care and specific structural and cultural dimensions of massive and systemic neglect since the arrival of the pandemic. We point out some strategies and possibilities to avoid similar future settings based on this theoretical proposal and the experiences of the doctors interviewed.
Este artigo analisa o processo alimentar e a nutrição infantil, especificamente amamentação e ablação, como formas de estudar as desigualdades de gênero. O estudo foi realizado entre famílias zoque em um município com alta prevalência de desnutrição infantil no sudeste do México. O método etnográfico e as técnicas da entrevista em profundidade e da observação participante foram utilizadas para recuperar o ponto de vista dos pais e mães de 14 crianças zoqueis. Os resultados mostram que a desigualdade no acesso à alimentação sofrida por mulheres, meninas e meninos por razões econômicas, culturais e de gênero afeta o estado nutricional e terá repercussões a longo prazo, por isso é urgente elaborar programas ou iniciativas com uma perspectiva de gênero que permite que a sociedade zoque reconheça os direitos das mulheres e melhore seu status.
Desde la perspectiva de la epidemiología sociocultural, se analiza cómo en Ocotepec, Chiapas, los saberes maternos sobre las enfermedades diarreicas y las infecciones respiratorias agudas, estudiados a partir de las representaciones sociales y prácticas de atención, inciden en la búsqueda de atención apropiada. Los resultados demuestran que no hay un reconocimiento de los signos de alarma de deshidratación y neumonía, las complicaciones más graves, lo que causa defunciones en niños menores de cinco años que pudieron evitarse con tratamientos adecuados, que no fueron accesibles debido a los procesos de medicalización y mercantilización de las prácticas médicas locales, y la falta de acceso a los servicios públicos de salud. demonstrate that the "danger signs" of the most severe threats of those illnesses: dehydration and pneumonia, are not recognized. This problem leads to children deaths due to the lack of use of appropriate treatment, that was not reachable because of the processes of medicalization and commodification of the local medical practice, and the lack of access to public medical attention.
