Nous rapportons un cas de cardiomyopathie dilatée d'origine hypocalcémique secondaire à une hypoparathyroïdie, chez une femme de 38 ans. Sa calcémie était effondrée à 30 mg/L et l'échocardiographie avait montré une cardiomyopathie dilatée hypokinétique avec baisse de la fraction d'éjection du ventricule gauche à 31,4%. Elle a reçu une calcithérapie avec de la vitamine D3 et l'évolution était marquée par la normalisation de la taille des cavités cardiaques et de la fraction d'éjection ventriculaire gauche (FEVG) après normocalcémie.
Aims: To establish the epidemiological and clinical aspects of atrioventricular block (AVB). Patients and method: We have realised a descriptive retrospective study during 32 months from 1 January 2013 to 31 August 2015. Results: The overall prevalence of AVB was 2.43%. The average age of our patients was 61.38 years old with extremes of 17 and 89. Men and the elder were the most affected. Hypertension was the most frequent cardiovascular risk factors. The circumstance of discovery was dominated by dyspnea. Bradycardia only affected 35% of patients. Signs of heart failure were present in 57% of cases. The majority of patients had a secondary AVB due to a hypertensive heart disease. Conclusion: Atrioventricular block is a reality in cardiology practice in Madagascar. Taking care of hypertension could reduce its prevalence. Indeed, prevention should be intensified due to the cost of the pacemaker, still expensive for the majority of Malagasy people. Méthode: Nous avons effectué une étude rétrospective descriptive portant sur une période de 32 mois allant du 1 er Janvier 2013 au 31 Août 2015. Résultats : La prévalence globale des BAV était de 2,43 %. L'âge moyen de nos patients était de 61,38 ans avec des extrêmes de 17 et 89 ans. Les hommes et les sujets âgés étaient les plus touchés. L'hypertension artérielle (HTA) constituait le facteur de risque cardio-vasculaire le plus fréquent. La circonstance de découverte était dominée par la dyspnée. La bradycardie n'avait concerné que 35% des patients. Les signes d'insuffisance cardiaque étaient présents dans 57 % des cas. La majorité des patients avaient un BAV secondaire à une cardiopathie sous-jacente. Conclusion:Le bloc auriculo-ventriculaire constitue une réalité dans la pratique médicale cardiologique à Madagascar. La prise en charge correcte de l'HTA pourrait réduire sa prévalence. En effet, la prévention devrait être intensifiée vue le coût du pacemaker, encore exorbitant pour la majorité de la population Malagasy.
Introduction: Chronic renal disease is defined as lasting impairment lasting more than 3 months of the function or renal structure that has an impact on health. The challenge is to avoid progression to chronic end stage renal failure. Our study aims to identify the factors associated with the progression of chronic kidney disease. Methods: We performed a retrospective, analytical study from the Befelatanana Nephrology Department from January 2017 to December 2017. Patients were included if they had deteriorating renal function. Results: The median age of patients with impaired renal function was 52.5 years and men were 85. Patients with a history of diabetes or urinary tract infection, hypertensive patients on admission, and patients with under furosemide were among the most exposed according to the final model of multivariate analysis. Conclusion: A strategy to combat the progression of chronic kidney disease involves the prevention and optimal management of risk factors that are significantly related to this renal worsening, in particular, a well-used and well-monitored use of furosemide. Resume : Introduction : La maladie rénale chronique se définit par une altération durable plus de 3 mois de la fonction ou de la structure rénale ayant une impact sur la santé. Le défi consiste à éviter l'évolution vers une insuffisance rénale chronique terminale. Notre étude a pour objectif d'identifier les facteurs associés à la progression de la maladie rénale chronique. Méthodes : Nous avons effectué au service de Néphrologie de Befelatanana une étude rétrospective, analytique allant du mois de janvier 2017 au mois de décembre 2017. Les patients étaient inclus si ayant une détérioration de la fonction rénale. Résultats : L'âge médian des patients avec détérioration de la fonction rénale était de 52,5 ans et les hommes étaient au nombre de 85. Les patients avec antécédent de diabète ou d'infection urinaire, les patients hypertendus à l'admission et les patients mis sous furosémide figuraient parmi les plus exposés selon le modèle final de l'analyse multivariée. Conclusion : Une stratégie de lutte contre la progression de la maladie rénale chronique fait appel à la prévention et la prise en charge optimale des facteurs de risque lié significativement à cette aggravation rénale, en particulier, une utilisation à bon escient et bien surveillée de furosémide.
Aims: Describe the lipid profile of patients with chronic kidney disease and to assess the role played by dyslipidemia in the kidney disease progression. Patients and method: It was a retrospective descriptive and analytical study carried out at the Nephrology Unit of the University Hospital Befelatanana Antananarivo, from may 2008 to february 2015. We included in the study patients diagnosed as carriers of chronic kidney disease with abnormal lipid status. Patients who received henodialysis treatment were excluded. Results: Ninety nine cases were selected, so as 48 men and 51 women. Their ages ranged from 18 to 87 years-old, with a mean of 50,47 years-old. Arterial hypertension was the most important comorbidity factor found, followed by age (41,41%) and smoking (40,40%). Patients with CKD stage 5 was the majority (54,55%). Lipid profile was mainly compounded by Low HDL cholesterolemia (44,44%). Hypertriglyceridemia was only found in 10,10% of cases. In univariate analysis, only a low HDL cholesterolemia was associated significantly with cardiovascular event (p = 0,04). We found significant association between dyslipidemia and glomerular filtration rate in adjusted analysis with proteinuria (p < 0,05). Conclusion: Our study demonstrated that screening and treating abnormal cholesterolemia is necessary in patients with chronic kidney disease. Large-scale prospective studies, assessing the impact of treating dyslipidemia in kidney disease should be carried out. (Résumé: Buts: Décrire le profil lipidique des patients avec maladie rénale chronique et évaluer le rôle de la dyslipidémie dans la progression de la maladie rénale. Méthode: II s'agissait d'une étude retrospective, descriptive et analytique réalisée dans le service de Néphrologie du Centre Hospitalier Universitaire Befelatanana Antananarivo du mois de mai 2008 à février 2015. Nous avons inclus dans l'étude les patients diagnostiqués comme porteurs de maladie rénale chronique avec un bilan lipidique anormal. Les patients ayant bénéficié d'un traitement de suppléance ont été exclus. Résultats: Quatre vingt dix neuf cas ont été retenus dont 48 hommes et 51 femmes. L'âge variait de 18 à 87 ans, avec une moyenne de 50,47 ans. L'hypertension artérielle était le facteur de comorbidité le plus important (79%), suivi du tabagisme (40%). Les patients au stade 5 de la maladie rénale chronique étaient majoritaires (54,55%). Les anomalies lipidiques étaient prédominées par l'hypoHDL (44.44%). L'hypertriglycéridémie n'a été retrouvée que dans 10.10% de cas. Ajustée à la protéinurie, les paramètres lipidiques à l'exception de la triglycéridémie étaient associés significativement à une baisse de la DFG (p < 0,05). Conclusion: Notre étude a démontré l'importance du traitement de l'anomalie de la cholestérolémie chez les malades rénaux chroniques. Des études prospectives sur l'impact du traitement de la dyslipidémie dans la maladie rénale devraient être effectuées dans une large population.
Anomalous left coronary artery from pulmonary artery ALCAPA, is a rare congenital heart disease. It affects one child in 300,000 births. It causes myocardial ischemia leading to impaired left ventricle systolic function and signs of left heart failure. Several authors have already reported cases of ALCAPA. We are reporting for the first time a case seen in Madagascar. This is a Franco-Malagasy child aged 4 months and 3 weeks, hospitalized at the Mother and Child University Hospital of Tsaralalana for progressively worsening dyspnea. At entry, the infant presented with cyanosis and signs of cardiogenic shock: hypotension, oligoanuria and cold extremities. The physical examination essentially found pulmonary rales, a 2/6 systolic murmur of mitral insufficiency and hepatomegaly. Chest X-ray showed significant cardiomegaly. The electrocardiogram found a Q wave of necrosis in DI, AVL and V2 to V6. The echocardiogram coupled with Doppler performed at the Soavinandriana Hospital concluded that the left coronary artery was connected to the pulmonary artery. The progression after 6 days in hospital was marked by the disappearance of signs of shock and dyspnea after treatment with Dobutamine, diuretics and Salbutamol. The child was proposed for a surgical cure, the gold standard of treatment for ALCAPA. Resume: La naissance anormale du tronc coronaire gauche à partir de l'artère pulmonaire ou ALCAPA, de l'anglais anomalous left coronary artery from pulmonary artery ou encore syndrome de Bland-White-Garland, est une pathologie congénitale rare. Elle affecte un enfant sur 300 000 naissances. Elle entraine une ischémie myocardique à l'origine d'une altération de la fonction systolique du ventricule gauche et des signes d'insuffisance cardiaque gauche. Plusieurs auteurs ont déjà rapporté les cas de AL-CAPA. Nous rapportons pour la première fois un cas vu à Madagascar. Il s'agit d'un nourrisson Franco-Malgache de 4 mois et 3 semaines, hospitalisé au Centre Hospitalier Universitaire Mère-Enfant de Tsaralalana pour une dyspnée d'aggravation progressive. A l'entrée, le nourrisson présentait une altération de l'état général, cyanose et des signes de choc cardiogénique : hypotension artérielle, oligoanurie et extrémités froides. L'examen physique retrouvait essentiellement des râles crépitants diffus, un souffle systolique 2/6 d'insuffisance mitrale et une hépatomégalie. La radiographie du thorax de face montrait une cardiomégalie importante. L'électrocardiogramme a retrouvé une onde Q de nécrose en DI, AVL et de V2 jusqu'en V6. L'échocardiogramme couplé au Doppler réalisé au Centre Hospitalier de Soavinandriana a conclu à l'anomalie de connexion du tronc de l'artère coronaire gauche à partir de l'artère pulmonaire. L'évolution après 6 jours d'hospitalisation a été marquée par la disparition des signes de choc et de la dyspnée après un traitement par Dobutamine, diurétiques et Salbutamol. L'enfant a été proposé pour une cure chirurgicale, le gold standard du traitement de l'ALCAPA.
