Radiation-induced hemorrhagic gastritis is a serious and rare complication of radiation therapy. Optimal therapies in the pediatric population are not well established. We report a 2-year-old female diagnosed with rhabdomyosarcoma who developed hemorrhagic gastritis following chemotherapy and radiation therapy. The patient presented with acute onset anemia, hematemesis, and melena. Endoscopies revealed circumferential ulceration at the pylorus with spontaneous oozing that failed to respond effectively with multimodal medical and endoscopic therapies. Following hemodynamic stabilization, the patient was treated with hyperbaric oxygen therapy with excellent clinical response of the bleeding. Further research on the benefit of hyperbaric oxygen therapy is warranted to determine if this treatment can reduce the incidence of gastrointestinal complications in patients who have received radiation therapy.
Исследование было проведено у 60 больных в остром периоде ишемического инсульта. Включение мексидола в ком плекс интенсивной терапии у больных с ишемическим инсультом способствует купированию локального ДВС синд рома и профилактирует вторичное повреждение головного мозга и генерализацию системной воспалительной реак ции. Темп снижения содержания растворимых фибрин мономерных комплексов и фибриногена в крови был выше, чем у больных не леченных мексидолом. К 7 суткам уровень растворимых фибрин мономерных комплексов у выжив ших больных снижался в среднем до 8±2,7 мг%, уровень фибриногена снижался в среднем до 2-4 г/л. Мексидол оп тимизирует параметры мозгового кровотока, улучшает венозный отток из полости черепа и может быть рекомендован для включения в схему интенсивной терапии у больных с сосудистыми заболеваниями головного мозга.The study was conducted in 60 patients with acute ischemic attack. Mexidole included into a complex of intensive care in patients with stroke promoted the abolishment of local disseminated intravascular coagulation and prevented sec ondary cerebral lesion and the generalization of a systemic inflammatory reaction. The rate of a reduction in the blood levels of soluble fibrin monomer complexes and fibrinogen was higher than that in mexidole untreated patients. By day 7, the levels of soluble fibrin monomer complexes and fibrinogen in survivors decreased on average to 18.0±2.7 mg% and to 2-4 g/l, respectively. Mexidole optimizes the parameters of cerebral blood flow, improves venous outflow from the cranial cavity, and may be recommended for its inclusion into the intensive treatment regimen in patients with cere brovascular diseases.Инсульты становятся все более значимой при чиной заболеваемости и смертности. К сожалению, далеко не всегда это так. Высокий уровень летальности подтверждает это.Дискутируемым направлением интенсивной терапии является использование антиоксидантов. Известна точка зрения, что препараты этой груп пы не доказали своей эффективности в рандоми зированных проспективных исследованиях.
