Purpose of the study. To evaluate the effectiveness of unilateral spinal anesthesia as a method of pain relief during foot surgery. Analyze the effectiveness of unilateral spinal anesthesia versus sciatic nerve block. To develop an algorithm for performing unilateral spinal anesthesia during foot surgery. Materials and methods. We examined 64 patients who were treated in the department of purulent surgery of the 3rd city hospital in Zaporozhye with a diagnosis of diabetic foot syndrome in the period from 2017 to 2019. Research methods. Assessment of the level of pain intensity using the standard visual analogue scale and the concentration of glucose in the blood serum. Monitoring of the parameters of the cardiovascular system, the temperature of the skin of the lower leg during anesthesia, Doppler scanning of the peripheral circulation in the foot. Results. 4 hours after performing anesthesia, the intensity of pain during movement of the lower limb, according to the visual analogue scale in the group with unilateral spinal anesthesia was 2,3 times less than in the group with sciatic nerve block (p < 0,05). After 10 hours – in the sciatic nerve block group there was a 9,4% increase in comparison with the unilateral spinal anesthesia group (p < 0,05). Compared with the unilateral spinal anesthesia group, in the sciatic nerve block group 20 minutes after anesthesia, the heart rate decreased by 7,2% (p < 0,05). After unilateral spinal anesthesia, the level of systolic blood pressure on 1 toe of the foot increased by 18,3% 20 minutes after anesthesia, and 20 minutes after sciatic nerve block – by 13,7% (p < 0,05). In patients with the unilateral spinal anesthesia group, the temperature on the operated lower limb after anesthesia increased by 3,8%, in the sciatic nerve block group – by 2,8% (p < 0,05). Conclusions. Regional anesthesia is the main method of choice for foot surgery compared to general anesthesia. sciatic nerve block does not in all cases lead to 100% surgical anesthesia and requires additional medical analgosedation. For foot surgery, unilateral spinal anesthesia is preferred.
The article presents the theoretical justification and comparative experience in the formation of a unilateral spinal block by tachyphylaxis in urgent surgery during operations on the lower extremities. The possibility of influencing the duration and severity of motor blockade on the operated side was established using the tachyphylaxis method for administering the anesthetic without decreasing the quality of anesthesia as a whole and without increasing the amount of anesthetic administered. The possibility of reducing the development time of full anesthesia to a minimum fixation time of anesthetic on nerve tissue (10 minutes) due to preliminary «sensitization» to nerve fiber anesthetics was established. The possibility of reducing the need for postoperative analgesia by increasing the duration of the postoperative painless period has been established. ДЗ «Запорізька медична академія післядипломної освіти МОЗ України», м. Запоріжжя, Україна стрес-реакція на операційну травму у хворих із синдромом діабетичної стопи, оперованих в умовах регіонарної анестезії Резюме. Актуальність. У нашій роботі ми проводимо порівняння таких видів регіонарної анестезії, як одностороння (унілатеральна) спінальна анестезія, традиційна спінальна анестезія і блокада сідничного нерва на стороні проведення оперативного втручання, і аналіз стрес-реакції організму хворого на операційну травму при хірургічному лікуванні хворих із синдромом діабетичної стопи. Мета: порівняти ефективність, надійність і безпеку таких методів регіонарної анестезії, як одностороння й традиційна спінальна анестезія, а також блокада сідничного нерва на стороні хірургічного втручання, на підставі аналізу рівня катехоламінів у добовій порції сечі пацієнта й показників вмісту глюкози в крові. Матеріали та методи. Було обстежено 96 пацієнтів з діагнозом «цукровий діабет, синдром діабетичної стопи». Пацієнти були розподілені на 3 групи (по 32 пацієнти в кожній) залежно від методу знеболювання: 1-ша група-хворі, прооперовані в умовах односторонньої спінальної анестезії; 2-га група-хворі, прооперовані в умовах традиційної спінальної анестезії; 3-тя група-хворі, прооперовані в умовах блокади сідничного нерва. У кожній групі було по 50 % хворих чоловічої та жіночої статі. Концентрацію глюкози в сироватці крові визначали глюкозооксидазним методом до анестезії (на операційному столі), у першу й третю післяопераційну добу. Рівень катехоламінів у сечі визначався в першу післяопераційну добу. Результати. Встановлено, що майбутнє хірургічне втручання, сама операція і післяопераційний період викликають ендокринну відповідь, яка характеризується підвищенням вмісту в крові гормонів стресу, перш за все кортизолу й адреналіну. Підвищення рівня глюкози в крові є важливою ознакою стресу. Стабільні цифри рівня глюкози в крові свідчать про сприятливий перебіг періопераційного періоду у хворих, що перенесли операцію на стопі в умовах регіонарної анестезії. Висновки. Основне завдання анестезіологічного супроводу операціїадекватне знеболювання з відсутністю стрес-реакції організму хворого на операційну травму. При використанні регіонарної анестезії вміст у крові гормонів, що визначають стресову відповідь організму на хірургічну агресію, значно нижче, ніж при загальній анестезії.
Introduction: In the present article we draw a comparison between such types of regional anesthesia (RA) as unilateral spinal anesthesia (USA), traditional spinal anesthesia (SA) and block anesthesia of peripheral nerve trunk (block anesthesia of sciatic nerve (BASN) on the side of operative intervention) during surgical treatment of patients with the syndrome of diabetic foot. The aim of the article is to compare effectiveness, reliability and safety of such methods of RA as unilateral and traditional spinal anesthesia, as well as block anesthesia of sciatic nerve on the side of operative intervention. Materials and methods: 96 patients with the syndrome of diabetic foot, who received treatment in the department of purulent surgery, were examined. All patients underwent operative interventions of varying degrees of complexity depending on the nature and seriousness of damage degree of extremities, in terms of RA types under investigation. Effectiveness of regional block anesthesia, the time when anesthesia takes effect, duration and deepness were evaluated. Results: It should be noted that USA, as a variant of pain management during operations of foot, provides the most adequate level of surgical pain management in comparison with the other types of anesthesia. During block anesthesia of sciatic nerve more than half of patients are not provided with 100% surgical analgesia, which can lead to additional medicamentous analgosedation. According to the results obtained, pain sensitivity in patients after BASN is reliably higher than in patients after USA and SA in all stages of the study. Conclusions: RA is the main method of choice during operations on foot in comparison with the methods of general anesthesia. Unilateral spinal anesthesia is carried out with the use of smaller amount of anesthetic than traditional spinal anesthesia and block anesthesia of peripheral nerve trunk, which significantly reduces sympathectomy area and leads to the stable hemodynamic during the peri-operative period.
