La platibasia (impresión basilar, invanginación basilar) es una deformación del hueso occipital y de la parte alta de la columna cervical. De acuerdo con Ray 1 , el proceso consiste fundamentalmente en el desplazamiento del agujero occipital hacia el interior de la cavidad craneana y en la luxación de las vértebras cervicales superiores hacia arriba y adelante. A pesar de esta definición, la deformación presenta numerosas variaciones en su forma y grado. El tamaño, la forma, el contorno y la situación del agujero occipital y del canal cervical superior están alterados. La capacidad de la fosa posterior está disminuida. La afección es congénita, aunque se han apresentado algunas observaciones de platibasia adquirida como resultado de un proceso patológico de la región. En cierto nú-mero de enfermos no produce manifestaciones nerviosas en ninguna época de la vida; en otros, se asocian trastornos congénitos del sistema nervioso o manifestaciones neurológicas progresivas que pueden simular la siringomielia o la siringobulbia, la esclerosis en placa, la esclerosis lateral amiotrófica, la parálisis espástica progressiva, los tumores de fosa posterior o craniospinales, el síndrome de degeneración espinocerebelosa e la hidrocefalia simples progresiva. La observación de dos enfermos nos ha inducido a esta publicación teniendo en cuenta las dificultades que desde el punto de vista diagnóstico suscita con otras enfermedades y el resultado quirúrgico muy bueno obtenido en uno de ellos. Innatas condiciones de talento lo colocaron rápidamente, primero entre sus compañeros del colegio secundario y en la Facultad de Ciencias Médicas de Buenos Aires. Cursó estudios secundarios en el Colegio Nacional de Santiago del Estero, recibiéndose a los 16 años de Bachiller en el año 1923, con medalla de oro. Antes de este mérito, en el año 1922, se le adjudica el premio "Damas Patricias", con medalla de oro, por su monografía histórica titulada "Juan Felipe Ibarra, su vida y su tiempo". En 1924 ingresa a la Facultad de Ciencias Médicas y tres años después, en mérito a sus relevantes clasificaciones, es designado por concurso, Practicante del Hospital Nacional de Clínicas. Asistió como tal al Instituto de Clínica Quirúrgica que dirigía en aquel entonces el Profesor José Arce, su primer maestro en la cirugía. En 1928, publica sus dos primeros trabajos documentados sobre "Tumores del lóbulo occipital" y "Rigidez descere¬ brada en los tumores de la epífisis", en colaboración con el Prof. Manuel Balado. Estas monogrfías figuran entre las primeras producciones serias y sistemáticas en materia de cirugía neurológica escritas en el país, puesto que las publicaciones sobre la especialidad, hasta esos momentos, se limitaban a meros casos clínicos aislados.
