Background As life expectancy grows, so do the challenges of caring for an aging population. Older adults, including people with dementia, want to live independently and feel in control of their lives for as long as possible. Assistive technologies powered by artificial intelligence and internet of things devices are being proposed to provide living environments that support the users’ safety, psychological, and medical needs through remote monitoring and interventions. Objective This study investigates the functional, psychosocial, and environmental needs of people living with dementia, their caregivers, clinicians, and health and social care service providers toward the design and implementation of smart home systems. Methods We used an iterative user-centered design approach comprising 9 substudies. First, semistructured interviews (9 people with dementia, 9 caregivers, and 10 academic and clinical staff) and workshops (35 pairs of people with dementia and caregivers, and 12 health and social care clinicians) were conducted to define the needs of people with dementia, home caregivers, and professional stakeholders in both daily activities and technology-specific interactions. Then, the spectrum of needs identified was represented via patient–caregiver personas and discussed with stakeholders in a workshop (14 occupational therapists; 4 National Health Service pathway directors; and 6 researchers in occupational therapy, neuropsychiatry, and engineering) and 2 focus groups with managers of health care services (n=8), eliciting opportunities for innovative care technologies and public health strategies. Finally, these design opportunities were discussed in semistructured interviews with participants of a smart home trial involving environmental sensors, physiological measurement devices, smartwatches, and tablet-based chatbots and cognitive assessment puzzles (10 caregivers and 2 people with dementia). A thematic analysis revealed factors that motivate household members to use these technologies. Results Outcomes of these activities include a qualitative and quantitative analysis of patient, caregiver, and clinician needs and the identification of challenges and opportunities for the design and implementation of remote monitoring systems in public health pathways. Conclusions Participatory design methods supported the triangulation of stakeholder perspectives to aid the development of more patient-centered interventions and their translation to clinical practice and public health strategy. We discuss the implications and limitations of our findings, the value and the applicability of our methodology, and directions for future research.
Background Evidence suggests that health care data sharing may strengthen care coordination, improve quality and safety, and reduce costs. However, to achieve efficient and meaningful adoption of health care data-sharing initiatives, it is necessary to engage all stakeholders, from health care professionals to patients. Although previous work has assessed health care professionals’ perceptions of data sharing, perspectives of the general public and particularly of seldom heard groups have yet to be fully assessed. Objective This study aims to explore the views of the public, particularly their hopes and concerns, around health care data sharing. Methods An original, immersive public engagement interactive experience was developed—The Can of Worms installation—in which participants were prompted to reflect about data sharing through listening to individual stories around health care data sharing. A multidisciplinary team with expertise in research, public involvement, and human-centered design developed this concept. The installation took place in three separate events between November 2018 and November 2019. A combination of convenience and snowball sampling was used in this study. Participants were asked to fill self-administered feedback cards and to describe their hopes and fears about the meaningful use of data in health care. The transcripts were compiled verbatim and systematically reviewed by four independent reviewers using the thematic analysis method to identify emerging themes. Results Our approach exemplifies the potential of using interdisciplinary expertise in research, public involvement, and human-centered design to tell stories, collect perspectives, and spark conversations around complex topics in participatory digital medicine. A total of 352 qualitative feedback cards were collected, each reflecting participants’ hopes and fears for health care data sharing. Thematic analyses identified six themes under hopes: enablement of personal access and ownership, increased interoperability and collaboration, generation of evidence for better and safer care, improved timeliness and efficiency, delivery of more personalized care, and equality. The five main fears identified included inadequate security and exploitation, data inaccuracy, distrust, discrimination and inequality, and less patient-centered care. Conclusions This study sheds new light on the main hopes and fears of the public regarding health care data sharing. Importantly, our results highlight novel concerns from the public, particularly in terms of the impact on health disparities, both at international and local levels, and on delivering patient-centered care. Incorporating the knowledge generated and focusing on co-designing solutions to tackle these concerns is critical to engage the public as active contributors and to fully leverage the potential of health care data use.
Background Social robots are anthropomorphised platforms developed to interact with humans, using natural language, offering an accessible and intuitive interface suited to diverse cognitive abilities. Social robots can be used to support people with dementia (PwD) and carers in their homes managing medication, hydration, appointments, and evaluating mood, wellbeing, and potentially cognitive decline. Such robots have potential to reduce care burden and prolong independent living, yet translation into PwD use remains insignificant. Method We have developed two social robots ‐ a conversational robot and a digital social robot for mobile devices capable of communicating through natural language (powered by Amazon Alexa) and facial expressions that ask PwD daily questions about their health and wellbeing and also provide digital assistant functionality. We record data comprising of PwD’s responses to daily questions, audio speech and text of conversations with Alexa to automatically monitor their health and wellbeing using machine learning. We followed user‐centric development processes by conducting focus groups with 13 carers, 2 PwD and 5 clinicians to iterate the design. We are testing social robot with 3 PwD in their homes for ten weeks. Result We received positive feedback on social robot from focus group participants. Ease of use, low maintenance, accessibility, assistance with medication, supporting with health and wellbeing were identified as the key opportunities for social robots. Based on responses to a daily questionnaire, our robots generate a report detailing PwD wellbeing that is automatically sent via email to family members or carers. This information is also stored systematically in a database that can help clinicians monitor their patients remotely. We use natural language processing to analyse conversations and identify topics of interest to PwD such that robot behaviour could be adapted. We process speech using signal processing and machine learning classifiers to identify emotions and mood. Conclusion Affective social robots with voice and facial expressions have a great potential for enhancing dementia care at home. Combining responses to daily questions, topics of conversations and emotions from speech analysis can help assess daily status of PwD automatically and monitor their health and well‐being over time remotely.
