Objetivo: Evidenciar através da literatura a importância do intervalo de tempo entre o trauma e a intervenção cirúrgica em fratura transtrocantérica em idosos. Revisão bibliográfica: A população idosa é a que mais sofre fraturas transtrocantérica, principalmente durante a deambulação. A absorção do impacto durante a queda aumenta de acordo com a progressão da fragilidade óssea. Fatores como estado físico, idade avançada, comorbidades, mobilidade do membro antes da fratura e atraso no tratamento estabelecido influenciam na mortalidade desse indivíduo em até um ano após a fratura. Considerações finais: O fator de extrema relevância quando se escolhe o tratamento cirúrgico é o intervalo de tempo entre o trauma e a intervenção. Assim, a partir do presente estudo foi possível verificar que o melhor tempo para abordar o trauma está no intervalo das 24 a 48 horas. Entretanto, para que o cirurgião possa realizar a cirurgia nesse espaço de tempo, é preciso que o paciente esteja em bom estado geral. Outro quesito relevante é a idade, pois quando elevada aumenta o risco de morte. Sendo assim, indivíduos com idade igual ou superior a 85 anos são avaliados como um marcador independente, com o intuito de identificar o elevado risco de morte.
INTRODUÇÃO:O transplante de medula (TMO) é um tratamento que consiste na substituição de uma medula óssea enferma ou deficitária por um tecido saudável e compatível com o receptor, reconstituindo uma medula saudável. O transplante pode ser classificado como autólogo (medula do próprio paciente) ou alogênico (medula de doador). Além disso, o transplante pode ser realizado a partir de células precursoras do tecido. O procedimento da TMO é indicado para patologias que afetam as células sanguíneas, tais como leucemias agudas e algumas leucemias crônicas, imunodeficiências, anemias graves, entre outras. Diante do exposto, faz-se necessário identificar os fatores relacionados ao tratamento. OBJETIVOS: Identificar os fatores que influenciam na realização do transplante de medula óssea, tanto do receptor quanto do doador. METODOLOGIA: Realizou-se uma pesquisa bibliográfica, por meio das principais bases de dados on-line, como Biblioteca Virtual em Saúde, SciELO e PubMed. Na obtenção dos materiais, foram utilizados os seguintes descritores: fatores associados ao transplante de medula, fatores no transplante de medula. Foram considerados artigos publicados entre os anos 2018 e 2023. RESULTADOS: Quanto aos fatores do paciente receptor, a boa evolução do transplante depende do estado do paciente, do estágio da patologia e das condições nutricionais e clínicas tais como um mínimo de 2,5 x 10 6 células CD34 positivas para transplantes alogênicos. Os fatores de risco estão associados às drogas quimioterápicas ministradas, bem como possíveis infecções. Os fatores de influência do doador estão em torno da histocompatibilidade podendo ser total ou parcial. Assim, quanto maior for compatibilidade entre doador e o receptor maior serão as chances de sucesso do transplante de medula, diminuindo a ocorrência de rejeição. Nesse ínterim, a análise é feita a partir de exames como tipagem HLA, tipagem ABO Rh, AST, ALT, TP, TTPA, bilirrubina, depuração creatinina, entre outros. CONCLUSÃO: Os fatores em torno do transplante de medula são inúmeros e se encontram em torno das condições do paciente e do doador, envolvendo variáveis que vão de fatores genéticos de histocompatibilidade até o histórico de saúde dos envolvidos ao longo do procedimento.
