Entender as relações entre epidemiologia e segurança alimentar implica desvendar as causas da fome, o modo como ela se distribui na sociedade e com que intensidade afeta as populações. Partindo desse pressuposto, este ensaio busca entender a lógica da fome dentro das várias transições vivenciadas nas últimas décadas: epidemiológica, demográfica, risco, nutricional e desenvolvimento. Os reflexos da transição demográfica ocorrida no Brasil no século XX estão relacionados com a mudança dos padrões do estado nutricional da população, que passou de um quadro de alta prevalência de desnutrição para o aumento cada vez mais significativo da obesidade. Contudo, o paradoxo dessa situação reside no fato de que a maior parte dos fatores de risco, para ocorrência das doenças do mundo moderno, provém dos “excessos”, enquanto em todo o mundo milhões de pessoas ainda morrem pelas “carências”. Só a produção de alimento não assegura sua distribuição eqüitativa entre os vários grupos da sociedade. Só o acesso não garante a qualidade. As escolhas são individuais, mas as opções de estilo de vida implicam um espectro de correlações que vão da economia à cultura, tendo esse campo, em última análise uma base coletiva, dentro da esfera do meio ambiente e desenvolvimento.
