The authors report a pertinent documented with subsidiary examination uncommon case of sensitive-motor polyneuropathy, in the course of hyperthyroidism, looking like be coherent to give it the name of "thyrotoxic polyneuropathy". In despite of few scientific works on this subject, strong arguments are presented intending to link the process of hyperthyroidism and the progressive degeneration, distal, symmetric and simultaneous, of peripheric nerves. Final considerations show the difficulty of recognizing the etiopathogeny of the degenerative process of peripheral nerves remembering the possibility of toxic, carencial or auto-immune nature.
Interessante grupo sindrômico é constituído pelo que se convencionou chamar de epilepsias reflexas, em que existe o precipitar reflexo de crises associadas a estímulo sensorial prévio. O estímulo para o precipitar crítico poderá estar relacionado estritamente à sensação sensorial (epilepsia reflexa no sentido estrito) ou à associação intelectiva e emocional que um estímulo sensitivo poderá desencadear (epilepsia musicogênica intrínseca; epilepsia intrínseca da leitura). Para Gastaut 7 seria aconselhável restringir-se a expressão "epilepsia reflexa" para crises evocadas por estímulo sensitivo determinado e bem conhecido, capaz de produzir a descarga de um sistema aferente e sua reflexão em vários níveis do cérebro, ou seja, através de arco ou sistema reflexo conhecido. Desde a idade média já se sabia que certos estímulos poderiam provocar ou influenciar o aparecimento de crises. Galeno, citado por Penfield 112 , mencionava a estrangulação de uma extremidade corporal (estímulo nociceptivo) como mecanismo de defesa efetivo para evitar o começo de crise jacksoniana motora. Este foi o primeiro relato em que se mencionou o cessar reflexo de crise epiléptica por estímulo sensitivo reconhecido. Caelius Aurelianus, citado por Penfield 13 , em 1755, já mencionava grupo de estímulos que poderiam causar crises convulsivas, tais como: sons altos ou estímulos ópticos rítmicos (movimento das ondas, o fluxo das águas e objetos em movimento). Bubnoff e Heidenhain, em 1881, citados por Penfield 112 ' 13 fizeram observações que iniciaram a compreensão do mecanismo básico fisiológico da precipitação sensitiva. Estes autores ao estimularem o centro motor no córtex, verificaram que, a partir deste evento, estí-mulos que supostamente seriam sub-liminares, passavam a ser efetivos para desencadear crises se, brevemente, antes da estimulação, a pele de determinada região fosse exposta a um estímulo táctil. Gordon Holmes 9 , em 1927, ao estudar distúrbios produzidos por lesões corticais, verificou que crises convulsivas poderiam ser desencadeadas a partir dessas lesões se o paciente recebesse estí-mulo táctil, principalmente na mão.
