The primary diagnosis of most cognitive disorders is clinically based, but the EEG plays a role in evaluating, classifying and following some of these disorders. There is an ongoing debate over routine use of qEEG. Although many findings regarding the clinical use of quantitative EEG are awaiting validation by independent investigators while confirmatory clinical follow-up studies are also needed, qEEG can be cautiously used by a skilled neurophysiologist in cognitive dysfunctions to improve the analysis of background activity, slow/fast focal activity, subtle asymmetries, spikes and waves, as well as in longitudinal follow-ups.
RESUMO -Realizamos estudo com o intuito de normatizar a atividade elétrica cerebral de base de indivíduos adultos e normais, com EEG quantitativo e topográfico. Foram avaliados 20 pacientes que apresentaram exames clínico, laboratoriais e de análise visual do EEG normais, além do mini-exame do estado mental com escores superiores a 27 pontos. Após a aplicação da transformada rápida de Fourier (FFT) encontramos distribuição do espectro com padrão monomodal no histograma de barras às custas de maiores potenciais em regiões posteriores na faixa de frequência alfa. Ao analisarmos a média dos espectros nas diferentes bandas de frequência (delta, teta, alfa, beta 1, beta 2, beta 3) encontramos alguns padrões que diferem dos achados tradicionalmente descritos na eletrencefalografia com análise visual, tais como a presença de atividade alfa em regiões anteriores e atividade delta difusa durante a vigília. Os comportamentos das bandas beta 2, e beta 3, também com distribuição difusa, não são os classicamente encontrados. Por outro lado, outros achados são congruentes à análise visual, como o predomínio alfa posterior e a maior presença de atividade teta em regiões centrais. Quantitative and topographic EEG: a study of normal adult populationABSTRACT -We studied the electric brain activity during wakefulness in 20 Brazilian people through digital EEG and spectral analysis in order to propose a standardization for Brazilian adult population. All this group is healthy with laboratory examinations and mini-mental state (scores higher than 27) evaluation normal. After Fourier fast transformation (FFT) calculation, we found a histogram display with monomodal distribution, with higher values in alpha band. Analyzing the average of these results, different standards from the analogical traditional EEG were found, as the distribution of alpha band and delta activity behavior. The beta 2 and beta 3 behavior showed a diffuse distribution, that is not the usual. By the other hand, other findings are congruent to the analogical EEG as the alpha posterior predominance and the bigger presence of theta activity at the central regions.KEY WORDS: quantitative and topographic electroencephalography, brain mapping, normal parameters.A medicina vem se informatizando com rapidez nos últimos anos, beneficiando muito a pesquisa diagnóstica em diversas especialidades. Dentro deste contexto não poderia ficar excluída a neurofisiologia clínica e, mais propriamente, a eletrencefalografia, na qual passamos a dispor do eletrencefalograma (EEG) digital e mapeamento cerebral (também denominado EEG quantitativo e topográfico) (EEGQT) [1][2][3] . A proposta de montar um estudo de padronização do normal do EEGQT foi motivada por não haver até o momento, em revisão bibliográfica por nós efetuada, nenhum estudo semelhante com a população brasileira.
RESUMO -Estudamos com EEG quantitativo e topográfico (mapeamento cerebral) quatro pacientes com exames pregressos de EEG convencional mostrando foco de linha média. O estudo quantitativo revelou-se mais preciso na definição do foco, localizando o seu hemisfério de origem e definindo a atividade focal como parassagital. Estes achados deverão ser validados em estudos futuros com amostragem maior.PALAVRAS-CHAVE: epilepsias, focos, linha média, EEG quantitativo, mapeamento cerebral. Epilepsies with midline foci: a study with quantitative and topographic EEGABSTRACT -We report a topographic and quantitative EEG (EEGQT) study of four patients with analogic EEG diagnostics of midline foci. The new study with EEGQT offered an increased definition of the electrical source with advantages in foci localization. These findings should be confirmed with studies including a greater number of patients.KEY WORDS: epilepsies, foci, midline, quantitative and topographic EEG, brain mapping.Desde que foi descrito por Penfield e Jasper em 1946, o foco de linha média (FLM) não tem recebido muita atenção dos pesquisadores 1 . Isto ocorre provavelmente pelas dificuldades técnicas em definir o FLM como uma descarga focal ao invés de generalizada, como já alertavam Tükel e Jasper ao caracterizá-lo 2 , e também por ser um evento pouco frequente, ocorrendo em cerca de 0,05% dos exames de eletrencefalograma (EEG) realizados em um laboratório especializado 3 . Este achado foi melhor definido por Pedley e col. 4 , em 1981, que o caracterizaram como ocorrendo predominantemente em crianças e jovens, associando-se fortemente a crises clínicas e sofrendo ação do sono como elemento ativador. É condição principal o EEG mostrar reversão de fase em FZ, CZ ou PZ 5,6 . Alguns autores associam-no clinicamente a uma prevalência de crises parciais complexas 6,7 , outros encontraram maior relação com crises generalizadas tônico-clônicas 3 .Relatamos os achados em quatro pacientes com FLM e que foram estudados também por EEG quantitativo e topográfico (EEGQT). Devemos destacar que não encontramos na literatura estudos com EEGQT e FLM.
