O estudo objetivou avaliar fatores sociais e de saúde envolvidos no fato de o idoso (60 anos ou mais) sair de casa. Foram entrevistados 5.898 idosos identificados por visita domiciliar, aleatoriamente selecionados em 59 cidades do Rio Grande de Sul, Brasil. A associação entre o desfecho e as variáveis independentes foi analisada de forma múltipla por meio de regressão logística. Fatores favorecedores à saída de idosos de casa: ser do sexo masculino, ser de faixa etária mais jovem, ser casado, ter artrose, realizar atividades específicas com facilidade e ter boa autopercepção de saúde. A presença de cardiopatias foi um fator negativo para sair de casa. Em face da importância da vida social na qualidade de vida e na política de envelhecimento ativo da Organização Mundial da Saúde, é fundamental considerar condições clínicas que permitem aos idosos manterem-se ativos em comunidade. Estudos como este podem auxiliar na adequação das políticas públicas para idosos, principalmente atuando em condições modificáveis, como as clínicas e funcionais.
Objective.
To describe the life expectancy, healthy life expectancy, disease burden, and leading causes of mortality and disability in adults aged 65 years and older in the Region of the Americas from 1990 to 2019.
Methods.
We used estimates from the Global Burden of Disease Study 2019 to examine the level and trends of life expectancy, healthy life expectancy, years of life lost, years lived with disability, and disability-adjusted life years (DALYs).
Results.
Across the Region, life expectancy at 65 years increased from 17.1 years (95% uncertainty intervals (UI): 17.0–17.1) in 1990 to 19.2 years (95% UI: 18.9–19.4) in 2019 while healthy life expectancy increased from 12.2 years (95% UI: 10.9–12.4) to 13.6 years (95% UI: 12.2–14.9). All-cause DALY rates decreased in each older persons’ age group; however, absolute proportional DALYs increased from 22% to 32%. Ischemic heart disease, stroke, and chronic obstructive pulmonary disease were the leading causes of premature mortality. Diabetes mellitus, age-related and other hearing loss, and lower back pain were the leading causes of disability.
Conclusion.
The increase in life expectancy and decrease of DALYs indicate the positive effect of improvements in social conditions and health policies. However, the smaller increase in healthy life expectancy suggests that, despite living longer, people spend a substantial amount of time in their old age with disability and illness. Preventable and controllable diseases account for most of the disease burden in older adults in the Americas. Society-wide and life-course approaches, and adequate health services are needed to respond to the health needs of older people in the Region.
RESUMO:O objetivo deste estudo é verificar a relação entre funcionalidade e autopercepção de saúde (APS) entre idosos jovens e longevos. Foi realizada uma análise secundária dos dados da Pesquisa Nacional de Saúde do Brasil de 2013. A regressão linear foi utilizada para verificar a correlação entre APS e funcionalidade, avaliada pelo nível de independência para atividades básicas (ABVD) e instrumentais de vida diária (AIVD). Participaram 11.177 idosos. Quanto maior a idade, menor o nível de AIVD (p<0,001) e de ABVD (p<0,001). Melhores níveis de funcionalidade correlacionavam com melhores níveis de APS. Essa relação perdia a intensidade ao comparar níveis mais elevados de APS (boa e muito boa), exceto para a AIVD entre as idades de 80-89 e 90+, que apresentaram melhores níveis de AIVD entre os níveis mais elevados de APS. Observou-se diferenças no comportamento da relação entre funcionalidade e a APS em diferentes faixas etárias.

RELATIONSHIP BET WEEN FUNCTIONALIT Y AND SELF-PERCEPTION OF HEALTH AMONG BRAZILIAN ELDERLY AND LONG-LIVED PEOPLEABSTRACT: This study aims to verify the relation between functionality and self-perception of health (SPH) in elderly and long-lived people. It was made a secondary analysis of data from the Brazilian National Health Survey (NHS) from 2013. The linear regression was used to verify the possible correlation between SPH and functionality, which was assessed by the level of independence in activities of daily living (ADL) and instrumental activities of daily living (IADL). 11.177 elderly people participated in the NHS. Higher the age, lower the level of IADL (p <0.001), the same for the ADL (p <0.001). Better levels of functionality were correlated with better levels of SPH. This relationship lost intensity when comparing higher levels of PHC (good and very good), except for IADL in people between the ages of 80-89 and 90+, who presented better levels of IADL between the higher levels of PHC. We noted differences in the behavior of the relation between functionality and PHC in different age groups.
