Es wird in der Literatur als Ursache fiir die Positivits der T-Zacke des Ekg im allgemeinen die Tatsache angegeben, dalii, die Erregnng des Herzens zwar an der Basis beginne, dort aber nicht auch zuerst beendet sei; vielmehr erhalte die Basis ihre Erregung zuerst, verliere sie aber anch zuletzt. So komme es dazu, dal~ das monophasisehe Ekg der Basis l~nger andauere als das der Spitze. Konstruiere man also aus der Differenz der beiden Kurven das iibliche Extremit~ten-Ekg, so mfis~ der Anteil der Basis am Anfang und am Ende fiberwiegen, und demnach Anf~ng (R) und Ende (5") den gleichen Aussehla,gsinn zeigen: T ist, wie man sich in der klinisehen Sprache ausdriickt, ,,positiv" 1. Es wird damit zugleich behauptet, dal~ der we]lenfSrmige Vorg~ng der Erregungsausbreitung selbst und seine Latenzzeiten an Spitze und Basis keine Rolle spielen gegeniiber d~er Zeitverschiedenheit in der Dauer der StrSme an Spitze und Basis selbst. Wiirden ni~mlieh diese StrSme an Spitze und Basis gleichlange dauern und die Erregung sich yon der Basis zur Spitze fortpflanzen, so mfiBte die Differenzkurve beider Vorg~nge, also das Extremits genau so aussehen wie jeder diphasische Aktionsstrom aueh: einem ersten ,,negativen" Ausschlag folgt eine zweite ,,positive" Welle, wobei negativ und positiv im Sinne der Elektrophysiologen gemeint ist, un4 neg~tiv also bedeutet, da~ die dem Reizursprung n~her liegende Elektrode negativ gegen die andere Bezugselektrode geworden ist 2 Diese Theorie hat trotz ihrer experimentellen Fundierung gewisse Schwierigkeiten.
