Özet: Bu derlemede travmada sonografi ile abdomen odaklı değerlendirme (AFAST), travmada sonografi ile toraks odaklı değerlendirme (TFAST) ve başucu akciğer ultrasonografik değerlendirme (VetBLUE) yöntemleri ele alınmıştır. Ultrasonografik muayeneyi gerçekleştirecek hekimin çok detaylı olmasa da belirli bir seviyede anatomi bilmesi ve kullandığı cihazın modlarına hakim olması gerekmektedir. Bunun dışında muayene teknikleri için yaklaşık 3 ile 6 dakika gibi kısa bir zaman yeterli olmaktadır. Ultrason birçok yardımcı tanı aracının aksine non-invazif güvenli ve daha hızlı bir yöntemdir. Çoğu araştırmacı bunun üzerine giderek acil kliniklerinde ultrason kullanarak hayati önem arz eden vakalarda hem hastanın zaman kaybını azaltmak, hem de hekimin daha kolay bir şekilde belirli hastalıkları elimine etmesini sağlayan birçok muayene tekniğinin geliştirilmesine yardımcı olmuştur. Anahtar kelimeler: Acil veteriner hekimliği, AFAST, TFAST, ultrason, VetBLUE Ultrasonographic Examination Methods in Emergency Veterinary Medicine Summary: In this review, abdominal focused assessment with sonography for trauma (AFAST), thorax focused assessment with sonography for trauma (TFAST) and bedside lung ultrasonographic examination (VetBLUE) methods in trauma are discussed. The physician who will perform the ultrasonographic examination should know a certain level of anatomy and be in control of the modes of the device. In addition, a short time of about 3 to 6 minutes is sufficient for these examination techniques. Unlike most diagnostic tools, ultrasound is a non -invasive, safer and faster method. Many investigators have increasingly assisted in the use of ultrasound in emergency clinics to reduce the time loss of patients in vital cases, as well as the development of several examination techniques that allow the physician to eliminate certain diseases more easily.
Fundus imaging with a smartphone-based camera has been reported in a limited number of literature, particularly in patient groups (pediatric/geriatric ie.) considered to be incompatible. In this study, by examining both eyes of 20 hunting dogs, multiple shooting series (20 sequential shooting automatic series with 1-second interval) and video sequence (a video that started shooting at 30-second and/or 60-second intervals) were recorded. The patients were first examined without any intervention. Afterwards, mydriatic drops were applied and the examination was repeated within the effective period. During the examination, optic disc nerve head, tapetum lucidum, non-tepatal region, retinal vessels, and choroid vessels were visualized in the posterior segment. Focal light artifacts were common when photographing the tapetum lucidum. The minimum light intensity was chosen to display the tapetum lucidum. No significant difference was observed between the examinations performed before the mydriatic drop and the examinations performed after the mydriatic drop. Further studies are recommended to formally assess clinical benefit.
ÖZÇalışmada, ayakta abomasum deplasmanı operasyonu uygulanan ineklerde, modifiye dorsolumbar epidural anestezi ile her ikisi de α2-agonisti olan ksilazin ve deksmedetomidin'in etkinliklerinin karşılaştırılması amaçlandı. Çalışmada iki grup oluşturuldu (n=12). Birinci gruptaki ineklerin epidural anestezisi ksilazin-lidokain hidroklorid kombinasyonu, ikinci gruptakilerin ise deksmedetomidin-lidokain hidroklorid kombinasyonu ile gerçekleştirildi. Abomasum deplasmanı operasyonuna alınan ineklerin preoperatif rektal ısı, nabız, solunum sayısı, ruminasyon sayısı kaydedildi. İlk interlumbar aralıktan, 18 G Touhy iğnesi ile orta hattan negatif basınç yöntemiyle epidural boşluğa girildi. Gruplara uygulanan ksilazin (0.025mg/kg)-lidokain hidroklorid (0.1mg/kg) ve deksmedetomidin (0.001mg/kg)-lidokain hidroklorid (0.1mg/kg) karışımları %0.9' luk serum fizyolojik ile 6 ml'ye tamamlanarak 1 dakika sürede uygulandı. Ensizyona tepki ile cerrahi uygulama sırasında sedasyon, ataksi ve huzursuzluk skorları 10 dk aralıklarla kaydedildi. Postoperatif 3 saat'lik zaman diliminde, 30 dk aralıklarla takibe devam edilerek veriler kaydedildi. İki olguda tekniğin uygulanamaması, 1 olguda ise hayvanın yatması nedeniyle toplam 3 olgu çalışma dışı bırakıldı. Çalışmaya dâhil edilen bütün olgularda, teknik kolaylıkla uygulanabildi. Uygulanan tekniğin operasyon için yeterli düzey ve sürede, analjezi ve sedasyon sağladığı gözlendi. Kolay uygulanabilen bu teknik ile az miktarda ilaç ve doz sarfıyla preemptif analjezi sağlandı. Sonuç olarak; ineklerin abomasum deplasmanı olgularında uygulanan modifiye dorsolumbar epidural anestezi tekniğinin, her iki etken madde için de klinik pratikte uygulanabilir olduğu kanısına varıldı.
Middle ear diseases are common in cats. The aim of this study was to determine the normative tympanometric data of cats. Tympanometry, which provides objective data in the examination of the middle ear in humans, can be used as a supplementary examination, especially in cats with middle ear diseases where the external ear canal is normal. In this study, 30 healthy ears belonging to 15 cats which were classified as ASA-I (American Society of Anesthesiologyist) were determined and evaluated according to equivalent ear canal volume (V ea ), static acoustic admittance [Y tm (compliance)], tympanometric peak pressure (TPP) and gradient values under general anesthesia. Tympanometric measurements obtained from right and left ears were 0.61 ± 0.12 ml and 0.60 ± 0.12 ml V ea , 0.31 ± 0.11 ml and 0.31 ± 0.11 ml Y tm , -11.2 ± 26.9 daPa and -14.9 ± 29.2 daPa TPP, 0.19 ± 0.11 ml and 0.24 ± 0.18 ml gradient respectively.Tympanometry as a supplementary examination is an easy to perform test in healthy cats with consistent results. It has the advantages of being completed in a short time, easy evaluation and providing information about middle ear without advanced imaging techniques. Results of this study represents the normative tympanometric measurements of cats which can be helpful distinguishing normal and abnormal conditions. Further studies needed to present exact values of standart tympanometric measurements and to introduce other variables like acustic refleks, wideband and multifrequency tympanometric results.
Özet: Oküler yapıların önemli bir bölümü doğrudan ya da dolaylı yoldan muayene edilebilir. Kapsamlı bir oftalmik muayene yapılarak göz hastalıklarının birçoğuna hızlı ve hatasız teşhis koymak mümkün olabilmektedir. Retina, optik sinir vasıtası ile direk beynin ön bölümüne bağlanır. Görme bozukluklarının değerlendirilmesi için retinayı etkileyen hastalıkların önceden belirlenmesi sağaltıma erken başlama ve prognoz açısından oldukça önemlidir. Gelişmiş görüntüleme teknolojileri sayesinde, hastaların patolojilerinin gösterilmesinde, sunulmasında ve paylaşılmasında büyük yol alınmıştır. Bu "derleme" köpeklerde retinanın tanısal görüntülenmesi ile ilgili bilgileri toplayarak hastalıkların tespitini kolaylaştırmak amacıyla sunulmuştur. With a comprehensive ophthalmic examination, it is possible to quickly and accurately diagnose most ocular diseases. The retina is connected directly to the anterior part of the brain via the optic nerve. The prediction of diseases affecting the retina is very important for the evaluation of visual impairment in terms of early onset of treatment and prognosis. Owing to sophisticated imaging technologies, a great advancement has been taken for the demonstration, presentation and sharing of patient pathologies. In this review, it is presented to facilitate the detection of diseases by collecting information about the diagnostic imaging of the retina in dogs.
