esophageal reconstruction using right colon interposition background: Esophageal replacement therapy is indicated for benign and malignant esophageal lesions and can be performed using stomach, jejunum or colon. aim: To report the experience using right colon interposition for esophageal replacement. material and methods: Review of medical records of 41 patients aged 17 to 73 years (29 males), subjected to right colon interposition in the last 20 years. Indications, complications, mortality and long term results were recorded. results: Forty nine percent of patients had a malignant esophageal lesion. Esophagectomy was performed using a transhiatal route or thoracotomy in 44 and 30% of patients, respectively. Retroesternal ascension was the most common route used in 88% of patients. The most common surgical complication was cervical fistula in 29% of patients, followed by fistula of the anastomosis between colon and jejunum in 7% of patients. Pneumonia was the most common medical complication. Postoperative mortality was 7%. conclusions: The mortality in this series of patients is similar to that reported elsewhere.
10 cm BE. The mean time elapsed from intestinal metaplasia to low grade dysplasia was 9 months, to high grade dysplasia 56 months and to adenocarcinoma 82 months. From low grade dysplasia to early cancer it was 18 months, from high grade dysplasia to early cancer 14 months and from high grade dysplasia to advanced transmural cancer 14 months. All patients were subjected to esophagectomy. Five patients detected at State I are alive without any evidence of recurrence after 36 to 130 months after surgery. Five patients with advanced transmural carcinoma subjected to radical esophagectomy died because of progression of the malignancy between 3 and 24 months after surgery. Conclusions: Progression to adenocarcinoma may occur even in absence of reflux symptoms while on acid suppression therapy. INTRODUCCIÓNLa preocupación médica por la obesidad como un problema que afecta la salud de las personas, tiene en nuestro país, referentes que datan de varias décadas atrás. En el año 1927, el Dr. Enrique González publica en la Revista Médica de Chile, un extenso relato acerca del manejo y el tratamiento médico de la obesidad 1 . En el artículo, el autor se extiende en detalle en el análisis de las bases de la terapia que consideraba entonces un régimen alimenticio, higiene, fisioterapia y también medicamentos con formulaciones que incluían mercurio, arsénico y yoduro. El artículo aparece ilustrado con un caso clínico, incluyendo las fotos del antes y el después, y reseña el resultado de esta terapia en una mujer de 20 años, que inicialmente pesaba 140 kg y que al cabo de dos años tratamiento había logrado bajar a 75 kg. Desde antes y durante los muchos años que siguieron a esta comunicación, el tratamiento de la obesidad en nuestro país estuvo limitada al tratamiento medico.La historia quirúrgica del manejo de la obesidad mórbida como la conocemos hoy, se inicia en nuestro medio hacia 1986, cuando González y cols. del grupo del Hospital Van Buren de Valparaíso, comunican su experiencia con el Bypass yeyunoileal en 6 pacientes portadores de obesidad mór-bida 2 . Todos los pacientes tenían hipertensión arterial, dos de ellos tenían además diabetes y la mitad presentaba problemas osteoarticulares. En todos se realizó un Bypass yeyuno-ileal termino lateral. El promedio de peso del grupo antes de la cirugía era de 130 kg y éste bajó a 90 kg tras el procedimiento quedando con un porcentaje de exceso de peso de 51%. En 4 pacientes se normalizó la hipertensión arterial y se corrigió la diabetes en los 2 pacientes que la tenían. En el seguimiento se observó que todos los pacientes presentaron diarrea importante, en dos de ellos se evidenció disfunción hepática y en un caso hubo manifestaciones de urolitiasis.El segundo estudio quirúrgico nacional fue publicado 7 años después, en 1993, por el grupo de Awad y cols. del Hospital San Juan de Dios. En este trabajo se mostraban los resultados del tratamiento quirúrgico en 30 pacientes con obesidad mórbida 3 . En estos pacientes se realizaron 3 técni-cas quirúrgicas diferentes: a) una gastro-gastroanastomosis o comunicación central en 8 pacientes, b) partición gástrica con comunicación en curva mayor con refuerzo de malla de prolene en 18 pacientes y c) gastroplastía vertical con refuerzo de malla en 4 pacientes. No hubo mortalidad operatoria en esta serie y solo 2 pacientes tuvieron complicaciones postoperatorias. Los resultados no fueron buenos en los pacientes con comunicación central, con solo un descenso del 20% del peso preoperatorio. Los pacientes con comunicación en la curvatura mayor bajaron de peso, con estabilización a los 12 meses de operados, con recuperación de peso en 4 de ellos. Los 4 pacientes con gastroplastia vertical tenían menos de un año de seguimiento al momento de la publicación. Pocos años
Surgical treatment of achalasia. Experience in 328 patientsBackground: Patients with achalasia may require surgical treatment. Aim: To assess postoperative evolution, complications and mortality after surgical treatment of achalasia. Material and Methods: Analysis of 328 patients aged 13 to 80 years (51% females) with achalasia, operated in a period of 40 years. Open surgery was used in 165 patients and a laparoscopic modality (starting in 1994) in 163. Results: Patients subjected to open or laparoscopic surgery had similar demographic and manometric features. Mucosal injury during myotomy occurred in 20 (12%) and 10 (6%) of patients subjected to open or laparoscopic surgery, respectively (p < 0.05). Four patients operated using a laparoscopic approach had to be converted to open surgery. Two patients operated using an open approach had a postoperative leak. One patient had an abscess and one a hemoperitoneum. Conclusions: The surgical approach of choice for achalasia is laparoscopic, with a low incidence of complications.
RESUMENPresentamos el caso de neumomediastino espontáneo en un hombre joven sin asociación a algún desencadenante o patología, que consulta por dolor cervical irradiado a región supraclavicular derecha y con examen físico compatible con enfisema cervical. La evaluación inicial incluyó estudio radiológico de cuello, columna cervical y tórax. La tomografía axial computada demostró un neumomediastino con extensión al cuello sin patología pulmonar subyacente, se complementó el estudio con esofagograma. La evolución del paciente fue satisfactoria y asintomática, como se describe en los casos reportados en la literatura internacional. Se discute la fisiopatología del neumomediastino.PALABRAS CLAVE: Pneumomediastino espontáneo, dolor cervical, enfisema cervical. SUMMARYWe report a 22 years old male that consulted in the emergency room for a painful cervical mass that appeared spontaneously and grew rapidly. On physical examination cervical and supraclavicular subcutaneous emphysema was noted. Neck and chest CAT scan showed a pneumomediastinum in the absence of lung lesions. The condition subsided spontaneously in 24 hours and the patient remains asymptomatic. INTRODUCCIÓNEl neumomediastino espontáneo (NE) se define como la presencia de aire en el mediastino, no asociada a causa traumática, iatrogénica o enfermedad pulmonar subyacente conocida. Corresponde a una entidad poco frecuente, con una incidencia entre 1/800 a 1/42.000 consultas a los servicios de emergencias, en las escasas series publicadas.Descrito por primera vez por Hamman en 1939 1 , afecta principalmente a hombres jóvenes y mujeres embarazadas 2-4 . La clínica se asocia a disnea, disfagia y dolor cervical, aunque puede darse de manera infrecuente sólo este último, como manifestación aislada.Presentamos un caso de NE en un hombre joven sin asociación a desencadenante, que consulta por dolor cervical irradiado a región supraclavicular derecha.
