Background: Myocardial revascularization surgery (MRS) is the most common surgery in this country, with most being performed through the National Health System(SUS).
ResumoDos equipamentos utilizados pela área de saúde para provisão de cuidados assistenciais, os hospitais certamente são preferidos tanto pela população como também pelos profissionais da área. Isto ocorre devido às facilidades e segurança que oferecem face à concentração de recursos humanos e tecnológicos. Este fato, porém, representa custos elevados que devem ser amenizados com aumento da produtividade e da qualidade. Em relação a aspectos como gestão de qualidade, produtividade, dimensionamento de pessoal, aos poucos foram desenvolvidos os trabalhos no Brasil. Assim, foi realizada pesquisa que visa conhecer a produtividade dos hospitais, utilizandose de indicadores hospitalares como a média de permanência, índice de rotatividade, índice intervalo de substituição, e também a relação funcionários/ leito e/ou cama.
A experiência de um novo modelo de gestão dos serviços públicos de saúde implantada no Estado de São Paulo é analisada neste artigo, a partir da caracterização da especificidade da lei que regulamenta a criação das Organizações Sociais de Saúde, tendo como referencial a gestão estadual do Sistema Único de Saúde, seus antecedentes e a situação atual. Os dados apresentados foram coletados a partir de relatórios produzidos pela equipe técnica responsável pelo convênio entre a Secretaria e a Faculdade de Saúde Pública -- USP, para acompanhamento do projeto. Os resultados encontrados demonstraram o cumprimento satisfatório por parte das OSS/SUS, a partir das metas estabelecidas no contrato de gestão pela Secretaria, para o primeiro ano de atividade. O trabalho aborda, ainda, as perspectivas do novo modelo e da atual política. Os autores alertam para o fato de que a regulação, através de um novo contrato de gestão, deve considerar o desempenho das OSS, enfatizando processos, resultados e inserção sistêmica, referenciada pela demanda de saúde regional.

INTRODUÇÃOAo iniciar, torna-se necessário a definição de termos que vêm sendo utilizados neste processo, por isso é importante esse conhecimento. O primeiro deles, Certificação 1 é um processo pelo qual uma agência governamental ou uma associação profissional reconhece oficialmente uma entidade ou indivíduo como tendo encontrado certas qualificações predeterminadas. O segundo, a "acreditação" 2 que é o procedimento de avaliação dos recursos institucionais; voluntário, periódico e reservado, que tende a garantir a qualidade da assistência através de padrões previamente aceitos. Os padrões podem ser mínimos (definem o piso ou base) ou mais elaborados e exigentes, definindo diferentes níveis de satisfação. Qualidade 3 é o grau no qual os serviços prestados ao paciente aumentam a probabilidade de resultados favorá-veis que, em conseqüência, reduzem os desfavorá-veis dado o presente estado da arte.Pode-se afirmar que toda prestação de serviço em saúde tem dois componentes de qualidade: operacional, que é o processo propriamente dito, e outro de percepção, ou como os clientes percebem o tipo de serviço oferecido, e os prestadores se sentem na oferta que fazem.O tema acima, para fins didáticos, pode ser dividido em duas partes: gestão de processos e certificação para qualidade, concluindo-se com recomendações que viabilizem a sua prática. GESTÃO DE PROCESSOSUm dos primeiros passos para o entendimento da gestão de processos é o estudo do modelo de Fleming 4 (Fig. 1). Pode-se observar que os resultados dependem de processos que inevitavelmente estão na dependência da estrutura e do meio ambiente. A estrutura é definida pelos insumos, como área física, recursos materiais (equipamentos, ferGestão de processos e certificação para qualidade em saúde ramentas, financeiros, instrumentais, utensílios, tecidos, gases, órteses, próteses), recursos humanos e instrumentos de gestão, incluindo-se a estrutura organizacional (organograma) e os modelos teóricos aplicados na administração da instituição.Quanto ao processo, Fleming o define como toda tecnologia envolvida nos cuidados ao paciente. Hammer e Champy 5 entendem processo empresarial como um conjunto de atividades com uma ou mais espécies de entrada e que cria uma saída de valor para o cliente. Processo 6 também pode ser definido como um conjunto de atividades de trabalho inter-relacionados que se caracteriza por requerer certos insumos e tarefas particulares, implicando em um valor agregado com vistas a obter resultados. Procedimento, por outro lado, de acordo com o Aurélio 7 , também é processo, é método. Estrutura e processo caracterizam o meio interno da instituição.
