Рак анального канала -это редкое злокачественное заболевание дистального отдела желудочно-кишечного тракта, часто связанное с вирусом папилломы человека, самой распространенной инфекцией, передаваемой половым путем, во всем мире. В настоящее время доступны методы скрининга анальной интраэпителиальной неоплазии, предшественника рака анального канала, сочетающие цитологическое исследование соскоба с окраской по Папаниколау и аноскопию высокого разрешения с биопсией подозрительных участков. Известно, что анальный канал -это область, которую обследуют врачи-проктологи, однако и при эндоскопическом исследовании возможно выявление рака анального канала, в том числе -и на ранних стадиях опухолевого процесса. При проведении эндоскопического исследования нижних отделов желудочно-кишечного тракта, зачастую, наиболее частой причиной неполного осмотра толстой кишки при колоноскопии является плохая подготовка, реже -анатомические особенности толстой кишки. Врач-эндоскопист, осмотревший ободочную кишку, может пропустить изменения относящиеся к нижнеампулярному отделу прямой кишки и анальному каналу, быстро извлекая видеоэндоскоп и не используя методику осмотра данной области в ретрофлексии.В настоящем обзоре рассмотрены эпидемиология интраэпителиальной неоплазии и рака анального канала в Российской Федерации, канцерогенез рака анального канала, представлен алгоритм обследования пациентов группы риска, диагностические и лечебные эндоскопические методики, позволяющие своевременно диагностировать патологические состояния нижнеампулярного отдела прямой кишки и анального канала, также представлены современные возможности эндоскопического лечения данной патологии.Ключевые слова: анальная интраэпителиальная неоплазия, AIN, рак анального канала, ранний рак анального канала, вирус папилломы человека, ВПЧ, скрининг, анальная цитология, МСМ, ВИЧ, лечение рака анального канала Конфликт интересов. The problem of endoscopic diagnosis of early esophago-gastric junction cancer is one of the most important in modern endoscopy and oncology, since most malignant tumors of the esophagogastric junction are detected at late stage. The 5-year survival rate of such patients remains low. Definition of early esophagogastric junction cancer, resectability criteria, features of the course of the disease at early stage will be considered in this article. It has been proven that NBI-ME and NBI Dual Focus in combination with endosonography are the most accurate in the diagnosis of early esophagogastric junction cancer. The most typical endoscopic features of early esophagogastric junction cancer are discussed in this review. All these methods are applied to the patient in Herzen Moscow Cancer Research Institute, branch of National Medical Research Radiology Center.
Over the past decade, there has been an increase in the incidence of esophagogastric junction cancer. Since most of these tumors are detected at late stage, the 5-year survival rate of such patients remains low. Therefore, endoscopic diagnosis of early esophago-gastric junction cancer is one of the most important in modern endoscopy and oncology. Due to the development of modern endoscopic technologies, there has been an increase in the diagnosing such tumors at an early stage. All of this together means we need to update the treatment tactics in such patients. The team of authors aims not only to identify possible discrepancies in examination of patient with suspected early neoplasia, but also to demonstrate a rare complication of endoscopic intraluminal surgery.
Signet ring cell carcinoma is a specific histological subtype of gastric adenocarcinoma. The tumor name is due to the characteristic type of malignant cells nuclei, that take the form of a ring. Such cells contain a large amount of intracellular mucin, which shifts the nucleus to the periphery creating a typical microscopic appearance. Currently, there is no single point of view, regarding the treatment strategy of such patients, as well as the prognosis of the disease. Modern endoscopic technologies allow not only to detect signet ring cell carcinoma of the stomach at an early stage, but can also be the method of choice in the radical treatment of such patients. This clinical observation aims to demonstrate the results of endoscopic diagnosis of early signet ring cell carcinoma in two patients with different treatment approaches.
