Potassium hydroxide (KOH) is a strong alkali that has long been known to digest proteins, lipids, and most other epithelial debris of skin scrapings to identify fungal infections. To our knowledge, KOH has never been used for the treatment of molluscum contagiosum (MC). We evaluated 35 children with MC for the clinical effectiveness of treatment with topical 10% KOH aqueous solution. The solution was applied by the parents of affected children, twice daily, on each MC lesion. The therapy was continued until all lesions underwent inflammation and superficial ulceration. Thirty-two of 35 patients achieved complete clinical cure after a mean treatment period of 30 days. Three children discontinued treatment: two reported severe stinging of the lesions and refused further applications; the other, with giant MC lesions, developed a secondary infection with prolonged treatment. Therapy with KOH was found to be effective and safe in the treatment of MC in children.
Twenty-five year old male patient presenting with asymptomatic brown spots, on
cervical, axillary, inguinal and popliteal regions, for the last nine months.
Pathological examination showed hydropic degeneration of the basal layer, pigmentary
incontinence and moderate inflammatory lymphocytic infiltrate in the dermis. Lichen
planus pigmentosus inversus is a rare subtype of lichen planus characterized by
hyperchromic, asymptomatic or mildly pruritic macules, measuring from millimeters to
centimeters in diameter, with defined borders, affecting intertriginous areas, most
commonly in the axillae and groin of Caucasian patients. It presents unique lichenoid
histology. We report a case with typical clinical features, histology and
evolution.
Cutaneous metastasis is a phenomenon that results from a tumor spreading via
lymphatic or vascular embolization, direct implant during surgery or skin involvement
by contiguity. The primary malignant tumor that most commonly metastasizes to the
skin in women is breast cancer, which can be manifested through papulonodular
lesions, erysipeloid or sclerodermiform infiltration, en cuirasse. We report the case
of a female patient, 78 years old, with papular, scaly and confluent lesions in the
right breast for one year, progressing to edema and skin infiltration, reduction of
breast volume and plaque en cuirasse, and similar lesions in the contralateral breast
and abdomen for four months. The pathological diagnosis was invasive ductal breast
carcinoma with Paget-like foci, epidermal skin metastases and lymphatic
embolization.
SUMMARYThe authors study a patient carrying Aids, with exuberant dermatological manifestations of cryptococcosis. They stress the therapeutic effectiveness of short-term amphotericin B. The authors reviewed cases of cutaneous infection with Cryptococcus reported in the national and international literature, verifying that the frequency has increased with the AIDS epidemic. Also, they discuss about the differential diagnosis with some cases of dermatosis, particularly with the disseminated giant molluscum contagiosum. In relation to the therapy, they affirm that the choice of drug depends on the organ involved, as well as the immune state of the patient.
Resumo: Relato de cinco casos de tinea nigra palmaris observados em quatro crianças, do sexo feminino, e um adolescente, do sexo masculino, em Santos, litoral de São Paulo. Discutem-se os aspectos históricos, epidemiológicos, diagnósticos, terapêuticos e prognósticos embasados em revisão de literatura. Palavras-chave: cladosporium; epidemiologia; exophiala; tinea.
Summary
O propiltiouracil (PTU), droga empregada no tratamento do hipertireoidismo, pode provocar reações adversas em 3% a 12% dos pacientes, desde reações leves e frequentes, até quadros incomuns, graves e fatais. Relata-se o caso de um paciente de 14 anos de idade, em uso de PTU há três anos, que apresentou febre, bolhas hemorrágicas e úlceras necróticas, evoluindo com lesões purpúricas e nódulos violáceos em membros inferiores. A avaliação laboratorial e histológica mostrou-se compatível com vasculite cutânea leucocitoclástica, padrão encontrado nas vasculites por reação de hipersensibilidade. Após a retirada da droga antitireoidiana e a introdução de prednisona, houve regressão progressiva das lesões cutâneas. A importância deste estudo é alertar sobre a ocorrência desta grave manifestação cutâneo-sistêmica (vasculite), por droga de uso frequente em medicina interna, cuja taxa de mortalidade é de 10%. É obrigatório o diagnóstico precoce, visando à retirada da droga suspeita, e, se necessário, ao uso de corticosteroides e/ou imunossupressores.
