Objetivo: Identificar incidentes ocorridos com usuários da APS na cidade de Escada-PE. Materiais e Métodos: Estudo de caráter transversal, descritivo, de abordagem quantitativa e qualitativa. Realizado com profissionais da APS no estado de Pernambuco no ano de 2019, através da aplicação do questionário Primary Care International Study of Medical Errors (PCISME) os dados foram analisados com o suporte do software EpiInfo versão 7.2 e as narrativas foram analisadas pelo Discurso do Sujeito Coletivo com o suporte do DSCSoft 2.0.Resultados: Pode-se verificar que 40% dos incidentes estão relacionados a um paciente em particular, onde em sua maioria o paciente é do sexo feminino 78,94%,apresentando a média da idade de 41,41 anos, estando a maioria do erro apontado como moderado 10,00% ocasionado principalmente no consultório 20,00%. Sendo possível elencar por meio da análise das expressões cinco discursos em cinco categorias temáticas: Falha por recurso material ineficaz/insuficiente, Falha no cuidado, Falha administrativa, Falha com exame exploratório e Falha de competência técnica. Conclusões: O estudo demonstra que necessita do trabalho conjunto entre a gestão e todos os profissionais da Unidade básica de saúde, a fim de proporcionar ambiente de trabalho digno e atendimento qualificado.
