We present an experience report on the implementation of real-time telehealth in speech-language and hearing therapy for patients who were previously seen on an outpatient basis in a primary health care service. Twenty-five users were being monitored by the Speech-Language Therapy (SLT) team when the first cases of COVID-19 were notified in southern Brazil. Of these, it was judged that twelve patients required at least a monitoring call every two weeks. Teleconsultations were available in this first stage, on an emergency basis, during the implementation of the project in this format. The idea was to guarantee, due to the suspension of the SLT sessions, the maintenance of the care service for patients who could suffer worsening of their cases or even comorbidities. The appointments were carried out by video calls by SLT students, therapists of the extension project, and supervised by a speech-language therapist, synchronously. All conversations and orientations during the teleconsultation were conducted as calmly as possible and, in the case of infant patients, permeated by some playful activities. Telehealth has shown to be an efficient resource for the care of patients with SLT demands, enabling remote care with the same quality as face-to-face care. In addition, it has relevant potential, considering that there are a significant number of patients who need SLT assessment and live in regions where there is a shortage of qualified professionals.
Apresentamos um roteiro de atuação em telessaúde para atendimento fonoaudiológico de pacientes adultos em domicilio e o aplicamos em um projeto comunitário durante a Pandemia da COVID-19. O presente roteiro foi desenvolvido em três etapas distintas. Inicialmente foi realizada uma revisão de literatura afim de identificar aspectos facilitadores e as barreiras para o atendimento de pacientes em casa via telessaúde. A segunda etapa consistiu na implementação de um serviço telefonoaudiologia associado a uma Unidade Básica de Saúde, a qual possibilitou experiência para discussão de meios facilitadores na prática clínica, bem como maneiras de qualificar a teleconsulta fonoaudiológica. A terceira etapa abrangeu a elaboração do roteiro que incluiu, além da escrita, a discussão detalhada de cada aspecto elencado no mesmo, buscando sua efetividade e integralidade. Foram selecionados e simplificados neste roteiro os principais aspectos que interferem na teleconsulta e formas de qualificar a telefonoaudiologia. Assim, o roteiro foi organizado em 20 itens, divididos em antes, durante e depois do atendimento fonoaudiológico, visando a efetividade da conduta fonoaudiológica. O roteiro apresentado se propõe a ser suporte para a atuação em telefonoaudiologia. Entretanto, deve ser adaptado à realidade cultural e social dos profissionais e pacientes que a esta forma de atuação forem expostos. Além disso, não se trata de um conjunto de práticas e procedimentos fechados em si, deverá ser atualizado conforme novos procedimentos e tecnologias estejam acessíveis para a atuação em telefonoaudiologia.
RESUMO Apresenta-se um roteiro de atuação em telessaúde para atendimento fonoaudiológico de pacientes adultos em domicílio que foi aplicado em um projeto comunitário durante a Pandemia da COVID-19. O roteiro apresentado se propõe a ser suporte para a atuação em telefonoaudiologia. Entretanto, deve ser adaptado à realidade cultural e social dos profissionais e pacientes que a esta forma de atuação forem expostos. In this study, a script for the telepractice of speech language pathology and audiology services for adult patients at home, is presented. This tool was developed in three stages and used for a community project during the COVID-19 pandemic. A literature search was performed to identify the facilitators and barriers to remote patient care before implementing telepractice at a primary health care unit. This practical experience led to further discussions about the facilitators of clinical performance and different ways to remotely offer quality assessments and rehabilitation. Divided into three sections, the final script was organized into twenty items. To provide comprehensive patient care, detailed discussions were held about each item to verify the effectiveness and quality of the teleconsultations, for the script to be written. This guide is intended to be used as a tool for speech language pathology and audiology telepractice. However, it must be adapted to the cultural and social realities of the professionals and patients who use it. Additionally, it does not constitute a closed set of practices and procedures and should be updated as new procedures and technologies become available.
