Objetivo: Analisar as características da Hiperplasia Adrenal Congênita (HAC). Revisão bibliográfica: A HAC compreende um grupo de doenças autossômicas recessivas causadas por mutações em um dos genes que codificam enzimas envolvidas na esteroidogênese adrenal. Pode resultar da falha de qualquer uma das enzimas envolvidas na cascata de síntese de hormônios esteróides e a forma mais comum é causada pela deficiência do esteroide 21-hidroxilase (21-OHD), afetando cerca de 1:15.000 nascidos vivos. A discriminação da HAC em formas Clássicas e Não Clássicas é baseada no grau de deficiência da 21-hidroxilase, sendo a HAC clássica subdividida em uma forma perdedora de sal e em uma forma virilizante. Considerações finais: A HAC é uma doença genética que possui duas formas principais de apresentação. A HAC clássica, mais grave e potencialmente fatal, pode ser diagnosticada através do teste do pezinho no Brasil, o que pode prevenir crises perdedoras de sal que representam o pior prognóstico da doença. A HAC não clássica é geralmente diagnosticada na puberdade ou adolescência. O manejo desta condição é feito através dos glicocorticóides, tendo em vista todas complicações da administração desse medicamento.
