The implantation of a left ventricular assist device (LVAD) has become an essential requirement for managing patients with end-stage heart failure. However, aortic valve insufficiency is a contraindication for LVAD implantation in patients with end-stage heart failure, partly because of the decreasing efficiency of mechanical circulatory support and the eventual development of right ventricular failure. Herein, we present the first case of performing transcatheter aortic valve replacement in valve-in-ring along with LVAD implantation for the treatment of a 60-year-old male suffering from refractory heart failure due to dilated cardiomyopathy and pure aortic insufficiency in need of a new aortic bioprosthesis. A balloon-expandable bioprosthetic transcatheter heart valve was implanted into a previously sewn annulus ring into the aortic root via transaortic access. Subsequently, a centrifugal-flow LVAD was implanted. Postoperatively, the patient was in New York Heart Association Functional Class (NYHA) II with 6-min walk test of 310 m. The patient has completed 6 months of follow-up with no events. This novel and feasible surgical technique reduced the cardiopulmonary bypass time and duration of surgery. Furthermore, it avoids the risk of redo sternotomy and decreases the chances of paravalvular leakage and worsening of aortic regurgitation.
Left ventricular assist device (LVAD) outflow graft injury is a very rare complication after LVAD implantation and is usually treated surgically. This is a case report of successful stenting of the damaged outflow graft 2.5 years after LVAD implantation, followed by successful heart transplantation.
ФУНДАМЕНТАЛЬНЫЕ ИССЛЕДОВАНИЯ И МЕЖДИСЦИПЛИНАРНЫЕ ТЕХНОЛОГИИ ■ Актуальность. Коронавирусная инфекция (COVID-19) -это инфекционное заболевание, вызываемое коронавирусом SARS-CoV-2. Пациенты с сердечно-сосудистыми заболеваниями подвергаются особо высокому риску заболеваемости и смертности. Пациенты cо вспомогательным механическим устройством левого желудочка (LVAD) представляют собой уникальную популяцию, подверженную риску заболевания из-за сопутствующих заболеваний и функционально ослабленного иммунитета. Целью данной статьи явился анализ клинических случаев COVID-19 среди пациентов с LVAD в Республике Казахстан. Материал и методы. Были собраны и проанализированы характеристики LVAD-пациентов Республики Казахстан, перенесших СOVID-19 в период с марта 2020 г. по август 2021 г.: начальные проявления заболевания, сопутствующие заболевания, осложнения и клинические исходы. Результаты. Всего были опрошены 177 пациентов с LVAD, находящихся на поддержке и учете НАО «Национальный научный кардиохирургический центр». У 41 (23,2%) пациента были классические симптомы COVID-19. Средний возраст пациентов составил 55 лет (интерквартильный размах 17-75), 9 (21,9%) пациентов -женщины. Большинству пациентов (n=36, 87,8%) был имплантирован LVAD HeartMate 3. Наибольшее количество -19 (46%) -составили пациенты с дилатационной кардиомиопатией (ДКМП). Время LVAD-поддержки варьировалось от 1 до 83 мес до появления симптомов и диагностирования COVID-19. 13 (31,7%) пациентам потребовалась госпитализация в больницы по месту жительства в связи со снижением сатурации кислорода (менее 94%) и кислородозависимостью. 3 пациента (1,7% среди 177 пациентов, находящихся на LVAD-поддержке, и 7,3% среди пациентов с COVID-19) потребовали ухода и лечения в отделении интенсивной терапии. Заключение. Мы представили анализ клинических случаев LVAD-пациентов с COVID-19 в Казахстане. Следует отметить, что на сегодняшний день отсутствуют какие-либо рекомендации по ведению и лечению, нет данных об особенностях клинического течения COVID-19 у пациентов на LVAD-поддержке. Тщательный мониторинг клинического состояния, антикоагуляция и частый сбор параметров устройства и изображений места выхода чрескожного кабеля, обучение мерам предосторожности, ранняя диагностика, своевременно начатое лечение, поддерживающая психологическая помощь, мультидисциплинарный подход и тщательное наблюдение имеют решающее значение в ведении LVAD-пациентов с COVID-19.
