A expansão rápida da maxila assistida cirurgicamente (ERMAC) é um procedimento descrito na literatura e bastante utilizado em pacientes adultos jovens com maturidade óssea, os quais possuem um processo de interdigitações ósseas avançado. O objetivo deste trabalho é revisar a literatura analisando a quantidade e duração das ativações do aparelho expansor utilizado, as indicações para realização do procedimento cirúrgico, as técnicas cirúrgicas utilizadas com suas vantagens e complicações. As indicações principais são pacientes adultos jovens com maturidade óssea em que não há sucesso com expansão rápida da maxila (ERM) e que já possuem as suturas obliteradas. Os aparelhos de escolha recaem sobre os dento suportados (Hyrax) e dento-muco-suportado (Haas) com ativações que variam de uma volta à meia volta por dia, por um período de aproximadamente quinze dias. A ERMAC pode ser realizada por basicamente duas técnicas cirúrgicas, sendo as mais utilizadas as do tipo Le Fort I subtotal e Le Fort I modificada. Dessa forma, o tratamento com deficiência transversa deve ser realizado com uma equipe multidisciplinar por envolver várias especialidades na área da odontologia, resultando em um tratamento muito eficaz.
