The consolidation of Unified Health System (“SUS”) demands the democratization of relationships; however, in its 30 years, the separation between management and health care has been remarkable. Thus, the objective was to know how scientific articles address health management in SUS. An integrative literature review was carried out in the Scielo and PePSIC databases with the descriptors: “health management”; “co-management”; and “participatory management”, with 16 articles remaining. Thematic content analysis followed, resulting in the categories: “conceptions about health management in SUS”; “difficulties for health management”; “potentialities of health management”; and “proposals for health management”. We concluded that is necessary to bring social actors closer together and consider aspects such as affectivity and subjectivity in the management process, allowing workers, managers and users - all SUS operators - to recognize themselves in health production.
Resumo O Sistema Único de Saúde se sustenta pelos pressupostos do trabalho coletivo, de relações transversalizadas e do compartilhamento de saberes e práticas. Tais princípios, todavia, encontram dificuldade de efetivação perante o modelo biomédico e o desmonte da saúde pública nos últimos anos. Assim, objetivou-se analisar as relações entre profissionais de uma equipe gestora municipal e profissionais dos serviços de saúde. Para isso, realizou-se sete grupos focais com profissionais de gerência e trabalhadores e diretores de serviços de saúde, totalizando 100 participantes. As transcrições dos grupos compuseram um corpus que foi submetido à análise lexical do tipo classificação hierárquica descendente com o auxílio do software Iramuteq, originando cinco classes: “Cotidiano dos Serviços”, “Metáforas da Relação Gestão-Serviço”, “Cotidiano da Gerência”, “Construir Junto?” e “Relação Gestão e Serviço”. A relação gestão-serviços é marcada por ambiguidades. Ora é percebida como distante, e os profissionais da gestão são tidos como os que planejam e decidem e os dos serviços, como os que executam. Ora é vista como lugar de construção conjunta de projetos coletivos em torno de um sistema que dá certo. Isso afirma a discussão de um hibridismo entre o paradigma dominante e o paradigma emergente. Esse último com a intenção de incluir no rol das práticas cotidianas da saúde um olhar para as subjetividades presentes e a perspectiva da construção de relações transversalizadas entre os diversos atores sociais, demandando espaços que potencializam coletivos capazes de promover cogestão.
O objetivo deste trabalho consiste em apresentar uma experiência de acolhimento grupal em saúde mental em uma Unidade de Saúde da Família do município de Vitória-ES, em parceria com um projeto de extensão universitária. Os encontros eram conduzidos por uma psicóloga e uma graduanda em Psicologia, aconteciam uma vez por semana, com duração média de duas horas e participação de cerca de cinco usuários. Durante esses encontros, o principal recurso utilizado eram imagens extraídas de revistas, com o intuito de auxiliar os usuários a se expressarem e falarem das razões que os levaram a buscar o serviço de saúde. Essa configuração de acolhimento se mostrou bastante potente, uma vez que os participantes se sentiam confortáveis para falar de suas angústias, se acolhiam e trocavam experiências entre si. Ao compartilhar com seus pares, os participantes percebiam que suas queixas não eram só suas, mas que também se concretizavam na vida de outras pessoas que moravam no mesmo território. Neste sentido, os participantes não ocupavam um lugar passivo, mas de sujeitos autônomos e protagonistas de seu processo de saúde. Além disso, tal formato de acolhimento, contribuiu para ampliar as perspectivas a respeito da atuação da Psicologia para além do atendimento clínico tradicional. No entanto, também houve alguns desafios na realização de tal atividade, sobretudo no que diz respeito à condução do grupo e a falta de infraestrutura da Unidade de Saúde da Família.
