Endoscopic ultrasonography is a relatively new endoscopic method of examination to determine the invasion of tumors of the gastrointestinal tract, detection and sizing of pancreatic tumors, diagnosis of chronic pancreatitis, pathology of the biliary tract. The method combines the possibilities of two studies: endoscopic and ultrasound. The study is performed using a video endoscope, at the end of which is a scanning ultrasound sensor. The advantages of endoscopic ultrasound over traditional ultrasound examination through the anterior abdominal wall are that the ultrasound sensor under visual control through the lumen of the digestive tract can be carried out directly to the investigated object. The use of very high frequencies of ultrasound provides high image quality with a resolution of less than 1 mm, inaccessible to other research methods (ultrasound, computer tomography and magnetic resonance imaging, endoscopic cholangiopancreatography).
Резюме. Актуальність. Неалкогольна жирова хвороба печінки (НАЖХП)- ВступНеалкогольна жирова хвороба печінки (НАЖХП) охоплює спектр захворювань, тісно пов'язаних із ме-таболічними факторами ризику. Іншими словами, вона є печінковим проявом метаболічного синдрому. НАЖХП включає стеатоз, неалкогольний стеатогепа-тит (НАСГ) з цирозом або без нього та гепатоцелюляр-ну карциному за відсутності зловживання алкоголем, що визначається як добове споживання алкоголю в пе-рерахунку на чистий спирт більше ніж 30 г для чоло-віків та більше ніж 20 г для жінок [1]. Стеатоз печінки має сприятливий клінічний перебіг, оскільки запаль-не пошкодження печінки не розвивається. Необхідно розрізняти цю форму НАЖХП, що пов'язана зі спри-ятливим довгостроковим прогнозом, і НАСГ, для якого характерно прогресування: приблизно у 20 % пацієнтів із НАСГ розвиваються криптогенний цироз і гепатоце-люлярна карцинома [2].Актуальність проблеми полягає в тому, що НАЖХП є найбільш поширеним захворюванням печінки в за-хідних країнах; патологія охоплює 17-46 % дорос-лого населення з відмінностями залежно від методу діагностики, віку, статі та етнічної приналежності [1]. Резюме. Актуальність. Неалкогольна жирова хвороба печінки (НАЖХП)-хронічне дифузне захворювання печінки, що включає простий стеатоз, неалкогольний стеатогепатит (НАСГ) з можливістю прогресування до цирозу або навіть гепатоцелюлярної карциноми. Відомо багато причин стеатозу печінки, однак НАЖХП асоціюється переважно з інсулінорезистентністю і метаболічним синдромом (МС), що дало підставу розглядати НАЖХП як печінкову маніфестацію останнього. Печінка відіграє ключову роль в обміні жирних кислот (ЖК) і тригліцеридів, забезпечуючи синтез, накопичення, секрецію і окислення ЖК. При властивому МС абдомінальному ожирінні відзначається ексцесивний потік ЖК від вісцерального жиру через портальну вену в печінку з розвитком стеатозу. Накопичення ліпідів у печінці запускає каскад прооксидативних гепатотоксичних подій, що призводять до розвитку НАСГ і можливого його прогресування. Мета: визначення особливостей метаболічних змін на різних етапах розвитку НАЖХП. Матеріали та методи. В дослідження увійшли пацієнти з НАЖХП, які проходили обстеження та лікування у відділенні захворювань печінки та підшлункової залози ДУ «Інститут гастроентерології НАМН України». Усім хворим виконували ультразвукове дослідження органів черевної порожнини, проводили стандартні біохімічні дослідження та визначення в сироватці крові загального холестерину, β-ліпопротеїдів, тригліцеридів, ліпопротеїдів низької щільності, ліпопротеїдів дуже низької щільності, ліпопротеїдів високої щільності, глюкози, інсуліну. Результати. Метаболічні зміни у вигляді інсулінорезистентності та проатерогенних змін ліпідограми, що характерні для всіх хворих із НАЖХП, були більш виражені при стеатозі печінки. Висновки. Отримані результати обумовлюють необхідність обов'язкового дослідження показників ліпідного та вуглеводного обміну у пацієнтів зі стеатозом печінки для своєчасної корекції порушень, що зменшить прогресування НАЖХП.
Non-alcoholic fatty liver disease is characterized by fatty liver infiltration without any another common cause of steatosis (severe alcohol, drugs, etc.). Non-alcoholic fatty liver disease is associated with metabolic risk factors, which are diabetes type 2, dyslipidemia, obesity, and in some cases, it has a genetic predisposition as a main cause. The liver biopsy remains the “gold standard” for assessing the degree of steatosis, necrosis and liver fibrosis. However, non-invasive investigations, especially biochemical markers and visualization methods remain the first-line diagnostic analyses, as well as assessment of the response to treatment. In view of this, the aim of our research was to evaluate the validity of biochemical parameters of liver function and ultrasound in the diagnosis of non-alcoholic fatty liver disease. Patients diagnosed with non-alcoholic fatty liver disease were studied in this research Every patient underwent to both an examination and treatment in the Department of Liver and Pancreatic Diseases at the Institute of Gastroenterology, NAMS of Ukraine. All patients were exposed to ultrasound visualization of the abdominal organs, standard biochemical studies (content analyses of alanine aminotransferase, aspartate aminotransferase, total bilirubin and its fractions, activity of alkaline phosphatase, gammaglutamyltranspeptidase, X-lipoproteins, total protein, albumin, fibrinogen, international normalized ratio) were performed in the blood serum . Increased echogenicity of the liver and distal decrement of ultrasound, as the main ultrasonographic symptoms of liver steatosis, were determined with high incidence in all patients with non-alcoholic fatty liver disease. A number of symptoms (heterogeneity of the echo-structure of the liver of medium and coarse-grained nature, roundness of the lower edge of the liver, inequalities in the liver contour), the frequency of which is more closely related to the severity of inflammatory, as well as fibrotic changes, were observed more often in patients with non-alcoholic steatohepatitis and cirrhosis compared with steatosis. The deterioration in the visualization of small branches of the liver veins was determined as a result of the smoothness of the vascular pattern and its depletion. Moreover, the results showed an increment of the spleen volume, along with the enlargement of the splenic vein of patients with cirrhosis. All observed changes were considered as a component of portal hypertension and were induced with fibrotic transformation of the liver. The lack of correlation of the degree of fibrosis with the content of transaminases confirms the low diagnostic significance of these indicators. Nevertheless, the moderate direct correlation of the determined ultrasonographic indexes with degree of the fibrosis in the liver indicates the possibility of using this method for screening non-alcoholic fatty liver disease.
Патологія печінки і жовчовивідної системи / Pathology of Liver and Biliary Excretion System Оригінальні дослідження Original Researches Вступ Ураження печінки є одними з найбільш частих позакишкових проявів хронічних неспецифічних захворювань кишечника та виявляються у більш ніж третини пацієнтів. Деякі патологічні стани печінки, що виникають при неспецифічному виразковому коліті (НВК) та хворобі Крона (ХК), виходять на перший план у клінічній картині цих нозологій і визначають їх подальший несприятливий прогноз [1-4]. В роботах, присвячених питанню гепатобіліарної патології при хронічних запальних захворюваннях кишечника (ХЗЗК), не існує єдиної думки щодо механізму виникнення ураження печінки у таких хворих, чітко не визначені їх нозологічні форми. Ураження печінки пов'язано не стільки з тяжкістю захворювань кишечника, скільки з їх хронічним безперервним перебігом, який супроводжується порушенням обмінних процесів [5-8].
