Aim: To determine frequency of tumors with immunohistochemical markers of cancer stem cells (CSC) CD44+/CD24− in patients with breast cancer (BC) of different molecular subtype and to evaluate their prognostic value. Object: Surgical material of 132 patients with BC stage I–II, age from 23 to 75 years, mean age — 50.2 ± 3.1 years was studied. Methods: Clinical, immunohistochemical (expression CD44+/CD24−), morphological, statistical. Results: BC is characterized by heterogeneity of molecular subtypes and expression of markers (CD44+/CD24−). Immunohistochemical study of expression of CSC markers in surgical material has detected their expression in 34 (25.4%) patients with BC of different molecular subtypes. The highest frequency of cells with expression of CSC marker was observed in patients with basal molecular subtype (44.8% patients). Most of BC patients with phenotype CD44+/CD24 had stage I of tumor process (34.3%). Statistical processing of data has showen that Yule colligation coefficient equaled 0.28 (р > 0.05) that argues poor correlation between stage of tumor process and number of tumors with positive expression of CSC markers. Statistical processing of data has showen high correlation between presence of cells with expression of CSC markers and metastases of BC in regional lymph nodes (Yule colligation coefficient equals 0.943; р < 0.5). Difference in overall survival of patients with BC of basal molecular subtype depending on expression of CSC CD44+/CD24− markers was detected. Survival of patients with basal BC was reliably higher at lack in tumors of cells with CSC markers CD44+/CD24− and, correspondingly, lower at presence of such cells (р < 0.05). In patients with BC of luminal (A and B), HER-2-positive subtypes, significant change in survival of patients depending on expression of CSC markers was not determined (р > 0.05). Conclusion: Significance of tumor cells with markers CD44+/CD24− within the limits of molecular subtype of BC may be additional criterion for advanced biological characteristic of BC, and in patients with BC of basal molecular subtype — for predictive evaluation of individual potential of tumor to aggressive clinical course.
Background: The aim of this prospective randomized trial was to investigate the influence of metformin on the effectiveness of neoadjuvant systemic therapy in breast cancer patients with metabolic syndrome. Patients and methods: The study included 72 patients (from 31 to 77 years of age) who received neoadjuvant systemic treatment for stage II-III breast cancer at the National Medical University named after O.O Bogomolets, Municipal City Clinical Oncological Center, Department of Oncology, Kyiv during 2010-2014. Metabolic syndrome was diagnosed in all patients according to the International Diabetes Federation criteria. They were divided in two groups: group 1 that included 36 patients with metabolic syndrome and breast cancer who did not intake metformin during neoadjuvant systemic therapy, and group 2 that included 36 patients with metabolic syndrome and breast cancer who received metformin with neoadjuvant systemic therapy. Results: Complete clinical response was identified in 2 (6%) patients from group 1 and 10 (27.5%) patients from group 2. Overall clinical response rate (cCR + cPR) was achieved in 28 (77.5%) patients treated with metformin compared to 9 (25%) patients from group 1. A stable disease was observed in 19 (53%) patients who were not taking metformin. The rate of pathological complete response was 26.5% (9 patients) in the metformin group and 6% (2 patients) in the non-metformin group. Conclusions: Combined neoadjuvant systemic anticancer therapy of breast cancer patient with metabolic syndrome with metformin has a higher clinical and pathological overall response rate than treatment without metformin.
ОГЛЯДИ 12Клінічна ендокринологія та ендокринна хірургія 2 (47) 2014 ВСТУП Рак молочної залози (РМЗ) є одним із най-поширеніших онкологічних захворювань в Україні. За даними Національного канцер-реєстра України, стандартизований показник захворю ваності на РМЗ 2012 року склав 67,1 випадку на 100 тис. жіночого населення. Хоча захворюваність на злоякісні ново-утворення молочних залоз постійно збільшується, смертність від них має тенденцію до зменшення [1]. З біологічної точки зору РМЗ -це неоднорідна гру-па пухлин, які відрізняються за клінічним перебігом і чутливістю до лікування. Проте, навіть, морфо-логічно аналогічні пухлини мають різну чутливість до медикаментозної та променевої терапії, що визначає їх клінічний перебіг. Ці відмінності обу-мовлено обмеженістю морфологічної класифікації РМЗ для визначення прогнозу перебігу й тактики медикаментозного лікування. Дослідження екс-пресії генів клітинами РМЗ та їх кореляції з феноти-повими проявами дозволили доповнити морфо-логічну класифікацію шляхом виділення молеку-лярних підтипів. На підставі імуногістохімічного фенотипу карциноми молочної залози можна класифікувати мінімум на 5 молекулярних підтипів, які відрізняються між собою за прогнозом перебігу, відповіддю на медикаментозну терапію і показни-ками загального та безрецидивного виживання хворих [34,[44][45][46]. Молекулярні підтипи РМЗ, які мають принципове клінічне значення, згідно з рекомендаціями European Society of Medical Oncology (ESMO) наведено у табл. 1 [42].Молекулярний підтип РМЗ є провідним чинни-ком у визначенні тактики системної протипухлинної та/або цільової терапії. Проте у зв'язку з тим, що деякі пухлини характеризуються терапевтичною резистентністю та/або не мають потенційних мішеней для цільової терапії, існуючі критерії про-гнозу перебігу та визначення тактики лікування хворих на РМЗ вимагають доповнення. Останнім часом з'являється велика кількість досліджень, результати яких свідчать про прогностичне та тера-певтичне значення наявності рецепторів андрогенів (АР) у карциномах молочної залози [6,15,27,35,39,43]. Даний огляд літератури має на меті визначити прогностичний і терапевтичний потенціал АР у хво-рих на РМЗ. Summary. Brain metastases of solid tumors are the most common intracranial neoplasms in adults. We provide a short overview of the role of the blood-brain barrier in the pathogenesis of breast cancer brain metastases, and the effectiveness of systemic anticancer therapy in the treatment of such patients.
