Introdução: A imobilidade no leito de pacientes em unidades de terapia intensiva (UTI) favorece o maior tempo de permanência na unidade. Objetivos: Comparar a funcionalidade na admissão e na alta da UTI segundo a especialidade médica e correlacionar a funcionalidade na alta com o tempo de internação na UTI. Métodos: Estudo exploratório, longitudinal, realizado na UTI Geral de um hospital escola. Os pacientes foram divididos em grupos de acordo com a especialidade de internação: neurologia, pneumologia, gastroenterologia, politrauma e outros. O estado funcional prévio à internação foi obtido por meio da medida de independência funcional (MIF). Na alta foi aplicada a escala de mobilidade funcional em UTI (EMU). Resultados: Amostra de 174 pacientes, 53% do sexo masculino. O grupo politrauma apresentou idade significativamente menor que os outros grupos (p < 0,0001). O tempo médio de internação para os grupos neurologia e pneumologia apresentou correlação inversamente proporcional com p = 0 ,02 / r = -0,5 e p = 0,009 / r = -0,4 respectivamente. Não houve diferença significativa entre as médias da MIF na admissão (p = 0,11) e da EMU na alta (p = 0,24) entre as especialidades médicas em que os pacientes foram admitidos. Conclusão: Quanto maior o tempo de internação na UTI, menor a funcionalidade na alta em pacientes neurológicos e pneumopatas.
Abstract Generalized muscle weakness when related to the critical patient is an important and common complication in patients admitted to the intensive care unit (ICU). It is known that inactivity can lead to secondary dysfunctions and the main system is the osteomyelitis that may undergo muscle strength decrease by up to 30% in 7 days, and 20% in each additional week. The objective of the study was to correlate the functional status at admission with functional mobility at ICU discharge. It is an exploratory, longitudinal study developed in a school hospital. Patients underwent evaluation through functional independence measure (FIM) at admission. Afterwards, an early evolutionary mobilization protocol was applied, divided into three phases. At the time of discharge, the patients were reassessed by the functional mobility scale in the ICU (FMS). There was a significant (p=0.0001) correlation (r=0.5) between the admission FIM and the FMS at the time of ICU discharge from the critical patients included. There was a significant correlation (p<0.0001) positive (r=0.5) in the female patients and positive and weak (r=0.4) in the male patients. There was a correlation between the functional status at admission and the level of mobility at discharge in patients admitted to the ICU, i.e. , the higher the functionality before admission, the greater the functional mobility at discharge. Physical Therapy Specialty Resumo A fraqueza muscular generalizada quando relacionada ao doente crítico é uma complicação importante e comum em pacientes internados em unidade de terapia intensiva (UTI). Sabe-se que a inatividade pode acarretar disfunções secundarias e o principal sistema acometido é o osteomioarticular que pode sofrer diminuição da força muscular em até 30% em 7 dias, e 20% a cada semana adicional. O objetivo do estudo foi correlacionar o status funcional na admissão com a mobilidade funcional na alta de pacientes na UTI. Trata-se de um estudo do tipo exploratório, longitudinal, desenvolvido em um hospital-escola. Os pacientes foram submetidos a avaliação por meio da medida de independência funcional (MIF) na admissão. Após, foi aplicado um protocolo de mobilização precoce evolutivo dividido em três fases. No momento da alta, os pacientes foram submetidos a uma nova avaliação pela escala de mobilidade funcional em UTI (EMF). Verificou-se correlação significativa (p<0,0001) positiva (r=0,5) entre a MIF de admissão e a EMF no momento da alta da UTI dos pacientes críticos incluídos. Houve correlação significativa (p<0,0001) positiva (r=0,5) nas pacientes do sexo feminino e positiva e fraca (r=0,4) nos pacientes do sexo masculino. Houve correlação entre o statusfuncional na admissão com o nível de mobilidade na alta de pacientes internados em UTI, isto é quanto maior a funcionalidade antes da internação, maior a mobilidade funcional na alta. Introdução: A traqueostomia é um procedimento cirúrgico e uma alternativa mais segura para os pacientes com tempo de intubação prolongado ou falha no desmame ventilatório, pois facilita o desmame, possibilitando um menor tempo de ventilação mecânica. Objetivo: Documentar os pacientes em ventilação mecânica invasiva que necessitaram de traqueostomia e verificar a relação do procedimento com a clínica dos pacientes que foram a ele submetidos. Métodos: Pesquisa quantitativa, descritiva e transversal realizada a partir da coleta de dados de prontuários de pacientes submetidos a traqueostomia em uma Unidade de Terapia Intensiva (UTI) durante os meses de novembro de 2019 a agosto de 2020. Resultados: Foram incluídos 67 pacientes de ambos os sexos internados em uma Unidade de Terapia Intensiva que permaneceram sob intubação orotraqueal por 48 horas ou mais, divididos em dois grupos: cirúrgico e clínico. Em relação aos dias de ventilação mecânica antes da traqueostomia, esse período foi menor no grupo cirúrgico do que no grupo clínico (respectivamente: mediana = 8 e 12,5 dias; intervalo interquartil 6-12 e 8-14,5; p=0,0109). A principal indicação do procedimento no grupo cirúrgico foi de traqueostomia precoce por diagnóstico com perspectiva de ventilação mecânica prolongada (42,4%) e na clínica foi de nível de consciência inadequado para extubação (47,1%). Não houve diferença entre os grupos (p=0,3102) quanto aos dias de ventilação mecânica após o procedimento, grupo cirúrgico (mediana = 5 dias; intervalo interquartil 2-8) e grupo clínico (mediana = 4 dias; intervalo interquartil 2-7). Conclusão: Nível de consciência inadequado para extubação e opção por traqueostomia precoce em patologia com perspectiva de ventilação mecânica prolongada foram as principais indicações do procedimento em nossa Unidade. O tempo de ventilação mecânica invasiva antes da traqueostomia foi maior no grupo de pacientes clínicos do que no grupo de pacientes cirúrgicos.
