Bipolinis afektinis sutrikimas (maniakinė depresija) yra psichikos sveikatos būklė, kuriai būdingi pasikartojantys (mažiausiai du) nuotaikos ir aktyvumo lygio pakitimo epizodai. Jų metu nuotaika tampa pakili, padidėja energija bei aktyvumas (manija ar hipomanija), kartais nuotaika pablogėja, o energija bei aktyvumas sumažėja (depresija). Stipri išankstinė neigiama nuostata, stereotipinis apibūdinimas, socialinė atskirtis, statuso praradimas ir diskriminacija lėmė, jog stigmatizacija tapo viena opiausių problemų, su kuria susiduria bipoliniu afektiniu sutrikimu ir kitomis psichichikos ligomis sergantys asmenys. Šiame straipsnyje aptariamos įvairios diskriminacijos apraiškos, kurias dažniausiai patiria bipolinį afektinį sutrikimą turintys pacientai, kaip antai: ribojamos darbo galimybės bei profesinis tobulėjimas, sutrikdoma socialinė integracija, šie asmenys nepripažįstami visaverčiais bendruomenės nariais, jų šalinamasi, dažnai manoma, kad jie visą gyvenimą turi gyventi su tėvais, negali susirasti partnerio ir sukurti savo šeimos, išlaikyti stabilios finansinės padėties. Platesnius tyrimus šioje srityje sunkina psichosocialinės būsenos objektyvizavimas. Nors ilgus metus šią ligą besigydantys pacientai susilaukia daugiau negatyvaus vaizdavimo, tikėtina, jog jie turi daugiau vidinių išteklių įveikti kylančius sunkumus, nes ilgiau taiko išmoktas psichoterapijos metodikas. Pabrėžtina, kad nuolatinis, tęstinis psichiatrinio gydymo užtikrinimas, psichoterapijos užsiėmimai, šeimos palaikymas ir socialinis įsitraukimas į visuomeninę veiklą yra esminiai veiksniai, kurie neabejotinai prisideda prie sergančiųjų bipoliniu afektiniu sutrikimu gyvenimo kokybės bei klinikinės būklės gerinimo.
Mastocitozė – reta liga, kuriai būdinga vieno ar daugiau organų infiltracija plazminėmis ląstelėmis – mastocitais. Odos mastocitozė sudaro 85–90 proc. visų vaikams diagnozuojamų mastocitozės atvejų. Ji pasižymi pastoviais, nenykstančiais rudai-raudonos spalvos bėrimo elementais, atsirandančiais dar kūdikystėje, bet kurioje kūno dalyje. Mastocitoze sergančius vaikus vargina įvairaus stiprumo odos niežulys (83% atvejų), atsiranda veido raudonis (65% atv.), pūkšlinis bėrimas (53% atv.), diskomfortas epigastriumo srityje (41% atv.), kaulų laužymo pojūčiai (18% atv.), rečiausiai − galvos svaigimas (iki 12% atv.), dažnesnis mokyklinio amžiaus vaikams. Ligos eiga daugumai vaikų – gerybinė. Simptomai regresuoja paauglystėje, brendimo laikotarpiu. Daliai vaikų (5–30%), sergančių odos mastocitoze, simptomai išlieka ir gyvenimo eigoje liga progresuoja į sisteminę mastocitozę. Šios ligos diagnostika paremta apžiūros duomenimis, specifiniais klinikiniais požymiais ir galutinai patvirtinama biopsinės medžiagos histopatologinio tyrimo išvada. Serumo triptazės lygis koreliuoja su mastocitų proliferacijos laipsniu, todėl reikšmingas sisteminės mastocitozės diagnostikai. Įtarus sisteminį ligos plitimą, kaulų čiulpų biopsija vaikams atliekama itin retai, dažniau skiriama reguliari kontrolinė gydytojo dermatovenerologo apžiūra.
Europos šalyse ir Jungtinėse Amerikos Valstijose mirštamumas nuo kasos vėžio užima ketvirtą vietą tarp visų onkologinių ligų. Sergančiųjų kasos vėžiu 5 metų išgyvenamumas vidutiniškai siekia apie 5-10 procentų. Daugeliu atvejų ši liga aptinkama jau vėlyvose stadijose, tad chirurginis gydymas tėra galimas vos 15-20 proc. pacientų. Nustačius rezektabilų kasos vėžį, rekomenduojama gydymą pradėti skiriant neoadjuvantinę chemoterapiją, atlikti naviko rezekciją, po operacinio gydymo taikyti adjuvantinę chemoterapiją. Potencialiai rezektabilius navikus siūloma gydyti chemoterapijos preparatais. Naujausi šaltiniai nurodo, jog gydymas chemoradioterapija pasižymi teigiamu klinikiniu atsaku, ypač tais atvejais, jei navikas kontaktuoja su didžiosiomis arterijomis. Lokaliai išplitusio kasos vėžio gydymo planas sudaromas skiriant chemoterapijos kursą, po kurio, įvertinus klinikinį atsaką, galima papildomai skirti chemoradioterapinį gydymą. Ligos stadijai pažengus, išsivysčius metastatinei ligai, priklausomai nuo pacientų būklės, jiems taikytinas paliatyvusis gydymas arba įvairaus agresyvumo chemopreparatai.
