Trata-se de artigo que tem por finalidade discutir a realidade dos Hospitais de Custódia e Tratamento Psiquiátrico (HCTP) e a reforma antimanicomial, conferindo enfoque ao HCTP da Bahia. Com efeito, a pesquisa possui a seguinte pergunta orientadora: é possível compatibilizar a medida de segurança com a perspectiva da reforma antimanicomial, em especial o Hospital de Custódia e Tratamento da Bahia? Objetivando respondê-la, através da revisão bibliográfica, com o emprego do método hipotético-dedutivo de Karl Popper, buscou-se no primeiro capítulo abordar o conceito de vidas matáveis, da anulação dos indivíduos e a estigmatização das pessoas com sofrimento mental que cumprem medida de segurança. Em seguida, a partir da análise do entendimento jurisprudencial dos tribunais superiores, da previsão legal e dos dados secundários produzidos (relatórios), observou-se a realidade dos HCTP´s no Brasil e particularmente o da Bahia para, enfim, analisar a reforma antimanicomial e responder à pergunta problema do artigo, concluindo pela ausência de interesse social em se “derrubar os muros” e mudar a cultura segregadora e punitivista doas medidas de segurança.
