OBJETIVO: O estudo objetivou avaliar subsídios científicos sobre o manejo da epilepsia durante a gestação. METODOLOGIA: Foi realizada uma revisão integrativa da literatura nos bancos de dados SciELO e PUBMED sobre a fisiopatologia da epilepsia, sobre seu tratamento e sobre seu manejo clínico durante a gestação. Dos 57 estudos encontrados,foram utilizados 19 estudos que estiveram alinhados aos objetivos e situados no recorte temporal de 2016 a 2020. RESULTADOS: As drogas antiepiléticas clássicas induzem teratogenia em alto grau, como ácido valpróico e por conta disso deve ser evitado. Por outro lado, medicações mais recentes (exceto o Topiramato) como a Gabapentina têm baixo risco de teratogenia. A troca de medicações deve ser evitada sem necessidade, pois pode causar sobreposição de medicações e exposição do feto a dosagens altas de anticonvulsivantes. CONCLUSÃO: O tratamento da epilepsia é fundamental para o curso satisfatório da gestação, contudo, deve ocorrer de forma cautelosa e com acompanhamento médico rigoroso.
