Introdução: A hiperplasia condilar descreve a condição patológica causada por um excessivo crescimento do côndilo mandibular, tendo maior prevalência entre os 10 e os 20 anos de idade — o que pode ser atribuído aos efeitos da puberdade e ao aumento exponencial da taxa de crescimento corporal. A assimetria facial pode se mostrar mais aparente nessa idade e, com isso, se inicia a procura pelo tratamento, levando em conta que diversas formas de tratamento já foram propostas para esse tipo de patologia. Objetivo: Relatar um caso clínico, a fim de discutir os protocolos atuais de tratamento para a hiperplasia condilar. Relato de caso: Paciente com 20 anos de idade, leucoderma, sexo feminino, encaminhada ao serviço de Cirurgia e Traumatologia Bucomaxilo-facial da Associação Beneficente do Hospital Santa Casa de Campo Grande/MS, com histórico de assimetria facial. Por meio do exame físico, evidenciou-se assimetria facial severa e laterognatismo importante, com desvio para a esquerda, sendo solicitados exames complementares. Após diagnóstico de hiperplasia condilar mandibular, a paciente foi submetida a condilectomia proporcional e, em um segundo tempo, cirurgia ortognática. Conclusão: A utilização da condilectomia proporcional, com objetivo de trata- mento da assimetria facial causada pela hiperplasia condilar unilateral, pode se mostrar satisfatória, tendo em vista sua reduzida invasividade e boa taxa de sucesso.
