Introducción: El consumo de drogas en Colombia está aumentado y su tamización es baja. La Organización Mundial de la Salud diseñó el ASSIST (Alcohol, Smoking and Substance Involvement Screening Test) para determinar la prevalencia y el riesgo del consumo. El objetivo fue describir los usos del ASSIST en Colombia y el perfil de consumo de las poblaciones estu diadas con esta escala. Métodos: Revisión sistemática en Medline-Pubmed, Science-Direct, Scielo y Google-Scholar. Se garantizó exhaustividad, reproducibilidad y evaluación de la calidad. Se realizó sín tesis cualitativa mediante proporciones. Resultados: Se incluyeron nueve estudios con 4 736 sujetos. La mayor proporción de sujetos presentó un consumo de bajo riesgo; en estudiantes fue 16,1 % para tabaco, 18,9 % alcohol y 17,9 % cannabis; entre líderes sociales fue 100 % en tabaco y cannabis, y 80 % alcohol; en desplazados 35,8 % para alcohol y 6,6 % cannabis; y en gestantes 58,4 % tabaco y 58,4 % alcohol. Los factores asociados fueron el desplazamiento forzado, violencia sexual, edad, escolaridad, depresión, estrato social y bajo soporte socio-familiar. Conclusión: Se evidencia la necesidad de mejorar la tamización del uso de drogas en el país, dado que una detección temprana es clave para articular acciones clínicas, epidemiológicas y de política pública.
Las erupciones cutáneas están dentro de las reacciones adversas medicamentosas más frecuentes; por lo tanto, es habitual enfrentarse a estas en los diferentes ámbitos del ejercicio médico. Un enfoque ordenado basado en la identificación temprana del posible agente etiológico, la cronología de la exposición y las características clínicas de la reacción permiten categorizar la gravedad del cuadro. Con una sospecha clara puede optimizarse la toma de paraclínicos, elegirse el nivel de complejidad asistencial más adecuado y tomarse las medidas terapéuticas y preventivas pertinentes para cada caso. En la siguiente revisión narrativa se pretende abarcar los aspectos clave de las erupciones medicamentosas más graves y proponer un enfoque simple de estas en el servicio de urgencias.
Introducción. El sangrado en contexto de anticoagulación es uno de los riesgos que tienen ciertos pacientes durante determinados tratamientos, y es potencialmente mortal. Es importante conocer la farmacocinética de estas moléculas para predecir cuál será su comportamiento. Además se requiere juicio clínico en todos los casos de sangrado para determinar el manejo de acuerdo a la severidad. El objetivo de esta revisión es presentar un enfoque del paciente anticoagulado con sangrado en el servicio de urgencias. Temas tratados. Farmacocinética y farmacodinámica, generalidades, sangrado mayor y no mayor, opciones de reversión y tratamiento. Conclusión. En presencia de anticoagulantes orales directos (DOACS) se debe evaluar la gravedad del sangrado y el grado de anticoagulación, pues las estrategias de manejo se orientan dependiendo de si se trata de un sangrado mayor o menor.
