Objetivo: descrever o perfil epidemiológico de gestantes de alto risco, com determinantes primários para near miss materno. Método: trata-se de um estudo quantitativo, realizado em uma instituição pública. A coleta de dados ocorreu nos meses de outubro de 2016 a agosto de 2017. Os dados foram compilados no programa SPSS® versão 20.0. A pesquisa foi aprovada com parecer: 1.757.596, CAAE: 59935716700005231. Resultados: participaram do estudo 319 mulheres, 70,5 % com idade de 20 a 35 anos, 58,6% da raça branca. Em relação as características obstétricas 38,2% eram primíparas, 52,0% dos casais não tinham planejado a gestação, 99,4 % realizaram o pré-natal e realizaram mais de seis consultas (92,5%), quanto a resolução da gestação 60, 2% foram realizados cesariana. Entre os determinantes primários de near miss predominou-se: hipertensão (55,5%) e infecção (16,9%), seguidos por distúrbios respiratórios (6,9%); neurológicos (6,5%); cardíacos (6,3%) e hemorrágicos ou de coagulação (4,4%); e entre os menos expressivos estão as disfunções: renais (2,5%) e hepáticas (0,9%). Conclusão: O estudo foi importante para conhecer as gestantes que apresentam determinantes primários para near miss e assim adequar a assistência. Neste sentido, ações para o entendimento da mulher na prevenção de riscos maternos e neonatais, devem ser elaboradas e estimuladas, almejando prevenção e promoção da saúde entre as mulheres.
Objetivo: identificar as práticas obstétricas utilizadas no trabalho de parto e parto em parturientes classificadas como de alto risco Método: estudo quantitativo. A população foi constituída por puérperas classificadas como de alto risco. A coleta de dados ocorreu durante outubro de 2016 a agosto de 2017. Utilizou-se SPSS® 20.0. O estudo foi aprovado com número do parecer 1.757.596 Resultados: analisou-se 319 mulheres. O perfil prevalecido foram mulheres brancas (58,7%), com idade entre 20 a 35 anos (70,7%). A maior parte, realizou acompanhamento pré-natal (99,3%) e eram primíparas (40,4%). As patologias mais frequentes foram hipertensão (50,0%) e doenças associadas ao metabolismo (22,3%). Sobre a monitoração do trabalho de parto, 83,3% de ausculta do batimento cardio-fetal. A cesariana prevaleceu entre 60,8% das mulheres. Em relação aos métodos não farmacológicos para alívio da dor os mais utilizados foram: banho quente de aspersão (37,9%), bola para exercícios perineais (25,0%) Conclusão: todas as práticas obstétricas além de garantir uma evolução segura proporcionam uma experiência positiva sobre o parto, mesmo que a classificação seja de alto risco.Abstract: to identify the obstetric practices used in labor and delivery in parturients classified as high risk was the objective of the present study. Method: quantitative study. The population consisted of postpartum women classified as high risk. Data collection took place from October 2016 to August 2017. SPSS® 20.0 was used. The study was approved with opinion number 1,757,596 Results: 319 women were analyzed. The prevailing profile was white women (58.7%), aged between 20 and 35 years (70.7%). Most underwent prenatal care (99.3%) and were primiparous (40.4%). The most frequent pathologies were hypertension (50.0%) and diseases associated with metabolism (22.3%). About the monitoring of labor, 83.3% of auscultation of the fetal heartbeat. Cesarean section prevailed among 60.8% of women. Regarding non-pharmacological methods for pain relief, the most used were: hot spray bath (37.9%), perineal exercise ball (25.0) Conclusion: all obstetric practices, in addition to ensuring a safe evolution, provide an experience positive about childbirth, even if the classification is high risk.Keywords: Humanization of assistance. Aim: to compare the prevalence of exclusive breastfeeding (EBF) and to verify the factors associated with its interruption in premature infants hospitalized at Baby-Friendly hospitals. Method: this is a comparative study performed with 107 premature infants admitted to the neonatal unit of two hospitals in Paraná. Data were obtained through interviews with mothers and analysis of infant records at hospital discharge and 15 days after delivery. Results: the prevalence of EBF was 29.2% in the University Hospital and 15.3% in the Philanthropic Hospital. At hospital discharge, associations with clinical characteristics of infants predominated, and 15 days after discharge there was a greater association with the socio-demographic characteristics of the parents. Conclusion: the prevalence of EBF, which was shown to be low at the time of hospital discharge in both hospitals, decreased after discharge, reinforcing the need for other strategies, in addition to those provided at Baby-Friendly hospitals, for the promotion and support of EBF in preterm infants.
