Objetivo: identificar os fatores limitantes para a efetivação do processo de desospitalização e do funcionamento da atenção domiciliar no Brasil e seus fatores associados. Métodos: Realizou-se uma revisão integrativa da literatura com caráter qualitativo, com a seguinte questão norteadora: “quais são os fatores limitantes para a efetivação da desospitalização no Brasil, qual a contribuição da assistência humanizada nesse processo e quais as dificuldades e potencialidades dessa estratégia para a continuidade do cuidado?”. Utilizou-se como descritores em ciências da saúde (DECS) “Desospitalização” AND “Atenção domiciliar”, no mecanismo de dispositivo de busca do google acadêmico (Google Scholar). A amostra final é formada por 11 estudos. Resultados: as principais evidências encontradas na literatura relacionadas com a desospitalização relacionam-se com a individualização na organização e planejamento desse processo e as condutas da equipe multiprofissional voltadas para a elaboração de um plano terapêutico direcionado para a assistência domiciliar com aproximação entre profissionais da equipe multiprofissional e a família, promovendo orientações e treinamentos. Fatores como o reconhecimento do usuário, cuidador e família, identificar dificuldades e buscar meios para superá-las de forma eficaz e criativa são essenciais para garantir a funcionalidade da atenção domiciliar. Considerações Finais: O processo de desospitalização mostrou-se complexo e dependente de relações entre a gestão da instituição, os profissionais de saúde, os usuários, seus familiares e a rede de atenção à saúde (RAS). Observou-se que durante o processo de desospitalização, a família deve ser valorizada e receber atenção e apoio por parte da equipe interdisciplinar, além do treinamento necessário para atuar como cuidadores capacitados.
