Global concern broke out in late 2015 as thousands of children in Brazil were born with microcephaly, which was quickly linked to congenital infection with Zika virus (ZIKV). ZIKV is now known to cause a wider spectrum of severe adverse outcomes—congenital Zika syndrome (CZS)—and also milder impairments. This study aimed to explore the social and economic impacts of CZS in Brazil. Data was collected through mixed methods across two settings: Recife City and Jaboatão dos Guararapes in Pernambuco State (the epicentre of the epidemic), and the city of Rio de Janeiro (where reports of ZIKV infection and CZS were less frequent). Data was collected May 2017-January 2018. Ethical standards were adhered to throughout the research. In-depth qualitative interviews were conducted with: mothers and other carers of children with CZS (approximately 30 per setting), pregnant women (10-12 per setting), men and women of child-bearing age (16-20 per setting), and health professionals (10-12 per setting). Thematic analysis was undertaken independently by researchers from at least two research settings, and these were shared for feedback. A case-control study was undertaken to quantitatively explore social and economic differences between caregivers of a child with CZS (cases) and caregivers with an unaffected child (controls). We aimed to recruit 100 cases and 100 controls per setting, from existing studies. The primary caregiver, usually the mother, was interviewed using a structured questionnaire to collect information on: depression, anxiety, stress, social support, family quality of life, health care and social service use, and costs incurred by families. Multivariable logistic regression analyses were used to compare outcomes for cases and controls. Costs incurred as a result of CZS were estimated from the perspective of the health system, families and society. Modelling was undertaken to estimate the total economic burden of CZS from those three perspectives.
OBJETIVO: Este estudo avalia a associação entre fatores sociodemográficos, características maternas, organização dos serviços de saúde e near miss neonatal em maternidades públicas e privadas do Brasil. MÉTODOS: Trata-se de uma coorte prospectiva de nascidos vivos da pesquisa Nascer no Brasil, realizada entre 2011 e 2012. As variáveis foram estabelecidas a partir da literatura e organizadas em três níveis: distal, intermediário e proximal. A avaliação foi realizada a partir dos resultados das análises bivariadas e de seus respectivos valores-p, com nível de significância < 0,20, pelo teste de Wald. Para a análise multivariada, foram inseridas as variáveis contidas no nível distal, conservadas no modelo quando significativas (p < 0,05). O mesmo foi feito no ajuste dos níveis intermediário e proximal. RESULTADOS: No nível distal, nenhuma variável apresentou associação significativa com o desfecho. No nível intermediário, idade da mãe maior ou igual a 35 anos (risco relativo – RR = 1,32; IC95% 1,04–1,66), parto cesáreo (RR = 1,34; IC95% 1,07–1,67), uso de fumo (RR = 1,48; IC95% 1,04–2,10), síndrome hipertensiva gestacional (RR = 2,29; IC95% 1,98–3,14), diabetes pré-gestacional (RR = 2,63; IC95% 1,36–5,05) e gestação gemelar (RR = 2,98; IC95% 1,90–4,68) foram variáveis associadas ao desfecho. No nível proximal, o pré-natal não adequado (RR = 1,71; IC95% 1,36–2,16) e a localização do hospital/maternidade na capital (RR = 1,89; IC95% 1,40–2,55) foram associados ao near miss neonatal. CONCLUSÃO: Os resultados mostram que o near miss neonatal foi influenciado pelas variáveis da organização dos serviços de saúde e pelas características maternas.
Resumo O objetivo deste artigo é identificar os efeitos das morbidades maternas sobre a composição nutricional do leite humano. Revisão sistemática da literatura utilizando protocolo PRISMA para a busca, seleção e extração de dados. De acordo com o fluxograma proposto a busca bibliográfica resultou em 1.582 artigos e, destes, 14 foram selecionados. Os artigos elegidos para a presente revisão foram publicados entre 1987 e 2016. Nove estudos verificaram diferenças significativas na composição nutricional do leite de mães com diabetes mellitus, hipertensão arterial ou excesso de peso em comparação ao grupo controle. A maioria dos estudos demonstrou que a presença de doenças crônicas acarreta modificações na composição nutricional do leite humano. Os principais resultados evidenciaram menor concentração de lactose e gordura no leite de mulheres com diabetes mellitus. Quanto a hipertensão, foi verificado níveis mais elevados de proteína total no colostro e no leite maduro. Para o excesso de peso, foi observado maior teor de gordura e energia. É de extrema importância que essas mulheres tenham acompanhamento nutricional contínuo a fim de minimizar o impacto dessas morbidades sobre a composição nutricional do leite humano.
