Prevalence of severe acute maternal morbidity was high, and was related especially to hospitalization and to newborn variables. The criterion for identifying cases and the use of the National Hospital Information System proved to be useful for monitoring maternal morbidity and mortality and increasing our knowledge of its related aspects, contributing to the improvement of the quality of pregnancy and delivery care.
OBJETIVOS: identificar os códigos de procedimentos constantes no Sistema de Informação Hospitalar - Sistema Único de Saúde (SIH-SUS) considerados como Morbidade Materna Extremamente Grave (MMEG) e construir um algoritmo para o manejo/preparação da base de dados de Autorização de Internação Hospitalar visando a captura dos casos de MMEG neste banco de dados. MÉTODOS: utilizaram-se os dados do SIH-SUS fornecidos pela Secretaria de Saúde de Juiz de Fora e referem se as internações ocorridas no SUS no período de 2006-2007. Foram selecionadas todas as internações cujo diagnóstico principal compreendia todo o capítulo XV da Classificação Internacional de Doenças (CID10) - Complicações da gravidez, aborto, parto e puerpério e/ou internações em que procedimentos obstétricos foram realizados. Para identificação dos procedimentos no SIH-SUS, considerados como MMEG, adotaram-se os critérios propostos pela Organização Mundial da Saúde. O algoritmo foi desenvolvido no software Microsoft Access. RESULTADOS: foram capturados 326 casos de MMEG entre as 8620 mulheres selecionadas, perfazendo uma taxa de 37,8/1000 mulheres. Os procedimentos mais frequentes foram transfusão de hemoderivados, "permanência a maior" e pré-eclampsia grave/eclâmpsia, com prevalências de 15,7/1000, 9,5/1000 e 8,2/1000, respectivamente. CONCLUSÕES: o algoritmo utilizado pode otimizar o uso do SIH-SUS para a captação dos casos de MMEG e gerar informações para os serviços de vigilância da morbimortalidade materna e avaliação de cuidados obstétricos.
Resumo Introdução Apesar das ações para prevenção, o câncer de mama (CAM) no Brasil apresenta elevada mortalidade, provavelmente devido à identificação do tumor em estádios avançados. Objetivo Analisar a mortalidade por CAM nas microrregiões de saúde de Minas Gerais (MG), de 2013 a 2017 e sua possível associação com a desigualdade social. Método Estudo ecológico, cuja unidade de análise foram as microrregiões de saúde de MG. Dados de mortalidade, sociodemográficos e de saúde foram extraídos do SIM, IBGE, PROADESS e DATASUS. Foram calculadas taxas de mortalidade específicas e padronizadas por idade, construídos mapas temáticos e realizadas análises estatísticas utilizando o Índice de Moran e a regressão simples e múltipla. Resultados De 2013 a 2017, ocorreram em MG 7.571 óbitos por CAM. As microrregiões com maior mortalidade estão localizadas no Centro e Leste e, com menor, no Norte e Nordeste. A maioria das variáveis apresentaram alto coeficiente de variação e foram significativas no modelo de regressão linear simples. Nos modelos múltiplos distal e proximais, somente o grau de urbanização foi significativa. Todas as variáveis apresentaram autocorrelação espacial significativa e dependência espacial. Conclusão Altas taxas de mortalidade nas microrregiões mais urbanizadas podem ser explicadas por fatores reprodutivos, comportamentais e distribuição dos recursos de saúde, presentes nos grandes centros urbanos.
OBJETIVOS: identificar os possíveis fatores que têm contribuído para o excesso da mortalidade neonatal no município de Juiz de Fora e avaliar a qualidade do preenchimento dos prontuários hospitalares. MÉTODOS: estudo caso-controle baseado em informações colhidas nos prontuários das três principais maternidades do município. Foram analisados 103 óbitos neonatais e amostra de 232 nascidos vivos. RESULTADOS: as variáveis peso ao nascer e índice de Apgar no quinto minuto foram importantes fatores preditivos para o óbito neonatal, independente do local de nascimento. Quando se comparou, o risco de morrer, entre os hospitais verificou-se que no Hospital 1 o risco foi 3,97 vezes maior que no Hospital 3. Baseado em consulta a especialistas, foi criado um escore para avaliação do prontuário, onde o Hospital 1 apresentou mediana mais baixa, tanto entre casos como em controles. CONCLUSÕES: a ausência de informações adequadamente registradas no prontuário é um indicador de precariedade na assistência, e, certamente, retarda a realização de conduta indicada. A pesquisa apontou deficiências, particularmente nos registros, da assistência perinatal oferecida nos três hospitais.
Objetivo: Avaliar o grau de suporte social percebido e sua relação com o autocuidado e o controle glicêmico dos indivíduos diabéticos tipo 2. Metodologia: Pesquisa transversal, realizada com 154 indivíduos diabéticos com idade superior a 18 anos, acompanhados em unidades de saúde. Foram analisadas variáveis sociodemográficas, antropométricas, clínicas, de autocuidado e de suporte social relacionadas ao controle glicêmico. Para a análise das variáveis de caráter genérico, foi realizado um estudo de frequência simples e calculadas as medidas de tendência central. Para verificar a associação das comorbidades com o controle glicêmico foi utilizado o teste Qui-quadrado. Resultados: Os participantes atingiram boa pontuação em algumas atividades do autocuidado e, apesar de terem pouco apoio social, se sentiam satisfeitos. Ao se associar as atividades de autocuidado com o suporte social recebido, não houve associação estatisticamente significativa. Conclusão: Houve associação entre algumas comorbidades, como Hipertensão, obesidade, nefropatias, depressão e o uso de insulina com o controle glicêmico, o que reforça a necessidade de estimular cada vez mais a busca do controle entre os indivíduos com diabetes tipo 2.
