Medicinal products of a biological origin are approved by the EMA at a centralized level. However, there is no harmonization about their use in Europe. The current regulation referring to the safety of biological medicinal products and biosimilars in Europe has been identified. The safety associated with medicinal products of a biological origin is assured by the pharmacovigilance system, which has evolved, but doesn't yet incorporate all of the specific information from this market segment, namely that related to the identification of drugs, and its use - including the prescription and dispensing, given the possibility of interchangeability and substitution. The terminology, information systems and traceability systems aren't entirely appropriate to ensure the safety requirements for therapy with medicinal products of a biological origin. Areas covered: This article aims to identify the prescription and dispensing profiles of reference biological medicines and biosimilars in the EU, and the determinants that support their safe use. Expert opinion: The European pharmacovigilance system must evolve to ensure the safety along all of the biologicals' therapeutic cycle. It must consider the safety for each of the medicines in addition to their safety pattern related to the eventual switching procedure.
Since the current legal framework stems from both European and national regulations, some gaps in the operational field may be expected. The goal of this paper is to identify these gaps and propose changes to the current information systems and pharmacovigilance regulations. Material and Methods:A qualitative analysis of current pharmacovigilance regulatory framework and supporting information system was conducted. Results: Current pharmacovigilance system does not seem to vouch for the safe use of biologicals and biosimilar drugs. The gaps found in reviewed materials may be attributable to their lack of specificity for biopharmaceuticals. Discussion: Biologicals therapy presents specific determinants related with the drugs, prescription, and traceability, without replication in any other segment of the pharmaceutical market. They are able to shape their safety profile. Conclusion:The existing pharmacovigilance's regulatory framework should be adjusted in order to improve the safety related with biopharmaceutical therapy. Some intervention measures are proposed. Estes fatores devem também ser tomados em consideração em presença de interpermutabilidade e substituição. Assim, é imperativo garantir o acompanhamento do rácio benefício risco, tendo em conta que no alinhamento entre os domínios regulamentar e operacional podem existir fronteiras de descontinuidade instrumental que urge identificar e suprir. Por ser universal, o sistema de farmacovigilância pode não estar a captar a informação específica dos medicamentos biológicos e biossimilares, que aqui se pretende identificar e caracterizar. Os modelos de financiamento das tecnologias de saúde têm vindo a tornar-se obsoletos. Este facto é reconhecido tanto pelos financiadores públicos -que detêm o poder de monopsónio associado em exclusivo ao financiamento pú-blico, como pela indústria farmacêutica -que detém o poder de monopólio conferido pelas patentes atribuídas aos medicamentos que desenvolve.Estão ancorados na eficácia e nos custos, não incorporando as demais dimensões do valor. Encontram-se desajustados dos modelos de autorização de introdução no mercado (AIM), bem como da incerteza associada à eficá-cia, efetividade, e segurança, e ainda da evolução que tem percorrido os sistemas de saúde.Acresce que os resultados em saúde são de magnitude diferente da que era usual: ganhos marginais, ou pelo contrário, resultados disruptivos. Por acréscimo, a qualidade de vida passou a integrar as prioridades estabelecidas, e os marcadores de eficácia já não são apenas de resultados finais, mas também de resultados intermédios, surgindo ainda os marcadores de efetividade.Propõe-se a transição do modelo atual centrado nas lacunas terapêuticas, para um outro centrado no valor, na linha da proposta da Organização Mundial de Saúde.1 Para a indústria farmacêutica, a elevada incerteza e os custos associados ao desenvolvimento regulamentar reduzem o retorno do investimento que se manteve estável durante largas décadas, incentivando o mercado farmacêutico a evoluir. Em 1975 a despesa com investigação e desenvolvimento (I&D) representava 9% do volume de vendas, tendo aumentado para 17% em 2010.2 Contudo, este esforço não foi acompanhado pelo incremento no número de moléculas desenvolvidas ou aprovadas pelas autoridades reguladoras.2 Nesse intervalo de tempo observou--se um aumento do número de fusões e aquisições, e a emergência de novos modelos de negócio, focalizados na captação externa de valor.2 Neste contexto, os preços dos medicamentos aprovados por procedimento centralizado tendem a ser 70% superiores aos aprovados por procedimento descentralizado, ou nacional, sendo mais elevados quanto maior a idade dos concorrentes, e independentes do grau de inovação. 3Por outro lado, as tecnologias de saúde possuíam uma intervenção padronizada, transversal a toda a população, mas agora têm vindo a focalizar-se em segmentos populacionais com genótipos específicos, que permitem antecipar o sucesso terapêutico. Também os agentes da decisão já não estão cingidos às autoridades reguladoras e financiadoras, e à indústria farmacêutica, alargando-se o leque no sentido de uma inclusão mais vasta dos representantes da sociedade civil.Neste contexto, a inovação deve ser perspetivada concedendo a primazia a uma visão transversal, em que os resultados das tecnologias de saúde inovadoras vão moldar os processos em que são incorporadas, e são moldados pelos agentes que integram a gestão dessas tecnologias. ResumoDesde o acesso ao mercado até o termo da comercialização, os medicamentos percorrem ciclos de valor, que são aferidos pelo desempenho de variáveis endógenas e exógenas ao medicamento. Os ciclos de valor constituem intervalos de tempo durante o qual o valor se mantém constante. A definição do preço dos medicamentos deve ter lugar em função do valor observado em cada ciclo. O contrato como instrumento de regulação das relações jurídicas administrativas com os titulares das autorizações de utilização de medicamentos, dispositivos médicos e outras tecnologias de saúde compromete-os com os objetivos do sistema de saúde em que se inserem, enquadrando as condições de comparticipação ou aquisição mediante avaliação prévia.Palavras-chave: preço do medicamento; financiamento governamental; gastos em saúde. AbstractFrom access to term of marketing, medicines go through value cycles measured by the performance of endogenous and exogenous variables. Marketing access includes efficacy, safety profile, and therapeutic indications, whereas term of marketing comprises drug life cycle and public health priorities. The value cycles are time intervals during which the value is constant. The setting of the price of pharmaceuticals should occur according to the value observed in each cycle. A system of pricing based on value cycles is dynamic, transparent, and geared towards efficiency and sustainability. Agreement as a regulatory tool of administrative and legal relationship with the marketing authorization holder is committed to the goals of the health care system.
RESUMOEm presença da crise económica pode ter ocorrido uma adaptação dos sistemas de saúde com impacto na acessibilidade. Com o objetivo de avaliar esta hipótese foram recolhidos dados anuais para oito países europeusAlemanha, Reino Unido, Espanha, França, Itália, Irlanda, Grécia e Portugal -entre 2005 e 2011. Os países foram agrupados em três grupos, de acordo com a tipologia de intervenção externa. Foi considerado um período anterior e outro posterior à crise, e avaliada a diferença nas médias, através do teste t de Student, para as variáveis relacionadas com a percentagem de doentes que manifestou necessidades médicas não satisfeitas devido ao custo excessivo dos cuidados de saúde, da distância, ou das listas de espera. Depois procedeu-se a uma análise de correlação recorrendo ao cálculo do coeficiente de Spearman, entre as variáveis associadas à acessibilidade e a taxa de desemprego. Foi observado um aumento de doentes com necessidades médicas não satisfeitas por custo excessivo, durante a austeridade, exceto para a Alemanha e Reino Unido. Os valores identificados foram mais elevados para os segmentos de rendimentos mais baixos, e de níveis de instrução elementares. DOES AUSTERITY DRIVES HEALTHCARE ACCESSIBILITY? ABSTRACTThe financial crisis may impact on healthcare systems, and challenge healthcare accessibility by increasing the percentage of patients with unmet medical needs due to the cost, the distance, and waiting lists. In order to analyze if austerity drives healthcare accessibility, we take annual data at country level, for Germany, United Kingdom, Spain, France, Italy, Ireland, Greece, and Portugal, from 2005 to 2011 (n=56). The countries were clustered according to the external intervention during financial crisis. We consider a period ex-ante and another one ex-post crisis, and assess the difference in averages, by using the t Student test. This is followed by a correlation analysis by calculating Spearman's coefficient, to assess if there is an association between the unemployment rate and the percentage of patients with unmet medical needs, considering the educational levels and income quintiles, regarding each one of the clusters. We found that during austerity there is an increase in the percentage of patients with unmet medical needs due to excessive cost, except for Germany and United Kingdom, but a decrease in the percentage of patients with unmet medical needs due to the distance and waiting lists. These figures were higher for low income and educational levels. For the cluster of France, Italy and Spain, the healthcare accessibility improves with increasing unemployment rates; but for Greece, Ireland and Portugal there is no association between accessibility and unemployment rate.
