ResumoEste texto retoma, em forma de questão, o tema do V Congresso Brasileiro de Ciências Sociais e Humanas da ABRASCO, realizado em abril de 2011 em São Paulo, e indica que o lugar das Ciências Sociais na Saúde Coletiva é -e não poderia deixar de ser -central. Diferentemente do que ocorreu na Saúde Pública, a saúde coletiva nasceu na academia, em um momento em que não mais se tratava de organizar um sistema publico de saúde, mas de ampliá-lo e estendê-lo a toda a população do país. O próprio termo "saúde coletiva" evoca o estudo da saúde de uma coletividade enquanto sistema social. Assim, a presença das ciências sociais e das ciências humanas na saúde coletiva, mais do que importante, é a base mesma, o cimento que constitui e alicerça a área. Em que pese a especificidade do campo, dada pela perspectiva social contida na palavra coletiva, é a palavra saúde que vem sendo sempre retida e utilizada nas classificações internas e externas à área, como no caso das agências de fomento; nestas, a saúde coletiva é classificada como pertencente à grande área da saúde, com todas as conseqüências que isso implica, notadamente a submissão à lógica biomédica. Como toda classificação e hierarquização, este lugar impõe às subáreas da saúde coletiva os critérios de mérito das ciências médicas, e ao mesmo tempo pode ser utilizado para desvalorizar o conhecimento produzido pelas demais disciplinas. Resumo Este artigo revisita o padrão de assimilação e difusão das tecnologias de reprodução assistida (TRA) no Brasil, caracterizado por altíssima concentração desta tecnologia no setor privado da medicina e baixíssima regulação na aplicação destas técnicas. Este padrão gerou enormes dificuldades e barreiras ao acesso a essas técnicas, basicamente por razões econômicas. A partir da apresentação do panorama da história das TRA, o artigo focaliza dois casos que podem configurar propostas de solução para a excessiva exclusão ao acesso, ainda que não se modifique a mesma lógica privatizante que marca a RA no país, uma vez que o problema das infertilidades não foi priorizado, até o momento, no Sistema Único de Saúde. A primeira, trazida pela indústria farmacêutica, consiste em um programa de compras subsidiadas de medicamentos pelos usuários no chamado Programa Acesso. A segunda proposta, conhecida como doação compartilhada de óvulos, implica a troca, entre duas mulheres, de material reprodutivo biologicamente escasso (ovócitos). Esta troca ocorre, em geral, entre uma mulher que tem óvulos e não pode custear seu tratamento e outra que, não tendo mais óvulos para se reproduzir, custeia o tratamento da doadora. Ambas as propostas se difundem num quadro legal bastante mal regulado em termos da aplicação das TRA, mas que veda práticas não admitidas na Constituição Federal e criminalizadas na lei penal, como a comercialização de órgãos e tecidos humanos em geral, bem como pagamento a voluntários de testes para pesquisa, pela Resolução nº 466 (CNS, 2012). Em termos metodológicos, o artigo assume caráter monográfico e de debate da literatura, mas está baseado também na coleta de dados secundários recentes, assim como em inúmeras pesquisas empíricas realizadas ao longo dos últimos 25 anos pelas autoras.
Resumo: O aporte das ciências sociais, a partir dos anos 1970, através de suas teorias e metodologias já consolidadas, foi indispensável para o desenvolvimento e consolidação da área que, no campo da saúde, se tornou conhecida como Saúde Coletiva. Em contraste com sua participação, as ciências sociais sempre ocuparam um lugar subalterno neste campo, dominado, em seus primórdios, isto é, durante as décadas de 1970 e 1980, pelo planejamento em saúde e, a partir da década de 1990, pela epidemiologia. O domínio da epidemiologia acontece justamente quando, reconhecida pela Capes como uma área autônoma, esperava-se maior equilíbrio entre as diferentes disciplinas que compõem a área da Saúde Coletiva. Este estudo levanta questões sobre a situação das ciências sociais na área, em relação com a execução da política de fomento das agências nacionaisCapes e CNPq. A imposição de uma lógica custo-benefício de caráter predominantemente economicista e quantitativista, aplicada pelas agências na concessão de recursos, interfere tanto na produção quanto na circulação do conhecimento, com prejuízos para as ciências sociais. Contornar essa situação constitui um dos desafios que se colocam hoje para as ciências sociais e, em particular, para a área da Saúde Coletiva.
This paper discusses the role of medicine, ranging from the normative and technological control of women's sexuality ad reproductive process (childbirth, breastfeeding, contraception, and treatment of infertility) through the construction of a new (biological and social) reproductive model, based on a radical change of identities, relations, and forms of union between the sexes (a break with the traditional concept of marriage, growth of open unions, serial monogamy, etc.). This model is sustained by a radical distinction between sexuality and reproduction, related to a unique and horizontal model of sexuality (in opposition to the hierarchical 19th-century two-sexes model); the new model focuses mainly on pleasure and moves progressively away from social ties and affection. In addition to the contribution of medicine (notably through assisted reproduction) and sexology (universalization of the orgasm imperative), the paper also discusses the contribution of epidemiology (through AIDS-related research) to the transformation of a moral sexual norm into an abstract and merely statistical standard
