A growing number of research funding organizations (RFOs) are taking responsibility to increase the scientific and social impact of research output. Also reusable research data are recognized as relevant output for gaining impact. RFOs are therefore promoting FAIR research data management and stewardship (RDM) in their research funding cycle. However, the implementation of FAIR RDM still faces important obstacles and challenges. To solve these, stakeholders work together to develop innovative tools and practices. Here we elaborate on the role of RFOs in developing a FAIR funding model to support the FAIR RDM in the funding cycle, integrated with research community specific guidance, criteria and metadata, and enabling automatic assessments of progress and output from RDM. The model facilitates to create research data with a high level of FAIRness that are meaningful for a research community. To fully benefit from the model, RFOs, research institutions and service providers need to implement machine actionability in their FAIR RDM tools and procedures. As many stakeholders still need to get familiar with “human actionable” FAIR data practices, the introduction of the model will be stepwise, with an active role of the RFOs in driving FAIR RDM processes as effectively as possible.
IntroductionAll studies should report methods and findings in full, following credible and justifiable reporting guidelines. According to the guiding principles of the Ensuring Value in Research (EViR) Funders’ Forum (www.evir.org), this applies irrespective of the nature of the findings or whether the study was completed as planned.One way for a public funding agency to address evidence quality and transparency is to adaptively implement EQUATOR reporting guidelines (www.equator-network.org) in its funding procedure to ensure research quality ‘from proposal to publication’. The Netherlands Organisation for Health Research and Development, ZonMw, has created the ZonMw Reporting Checklist (ZRC), which was derived from EQUATOR reporting guidelines in order to systematically plan, monitor, and evaluate projects. The next step is experimenting with implementing the ZRC in ZonMw’s grant management system and procedures. Customization is possible based on the ‘comply or explain’ approach (80/20 rule).MethodsWe selected 15 EQUATOR reporting guidelines that covered basic research and health technology assessment through to implementation projects, supplemented with the reporting guideline for implementation studies (StaRI checklist). We conducted comparative content analyses (including rearrangement) to provide a greatest common denominator consisting of both standard and modular reporting elements. We completed the ZRC by adding other current requirements for responsible research practices with respect to diversity and gender, data management, open access, systematic reviews, recruitment and inclusion, registration, and impact.ResultsThe ZRC results in structured and validated in-house data on the objectives, design, conduct, and results of ZonMw projects. This is an important source for good research governance, impact assessment, and research on research.ConclusionsImplementation of the ZRC by a funding agency optimizes the quality, transparency, relevance, and impact of evidence, which legitimately and effectively improves health care for all.
De TSG-redactie heeft onlangs besloten een nieuwe rubriek te openen over 'Gegevens voor gezondheid en zorg'. Deze rubriek biedt de ruimte voor de bespreking van belangrijke ontwikkelingen rond landelijke en lokale gezondheidsen zorgregistraties, waartoe ook gezondheidsenquêtes en epidemiologische surveys worden gerekend. AanleidingGegevensverzamelingen vormen een belangrijke pijler onder het public health en gezondheidszorgbeleid. Het belang ervan zal alleen maar toenemen naarmate de informatiemaatschappij waarin we nu leven, zich verder ontwikkelt. Er is bovendien veel over te doen. De kwaliteit en continuïteit staan nogal eens onder druk, denk bijvoorbeeld aan de continuï-teitsproblemen die spelen of gespeeld hebben rond de Landelijke Medische Registratie van ziekenhuizen, epidemiologische bevolkingsonderzoeken zoals Nederland de Maat Genomen en lokale of landelijke gezondheidsenquêtes. Maar er is meer. Internationale vergelijkbaarheid en benchmarking binnen Nederland worden steeds belangrijker. Er is behoefte aan meer samenhang, betere toegankelijkheid en meer zichtbaarheid van het gebruik en het nut van al deze gegevensverzamelingen.Aan de andere kant is er ook vernieuwing. De website Zorggegevens.nl telt alleen al zo'n 180 landelijke registraties. Bij één derde daarvan verwachten de gegevensbeheerders dat er op korte termijn veranderingen zullen optreden in de organisatie of inhoud. Kortom, er is veel dynamiek, terwijl maar weinig professionals in de public health en in de organisatie van de gezondheidszorg daar goed zicht op hebben. Zelfs degenen die beroepsmatig veel met data of met informatiebeleid in de weer zijn, overzien vaak maar een gedeelte van het gegevenslandschap. DoelDe nieuwe TSG-rubriek wil daarom een forum bieden waar kennis en discussie over al deze ontwikkelingen bij elkaar komen en zichtbaar worden. De rubriek wil ook stimuleren dat er een beter op elkaar afgestemde infrastructuur van gegevensverzamelingen in Nederland ontstaat. Het streven daarbij is dat registraties en -enquêtes zo goed mogelijk gebruik maken van (inter)nationaal erkende classificaties, dat koppelingsmogelijkheden tussen gegevensverzamelingen versterkt worden, dat de regionale, nationale en internationale vergelijkbaarheid bevorderd wordt en dat zoveel mogelijk gegevens door derden gebruikt kunnen worden voor onderzoek en beleid (open access). OpzetVoor elk komend TSG-nummer zullen wij een sleutelfiguur uit een belangrijk gegevensverzamelend instituut in Nederland uitnodigen een bijdrage te schrijven over belangrijke lopende of verwachte ontwikkelingen in één of meer van hun registraties of enquêtes, een meer algemene ontwikkeling of een probleem dat hij of zij signaleert en aandacht verdient. Preventie van diabetes en hart-en vaatziekten is een speerpunt van het kabinetsbeleid. Met de principiële uitspraak van CVZ dat preventie geïndiceerd is voor mensen met een hoog risico, staat nu de weg vrij om de BeweegKuur in het basispakket te krijgen. Om dit daadwerkelijk te laten gebeuren moet aan een aantal voorwaarden worden voldaan, waarbij zowel beleid, als ook praktijk en onderzoek betrokken zijn. Voor ZonMw is de wisselwerking tussen onderzoek, beleid en praktijk de motor achter vernieuwing in de gezondheidszorg en preventie. Verondersteld kan worden dat een sterke interactie ook tot een snelle vernieuwing leidt. Is dat ook zo? En wat gebeurt er precies in de loop van zo'n proces? Een terugblik op de ervaringen van ZonMw met het uitzetten van onderzoek naar de BeweegKuur.Beleid: 'windows of opportunity' Onderzoek: welk bewijs is nodig?Een belangrijke overweging om preventieve interventies in het basispakket op te nemen is of de kosteneffectiviteit van de interventie bewezen is in -bij voorkeur -een randomized controled trial en of het gunstige effect ook op langere termijn (zeg twee jaar) stand houdt.In het voorjaar van 2008 gaf het ministerie van VWS daarom opdracht aan ZonMw om de BeweegKuur te laten onderzoeken op kosteneffectiviteit. ZonMw ging vervolgens met CVZ, VWS, NISB, RIVM en diverse onderzoekers na welke data gegenereerd zouden moeten (en kunnen) worden om tot pakketopname te kunnen overgaan, rekening houdende met de reeds lopende implementatie van de Beweegkuur. Al gauw werd duidelijk dat de resultaten pas na vier à vijf jaar beschikbaar zouden komen, dus na de ambtsperiode van de minster.Eveneens in 2008 bereidde het RIVM op verzoek van VWS een rapport voor met een overzicht van de wetenschappelijke literatuur en secundaire data-analyse over de kosteneffectiviteit van gecombineerde leefstijlinterventies op het gebied van bewegen en voeding. In het najaar bleek uit deze studie dat er voldoende bewijs leek te bestaan dat gecombineerde leefstijlinterventies kosteneffectief zijn. Het gunstige effect werd -meer nog dan bij mensen met (risico op) diabetes -gevonden bij mensen met overgewicht en een of meerdere risicofactoren voor hart-en vaatziekten. Voor verschillende subgroepen kon het RIVM berekenen wat de kosteneffectiviteit is van verschillende samenstellingen van leefstijlpakketten en wat een licht, middel of zwaar leefstijlprogramma zou mogen kosten.2 Voor CVZ was dit voldoende om te bepalen dat gecombineerde leefstijlinterventies zoals de BeweegKuur tot de stand van wetenschap en praktijk behoren en voor bepaalde indicaties in principe via het basispakket zouden kunnen worden aangeboden. 3,4Het door ZonMw uit te zetten onderzoek was dus niet meer nodig, redeneerde het ministerie van VWS. Wel was men van mening dat het protocol van de BeweegKuur moet worden aangepast aan de inzichten uit de RIVM-studie, dat de gunstige kosteneffectiviteit dan voor twee van de drie pakketten geldt en dat alleen voor het derde, zwaardere pakket meer bewijs nodig is.
