A revisão integrativa da literatura objetivou levantar o estado atual do conhecimento sobre modelos econométricos de projeção da força de trabalho no Brasil e identificar o mais apropriado para aplicação no âmbito da Atenção Primária em Saúde. A busca se deu a partir de diversas fontes: bases de dados eletrônicas, revistas, anais de congressos, referência cruzada e Plataforma Lattes, por meio de descritores e palavras-chave. A amostra foi constituída de nove publicações, as quais foram lidas, resumidas e analisadas. Os resultados apontam o uso de seis métodos de estimativa de recursos humanos distintos, porém em sua maioria as metodologias propõem cálculos preditivos para o dimensionamento de pessoal com vistas à alocação/realocação imediata de profissionais. Ressalta-se a impossibilidade de definir o melhor modelo econométrico para a projeção de recursos humanos em saúde na Atenção Primária, em virtude do pequeno número de publicações nacionais relativas ao tema, nas quais não se obteve homogeneidade, embora se verifique maior preocupação com a alocação da força de trabalho em saúde, mormente no âmbito hospitalar.
RESUMO Objetivou-se calcular o índice de rotatividade dos médicos brasileiros. Estudo descritivo, exploratório, quantitativo, realizado por meio de dados secundários, que foram analisados por estatística descritiva. Constatou-se maior índice de rotatividade nas regiões Sudeste e Sul, com médias superiores à nacional (36,7%). A menor média foi evidenciada na região Norte (24,7%). A estratificação por porte populacional aponta para maior rotatividade nos grupamentos de municípios com população entre dez mil e cem mil habitantes; e menor índice nos municípios de grande porte. A rotatividade não pode ser inteiramente compreendida sem que se analise o contexto no qual estão inseridos os profissionais. PALAVRAS-CHAVE
A cost-effectiveness analysis was conducted in screening for breast cancer. The use of conventional mammography, digital and magnetic resonance imaging were compared with natural disease history as a baseline. A Markov model projected breast cancer in a group of 100,000 women for a 30 year period, with screening every two years. Four distinct scenarios were modeled: (1) the natural history of breast cancer, as a baseline, (2) conventional film mammography, (3) digital mammography and (4) magnetic resonance imaging. The costs of the scenarios modeled ranged from R$ 194.216,68 for natural history, to R$ 48.614.338,31, for screening with magnetic resonance imaging. The difference in effectiveness between the interventions ranged from 300 to 78.000 years of life gained in the cohort. The ratio of incremental cost-effectiveness in terms of cost per life-year gains, conventional mammographic screening has produced an extra year for R$ 13.573,07. The ICER of magnetic resonance imaging was R$ 2.904.328,88, compared to no screening. In conclusion, it is more cost-effective to perform the screening with conventional mammography than other technological interventions.
Resumo A inspeção sanitária de Gerenciamento de Resíduos de Serviços de Saúde (GRSS) carece de metodologia de trabalho padronizada que proporcione bases seguras de avaliação de risco potencial (RP). O objetivo deste trabalho foi propor um instrumento baseado em lógica fuzzy capaz de padronizar a inspeção e gerar indicadores de controle sanitário. O modelo foi desenvolvido com 18 subsistemas relacionados ao GRSS, que resultaram da aglutinação de elementos identificados na inspeção, como aqueles que interferem na avaliação e permeiam todas as etapas do GRSS. A validação do modelo foi realizada em dez unidades de saúde no Rio de Janeiro, de maio a novembro de 2009. Os resultados obtidos no sistema seguiram os diagnósticos realizados pelos especialistas, mostrando a possibilidade de sistematização e racionalização das avaliações de RP. Esse sistema de apoio à decisão torna mais eficiente a gestão e o planejamento de ações na avaliação do RP.
Background: Several prognostic factors and composite scores based on either patients general condition and/or usual blood analyses have been described in the setting of advanced metastatic cancers to estimate patients overall survival (OS). Over the past decade, circulating tumor cells (CTC) have been repeatedly reported as being a strong prognostic tool in metastatic breast cancer patients. We took the opportunity of the observational part of a prospective clinical trial (CirCe01, NCT01349842) that included 3rd line metastatic breast cancer patients to assess and compare several prognostic factors/scores and circulating tumor cell count. Patients and Methods: Metastatic breast cancer patients were included before the start of a 3rd line chemotherapy at the Institut Curie, Paris, France. The following adverse prognostic factors were assessed prospectively and compared to overall survival: altered performance status (PS≥2), lymphopenia (<1,000 lymphocyte/microl), elevated CTC count (CellSearch, ≥5CTC/7.5ml), low albumin (<35g/l), elevated CA 15.3 (>30UI/ml), elevated CEA (>5ng/ml), elevated LDH (>50UI/l). Two composite scores were also evaluated: the Barbot score (combining Karnofsky index, number of metastases, albumin and LDH; Barbot JCO 2008) and the Prognostic Inflammatory and Nutritional Index (PINI; combining albumin, prealbumin, orosomucoid and C-reactive protein). Metastatic sites (liver, lung, bone) were also analyzed. Survival was analyzed by Kaplan-Meier curves; multivariate analysis was done using a Cox model. Results: 56 patients have included prospectively and 36 of them (64%) died. CA 15.3 and CEA levels, lymphopenia and metastatic sites had no significant impact on OS in univariate analysis. Discussion: CTC are an independent prognostic marker, even in advanced metastatic breast cancer. The disease aggressiveness being captured by the CTC count, usual clinical and biochemical tests appear to be sufficient to evaluate the patient prognosis, whereas more complex score (PINI, Barbot) appear to be of low interest.
