OBJECTIVE To characterize the technical issues and conditions of medicines conservation in Primary Health Care of Brazilian regions, responsible for pharmacy/dispensing unit profile; environmental, storage, and dose fractioning conditions; inventory control and waste management; fire and electrical failure safety items; transportation problems; advertising regulation; and pharmacovigilance.METHODS This article is part of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços (National Survey on Access, Use and Promotion of Rational Use of Medicines – Services)–, a cross-sectional and exploratory study, of evaluative nature, consisting of an information survey within a representative sample of municipalities, stratified by Brazilian regions, which constitute the study domains, and a sample of Primary Health Care services. Pharmaceutical services (PS) were directly observed with photographic record and face-to-face interviews with those responsible for the dispensing of medicines and over the telephone with those responsible for pharmaceutical services. Data were processed with the SPSS® software version 21.RESULTS The investigated dimensions showed relevant deficiencies and inequalities between the regions, generally more favorable in the Southeast and Midwest regions and weaker in the Northeast and North regions. We verified non-compliance with technical requirements and conditions essential to the conservation of medicines, which may interfere with the maintenance of stability and, thus, on their quality, efficacy, and safety. The regulation of advertising/promotion of medicines is still incipient and there is some progress in the structuring of mechanisms regarding pharmacovigilance.CONCLUSIONS The sanitary situation of medicines in Brazilian Primary Health Care is alarming due to the violation of the specific sanitary legislation for dispensing establishments and due to a wide range of requirements essential to the conservation of medicines. We observed a disconnection between the efforts made in the Brazilian Unified Health System to promote access to medicines for all population and the organization and qualification of pharmaceutical services.
OBJECTIVE To characterize the current stage of the institutionalization of pharmaceutical services in Brazilian cities.METHODS This study is part of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines), a cross-sectional, exploratory, and evaluative study composed by an information survey in a representative sample of cities, stratified by Brazilian regions. We interviewed municipal secretaries of health, responsible for pharmaceutical services, and pharmacists responsible for the dispensing of medicines. The variables selected from the interviews were grouped into five dimensions that defined three stages of pharmaceutical services institutionalization: incipient (0%-34.0%), partial (35.0%-69.0%), and advanced (70.0%-100%), estimated based on the interviewees’ answers. Frequencies were estimated with 95% confidence intervals. For the statistical association analysis, the Chi-square test was applied, with significance level of p<0.05.RESULTS Our results show a partial and heterogeneous process of institutionalization of pharmaceutical services in Brazil, and an advanced stage in formal structures, such as the municipal health plans and the existence of a standardized list of medicines. The analysed variables in the “organization, structure, and financing” dimension configured stages that range from partial to advanced. The management presented partial institutionalization, positively showing the existence of computerized system, but also disparate results regarding the autonomy in the management of financial resources. Indispensable items related to the structure expressed disparities between the regions, with statistically significant differences.CONCLUSION The study showed a partial and heterogeneous process of institutionalization of pharmaceutical services in Brazilian cities, showing regional disparities. Variables related to the normative aspects of institutionalization were positively highlighted in all dimensions; however, it is necessary to conduct new studies to evaluate the institutionalization of pharmaceutical services’ finalistic activities.
Foram caracterizadas as demandas judiciais por medicamentos em quatro municípios do Estado da Bahia, no período de 2006 a 2010, utilizando-se como fontes de dados informações relacionados a estas demandas arquivadas nas secretarias municipais de saúde. Observou-se que foram impetradas 228 ações solicitando 574 medicamentos, perfazendo uma média de 2,5 medicamentos. Informações relevantes, como relatórios médicos, não estavam presentes na maioria das solicitações arquivadas. Em três municípios, a maioria dos medicamentos solicitados não constava em nenhum elenco de programas da Assistência Farmacêutica do SUS. Entretanto, entre aqueles incluídos, chama atenção a demanda por via judicial por medicamentos do componente básico da assistência farmacêutica, que chega a 50% em um dos municípios. Nos quatro municípios prevalecem solicitações de medicamentos das classes Sistema Cardiovascular, Aparelho Digestivo e Metabolismo e Sistema Nervoso Central, segundo a classificação ATC. Os resultados revelam que a área técnica responsável pelo uso racional de medicamentos é reduzida a mero fornecedor destes insumos e evidenciam a necessidade de melhorias na gestão desse setor e de uma sistemática de comunicação e parceria com o judiciário, viabilizando uma tomada de decisão melhor fundamentada, sem riscos para o paciente, mas considerando também as normas técnicas que orientam o funcionamento do SUS.
Introdução: Com a justificativa de promover o Uso Racional de Medicamentos (URM) através da disseminação de informação técnico-cientifica objetiva, independente, atualizada e avaliada criticamente, com vista à melhoria da qualidade de vida da população, o Centro de Informação sobre Medicamentos (CIM), no âmbito de uma Secretaria Estadual de Saúde (SES), desenvolve diversas ações para instrumentalizar profissionais do SUS. Apesar do aumento significativo nos gastos com a aquisição de medicamentos no SUS nos últimos anos, a assimetria de informações entre prescritores, pacientes e farmacêuticos é uma importante barreira para o acesso aos medicamentos contemplados nas políticas públicas. As consequências podem ser tratamento inadequado, implicação de gasto familiar com a aquisição direta ou o acesso pela via judicial, acarretando custos adicionais para todo o sistema. Objetivo: Descrever e analisar a implantação de quatro ações estruturantes realizadas pelo CIM no período de 2019 a 2022: 1) Elaboração de notas e fichas técnicas baseadas em evidências científicas e legislação sanitária vigente; 2) Elaboração de boletim informativo eletrônico periódico contendo temas relacionados ao URM, Avaliação de Tecnologias em Saúde (ATS) e Assistência Farmacêutica (AF); 3) Implantação de uma seção no portal da internet da SES para a publicação das produções técnico-cientificas; e 4) Execução de programa de treinamento continuado dirigido a farmacêuticos da atenção primária, que aborda aspectos legais relacionados à AF, bases de dados científicos e conceitos da ATS. Método: Os dados foram obtidos dos documentos técnicos elaborados pelo CIM, relatórios de gestão e registros dos treinamentos realizados, relativos ao período de 2019 a 2022. Resultado: Foram elaboradas e revisadas no período 38 notas técnicas e 54 fichas técnicas sobre os medicamentos mais demandados. Foram publicadas 09 edições do boletim informativo, enviados através de correio eletrônico institucional para cerca de 900 endereços e que registram 520 acessos. A seção no portal da internet foi implantada em 2020, com acesso crescente, havendo registros de 3722 visitantes no período. A nota técnica mais acessada é sobre antiangiogênicos e a ficha mais acessada é sobre rivaroxabana. O programa de treinamento foi executado, com a participação de 358 farmacêuticos vinculados à atenção primária. DISCUSSÃO E Conclusão: As produções científicas do CIM, disponíveis em uma página na internet, contribuem para que farmacêuticos e prescritores tenham acesso a informações atualizadas sobre medicamentos e as políticas de saúde relacionadas, enquanto o boletim informativo, bem como o programa de treinamento vêm se consolidando como espaço de disseminação de informações qualificadas sobre AF e ATS. Assim, essas ações desenvolvidas pelo CIM instrumentalizam o público alvo para a tomada de decisão quanto às demandas relativas a medicamentos e desempenham importante papel na promoção do acesso e do URM.
Introdução: O tratamento dos transtornos mentais no SUS contempla a oferta de cuidados por equipe multiprofissional e a assistência farmacêutica. O decanoato de haloperidol (DH) é uma droga indicada em bula para tratamento de manutenção de pacientes psicóticos crônicos estabilizados que consta na Relação Nacional de Medicamentos Essenciais (Rename), sendo dispensada em farmácias de unidades de assistência em saúde mental do SUS. Os efeitos colaterais do DH são distonia, rigidez muscular, parkinsonismo, discinesia tardia, sedação, ganho de peso e outros, semelhantes à senilidade. Conhecer as características das prescrições desta droga é considerada uma etapa importante, que pode subsidiar o planejamento das ações de promoção da adesão ao tratamento e verificação da ocorrência de problemas relacionados ao medicamento (PRM). Objetivos: Caracterizar as prescrições atendidas pela farmácia de um centro de saúde mental do SUS em Salvador no tocante ao total de prescrições, vínculo institucional, idade média dos pacientes, sexo, percentual de pacientes acima de 55 anos e número médio de medicamentos por prescrição. Metodologia: As informações foram obtidas a partir da segunda via das prescrições atendidas e retidas pela farmácia nos meses de janeiro e fevereiro de 2019, bem como do prontuário dos pacientes matriculados na unidade. Resultados: Nos meses avaliados foram atendidas pela farmácia 3.749 prescrições, identificando-se a presença de DH em 156 delas (4,16%), dessas 125 (80,13%) são de pessoas cadastradas na unidade, 16 (10,25%) são provenientes de outras instituições da rede SUS e 15 (9,62%) de instituição privada. As prescrições eram majoritariamente de pacientes do sexo masculino (63,46%) e a média da idade da amostra foi de 44 anos, sendo que 39 (25%) das prescrições para pacientes com idade superior a 55 anos, dessas 22 (56,41%) de pacientes da Unidade. As prescrições continham em média 3,8 tipos de drogas, com variação entre 1 e 8 medicamentos. DISCUSSÃO: O elevado percentual de prescrições contendo DH e o maior percentual de prescrições para pacientes do sexo masculino devem ser analisados considerando que a farmácia também atende a pacientes de outros serviços do SUS ou suplementares. A média de idade é semelhante a outros estudos, porém o número médio de medicamentos por prescrição é maior. Um pouco mais da metade (56,41%) das prescrições de pacientes acima de 55 anos é oriunda da Unidade, permite um acompanhamento farmacoterapêutico pelo serviço da farmácia. Conclusão: Considerando que 25% das prescrições atendidas eram de pacientes acima de 55 anos, a polimedicação, os potenciais efeitos colaterais e interações medicamentosas, sugere-se investigar em pesquisas futuras a ocorrência de PRM com o DH. Neste sentido, a oferta do serviço de acompanhamento farmacoterapêutico pela farmácia, integrado à equipe de cuidado faz-se necessário, podendo contribuir para a melhoria na qualidade de vida desses pacientes.
