ObjectiveTo demonstrate economic impact of pharmaceutical evaluation in detection and prevention of errors in antineoplastic prescriptions.MethodsThis was an observational and retrospective study performed in a cancer hospital. From July to August 2016 pharmacists checked prescriptions of antineoplastic and adjuvant drugs. Drug-related problems observed were classified and analyzed concerning drug, pharmaceutical intervention, acceptability and characteristic of the error. In case of problem related to dose, we calculated a deviation percentage related with correct dose and value spent or saved. Data were analyzed using descriptive statistics with frequency and percentage.ResultsA total of 6,104 prescriptions and 12,128 medications were evaluated. Drug-related problems were identified in 274 (4.5%) prescriptions. Most of them was due to lack of information (n=117; 36.1%). Problems associated with dose accounted for 32.1% (n=98) of the total. In 13 cases (13.3%) ranging of prescribed dose was 50% greater than the correct dose. Intercepted drug-related problems provided savings of R$54.081,01 and expenses of R$20.863,36, therefore resulting in a positive balance of R$33.217,65. Each intervention promoted saving of R$126,78 with an acceptance rate of 98%. Main pharmaceutical interventions were information inclusion (n=117; 36.1%) and dose change (n=97; 29.9%). All errors were classified as error with no harm.ConclusionSimple actions such as prescription checking are able to identify and prevent drug-related problems, avoid financial losses and add immeasurable value to patient safety.
ResumoContextoComplicações do tromboembolismo venoso são encontradas frequentemente em pacientes internados, tanto em condições clínicas quanto em pós-operatórios.ObjetivoVerificar a quimioprofilaxia utilizada para tromboembolismo venoso em pacientes oncológicos internados, antes e após a realização de um programa de esclarecimento da sua importância.MétodosEstudo de corte transversal realizado em três momentos distintos: inicialmente antes do programa de conscientização da importância da profilaxia do tromboembolismo venoso, durante o período em que foi realizada e um ano após a etapa anterior. Para fins estatísticos, os pacientes foram divididos em alto risco e baixo risco, e estratificados quanto a erro na quimioprofilaxia em: precisavam, mas não fizeram profilaxia; não precisavam, mas fizeram profilaxia; fizeram profilaxia não padronizada; e não podiam, mas fizeram profilaxia.ResultadosForam avaliados 399 pacientes internados, sendo 56 pacientes antes do início do programa de conscientização, 255 durante o programa e 88 após um ano. Antes da realização da semana de conscientização, apenas 35,7% dos pacientes estavam recebendo a quimioprofilaxia adequada; após a semana de conscientização, houve um aumento do número de prescrições corretas, que passou para 63,9% (p < 0,001). Após um ano sem as aulas de conscientização, a manutenção da quimioprofilaxia não foi tão eficaz, com uma tendência ao aumento do número de profilaxias incorretas (p = 0,081).ConclusãoA quimioprofilaxia é utilizada em uma porcentagem muito pequena nos pacientes internados, sendo necessários programas de esclarecimento de sua importância na prevenção do tromboembolismo venoso e a realização de monitoramento contínuo para auxiliar na sua prescrição.
O objetivo deste estudo foi identificar o perfil de utilização de medicamentos anti-infecciosos de um hospital oncológico, e em seus setores de internamento adulto, transplante de medula óssea e unidade de terapia intensiva. Foram coletados dados de janeiro a dezembro de 2010, empregando a metodologia de Dose Diária Definida (DDD) por 100 pacientes-dia e análise farmacoeconômica dos custos diretos de compra. O consumo de anti-infecciosos identificado no hospital foi de 99,1 DDD/ 100 pacientes-dia, com predomínio do uso de cefalosporinas (38,8%) e quinolonas (21,9%). A análise econômica mostrou que os anti-infecciosos correspondem ao segundo grupo com maior custo direto de compra em um hospital oncológico, perdendo apenas para classe de medicamentos oncológicos, com forte impacto dos anti-infecciosos não padronizados pela instituição. O consumo de anti-infecciosos é maior nos setores de maior complexidade de cuidado, como transplante de medula óssea e unidade de terapia intensiva, havendo nestes setores um maior custo direto de compra de anti-infecciosos por 100 pacientes-dia em relação aos pacientes em setores de internamento adulto.
