Objetivo: Identificar factores asociados a calidad percibida de atención en la población adulta afiliada al Seguro Social en Salud (EsSalud). Material y métodos: Analizamos la Encuesta Nacional Socioeconómica de Acceso a la Salud de los Asegurados de EsSalud. La calidad percibida fue evaluada en consultorio externo, hospitalización y emergencias. Reportamos razones de medias ajustadas (RMa) estimadas mediante regresión de Poisson múltiple. Resultados: Estar empleado (RMa=0,98; IC95% 0,96-0,99), no conocer los servicios que brinda EsSalud (RMa=0,97; IC95% 0,95-1,00), no recibir información sobre los derechos como usuarios (RMa=0,95; IC95% 0,93-0,98), valorar como bajo el nivel de accesibilidad al EESS (RMa=0,97; IC95% 0,95-0,99) y no sentirse cómodo en el EESS al que acude (RM=0,92; IC95% 0,91-0,94) fueron factores asociados a menor puntaje de calidad percibida en consultorio externo. En hospitalización, tener entre 40-59 años (RMa=0,93; IC95% 0,88-0,97), contar con un índice de riqueza bajo (RMa=0,92; IC95% 0,87-0,97), no haber recibido información sobre los derechos como usuarios (RMa=0,90; IC95% 0,84-0,95) y no sentirse cómodos en el EESS (RMa=0,92; IC95% 0,89-0,96) se asociaron a menor puntuación de calidad. En emergencia, no sentirse cómodo en el EESS (RMa=0,89; IC95% 0,93-1,00) fue el único factor asociado a una menor puntuación de calidad percibida. Conclusiones: Identificamos factores relativos a los usuarios y a la oferta de servicios independientemente asociados a calidad percibida de atención, los cuales pueden ser considerados por los tomadores de decisiones para la mejor gestión de EsSalud.
Introducción: El planeamiento suicida es un paso previo al intento suicida. En distintas poblaciones se ha encontrado tasas altas de planeamiento suicida en estudiantes universitarios, incluso mayores a las de la población general de la que provienen. Objetivos: Determinar qué factores están asociados a planeamiento suicida en estudiantes de una escuela de nutrición. Diseño: Estudio transversal. Institución: Escuela Académico Profesional de Nutrición de la Facultad de Medicina San Fernando, Universidad Nacional Mayor de San Marcos. Participantes: Estudiantes de nutrición del primer al cuarto año académico. Intervenciones: Se administró una encuesta. Principales medidas de resultados: Significancia estadística en las pruebas de regresión logística. Resultados: En el análisis bivariado, ninguna variable estuvo asociada con planeamiento suicida. Al ajustar a variables socioeconómicas, solo la frecuencia de dificultad para conciliar el sueño en la noche tuvo una asociación significativa (p=0,044). Al ajustar a todas las variables la frecuencia de dificultad para dormir, mantuvo asociación significativa (p=0,039). Conclusiones: La dificultad para conciliar el sueño fue un factor de riesgo independiente significativo de planeamiento suicida en estudiantes de la Escuela Académico Profesional de Nutrición estudiada.
ResumenIntroducción: En el Perú, Lima y Callao constituyen regiones con riesgo alto de transmisión activa de tuberculosis. La tuberculosis multidrogorresistente se presenta en el 5,54% de los casos nuevos de tuberculosis pulmonar (TB MDR primaria) en Lima y Callao. Sus factores de riesgo no han sido estudiados en pacientes del Callao. Objetivos: Determinar los factores de riesgo para TB MDR primaria en pacientes atendidos en centros de salud de Callao, durante los años 2009-2010. Diseño: Estudio caso control. Lugar: Centros de Salud de Callao. Participantes: Participaron 29 pacientes con TB MDR primaria y 37 con tuberculosis sensible, según los registros del Programa de Control de Tuberculosis y las historias clínicas ubicadas en los centros de salud. Intervenciones: Toda la información fue obtenida de fuentes secundarias. En el análisis bivariado se calculó el OR de cada covariable. Para el cálculo de los OR multivariados se utilizó modelos de regresión logística. Principales medidas de resultados: Factores de riesgo para TB MDR. Resultados: En el análisis bivariado, el haber tenido contacto con pacientes TB MDR o XDR fue un factor de riesgo significativo (OR: 5,56; IC95%: 1,05 a 29,27); lo mismo se demostró en el análisis multivariado (OR: 14,56; IC95%: 1,52 a 139,54). Además, en el análisis multivariado, la variable edad ≥ 40 años mostró ser un factor protector significativo (OR: 0,94; IC95%: 0,90 a 0,99). Conclusiones: El tener contacto con pacientes TB MDR/XDR fue factor de riesgo y una edad menor a 40 años un factor protector para contraer TB MDR primaria en los pacientes del Callao estudiados. Multidrug-resistant tuberculosis represents 5.54% of the new cases of pulmonary tuberculosis (primary MDR-TB) in Lima and Callao. Involved risk factors have not been studied in patients from Callao. Main outcome measures: Risk factors for primary MDR-TB. Results: Prior contact with a MDR-TB or XDR-TB patient was a significant risk factor (OR: 5.56;) in bivariate analysis; the same was found in multivariate analysis (OR: 14.56; 95%CI: 1.52-139.54).
