Páginas 01 de 05 não para fins de citação Justificativa e Objetivos: A Terapia Nutricional Enteral (TNE) é utilizada para a manutenção ou recuperação do estado nutricional nos indivíduos com o trato gastrintestinal funcionante, com a ingestão oral parcial ou totalmente comprometida. O objetivo deste estudo foi avaliar a adequação da TNE entre o prescrito e administrado em pacientes internados em uma Unidade de Terapia Intensiva de um Hospital de Ensino do interior do Rio Grande do Sul. Métodos: Trata-se de um estudo descritivo, transversal e quantitativo realizado com 25 pacientes com idade ≥18 anos, que estavam em TNE exclusiva por período ≥72 horas. A coleta de dados foi realizada entre setembro a dezembro de 2017 por meio de um formulário do Serviço de Nutrição da instituição e o acompanhamento foi realizado até a alta da unidade ou óbito ou descontinuação da TNE exclusiva. Considerou-se, como adequada a administração energética e proteica ≥ 80% das necessidades. Os dados foram analisados através de estatística descritiva e testado a Correlação de Spearman no programa SPSS versão 20.0.Resultados: Dos pacientes avaliados, a maioria apresentava sobrepeso (52%), doença pulmonar (28%), iniciaram com a nutrição precoce em até 48 horas (92%), tiveram alta domiciliar (68%) e 20,0% apresentaram vômito ou diarreia. Observamos que a adequação do valor calórico ficou 88,25% ± 2,55, semelhante à adequação de proteína 88,49% ± 2,51. Conclusão: Houve uma adequação entre as calorias e a proteína prescrita e administrada superior ao esperado, permitindo uma efetiva administração calórico/proteica, sendo assim mais eficaz a conduta nutricional. O diabetes mellitus tipo 2 é uma doença metabólica caracterizada por hiperglicemia que ocorre tanto por resistência à ação da insulina como por deficiência na secreção deste hormônio. O objetivo deste estudo foi avaliar e relacionar o estado nutricional, as comorbidades e complicações associadas ao diabetes mellitus tipo 2 em pacientes idosos internados em um hospital do município de Santa Cruz do Sul, RS. A amostra foi composta por 25 pacientes idosos, internados em outubro e novembro de 2014. Foram verificados nos prontuários o gênero, o estado nutricional pelo índice de massa corporal (IMC) e a presença de comorbidades e complicações relacionadas ao diabetes. Conforme o IMC, 8% (n=2) dos idosos apresentaram magreza, 32% (n=8), eutrofia e 60% (n=15), excesso de peso. As comorbidades e complicações mais prevalentes foram hipertensão arterial sistêmica (HAS) (84%), doença arterial coronariana (36%), trombose (16%), pé diabético (16%), amputação de membro inferior (16%), insuficiência cardíaca congestiva (16%) e dislipidemia (16%). Os resultados demonstraram a predominância do excesso de peso, sendo o sexo feminino o mais afetado, e a HAS como a comorbidade mais prevalente entre os pacientes diabéticos.
Objective: To evaluate whether vitamin C can help to prevent obesity and hyperglycemia in Wistar rats treated with excess invert sugar to induce prediabetes. Methods: One hundred-day-old Male Wistar rats with a mean weight of 336.58±23.43g were randomly assigned to the following groups: (1) control, receiving water (C); (2) invert sugar control, receiving a 32% watery solution of invert sugar; (3) vitamin C control, receiving a watery solution of vitamin C (60mg/L), and (4) vitamin C plus invert sugar, receiving a watery solution of vitamin C and invert sugar. All animals had access to chow and water ad libitum and were treated for 17 weeks. Prediabetes was assessed according to two criteria: obesity (based on body mass indexand peritoneal fat content) and impaired glucose tolerance (assessed by the intraperitoneal glucose tolerance test and expressed as area under the curve) . Results: Group invert sugar control gained significantly more weight (p=0.035) and visceral fat (p<0.001) than groups vitamin C control and vitamin C plus invert sugar. Consequently, groups vitamin C control and vitamin C plus invert sugar had gained as little body mass index as group C by the end of the experiment. Vitamin C decreased the fasting glycemia of both groups supplemented with vitamin C and normalized the glucose tolerance of group vitamin C plus invert sugar, whose area under the curve matched that of group C. Conclusion: Vitamin C has anti-obesogenic and glycemia-lowering effects in Wistar rats, which might be promising to prediabetics. Future studies are needed to understand the anti-obesogenic and anti-hyperglycemic mechanisms of vitamin C in prediabetes.
Justificativa e Objetivos: A oferta de nutrientes no momento certo, pode minimizar os efeitos do catabolismo exacerbado, melhorando a evolução clínica do paciente. Por isso, o objetivo deste estudo foi avaliar e relacionar o estado nutricional, nutrição precoce e hiperglicemia com o desfecho clínico de pacientes críticos internados em uma unidade de terapia intensiva. Métodos: Trata-se de um estudo de coorte prospectivo, com pacientes críticos de um hospital do interior do estado do Rio Grande do Sul, de ambos os sexos, com idade superior a 20 anos. Foram coletadas informações como, doença (s) de base, diagnóstico principal, idade, sexo, data de internação, tipo e tempo de terapia nutricional instituída, via de administração da dieta, avaliação da glicemia, aplicação de insulina, complicações clínicas, dias de internação e o desfecho clínico. A glicemia e a via de dieta foram acompanhadas durante 72 horas de internação. Resultado: Participaram do estudo 66 pacientes, com média de 64,08±16,11 anos, 59,1% eram do sexo masculino, houve predomínio de sobrepeso (59,1%) e 48,5% internaram por algum evento cardiovascular. Nas 24 horas de internação 15,2% dos indivíduos eram hiperglicêmicos, seguido de 21,2% nas 48 horas e 19,7% nas 72 horas. Verificamos associação entre o desfecho clínico dos pacientes e a nutrição precoce em até 48 horas (p=0,004), assim como, infecção (p=0,036) e os tipos de vias alimentares utilizadas nas primeiras 48 horas (p=0,002) e em até 72 horas (p=0,001). Conclusão: A terapia nutricional precoce influencia diretamente no desfecho clínico de pacientes críticos, apresentando-se como um risco à vida. Descritores: Hiperglicemia. Estado nutricional.
