SUMMARY During 30 years clinical experience in equine obstetrics, certain conditions have been studied in some detail and a few of those which are of importance to the equine practitioner are discussed here. The pathogenesis treatment, and prognosis of these conditions is emphasised. RÉSUMÉ Au cours de 30 années d'expérience clinique en obstétrique équine, certaines conditions ont été particulièrement étudiées. Quelques unes d'entre elles, importantes pour le spécialiste équin, sont discutées. On insiste sur la pathogénie, le traitement et le pronostic de ces conditions. ZUSAMMENFASSUNG 30 Jahre klinischer Erfahrung in equiner Geburtshilfe erlaubten die detaillierte Beobachtung bestimmter Krankheitszustände; einige davon, die für den Pferdepraktiker von Bedeutung sind, werden hier diskutiert. Pathogenese, Behandlung und Prognose dieser Zustände werden hervorgehoben.
Summary The author reviews 40 years of clinical experience including more than 1000 cases of dystocia in mares of different breeds. The high incidence of stillbirth and perinatal mortality of the foal, severe lesions of the genital tract developing during foaling and in the early post partum period are discussed and suggestions made for their prevention and treatment. The advantages of partial embryotomy to avoid unnecessary caesarean sections are emphasised. Special attention is given to the causes of and approach to cases of prolonged gestation; to dystocia caused by retroversion or preparturient torsion of the uterus; and to transverse presentation developed from bicornual pregnancy, regarded as the most important indication for caesarean section. Recommendations are proposed to improve the results of caesarean section for the mare and foal. Résumé L'auteur passe en revue 40 années d'expérience clinique et plus de 1000 cas de dystocies observés sur des poulinières de races différentes. La grande fréquence des mortinatalités et des mortalités périnatales est signalée; les lésions graves de l'appareil génital apparaissant durant le poulinage ou immédiatement après sont discutées et des suggestions sont faites pour leur prévention et leur traitement. Les avantages de l'embryotomie partielle permettant d'éviter une opération césarienne sont soulignés. Une attention particulière est portée aux causes et à la façon de traiter les gestations prolongées, les dystocies par retroversion ou par torsion de l'utèrus, les dystocies par présentation transverse résultant de gestations bicornuales considérées comme les indications majeures de l'opération césarienne. Des recommendations sont formulées pour améliorer les résultats de l'opération césarienne, tant pour la jument que pour la foetus. Zusammenfassung Der Autor vermittelt eine Uebersicht über seine 40‐jährige klinische Erfahrung, die über 1000 Fälle von Dystokie bei Stuten verschiedener Rassen umfasst. Die häufigen Totgeburten und die hohe perinatale Fohlensterblichkeit, sowie die schweren Läsionen im Geschlechtsapparat, die sich unter der Geburt oder in der frühen postpartalen Phase entwickeln, werden diskutiert und Vorschläge zu ihrer Verhütung und Behandlung werden unterbreitet. Die Vorteile der teilweisen Embryotomie zur Vermeidung unnötiger Kaiserschnitte werden hervorgehoben. Besondere Aufmerksamkeit widmet der Verfasser den Ursachen verlängerter Trächtigkeiten, dann aber auch der Dystokie hervorgerufen durch Retroversion oder Torsion des vor der Geburt und der Querlage als Folge einer Zweihorn‐trächtigkeit, die als wichtigste Indikation für den Kaiser‐schnitt angesehen wird. Die vorgeschlagenen Empfehlungen sollten die Resultate der Sectio caesarea für Stute und Fohlen verbessern.
Summary The findings of a prolonged study of retained foetal membranes in a large number of mares are presented. The allantochorion situated in the non‐gravid horn of the uterus is the portion of the membranes most likely to be retained and the condition occurs more frequently in mares which have undergone abortion, dystocia, embryotomy and caesarean section. The most likely cause of retention of the membranes is disturbance of the normal uterine activity that occurs at parturition and delayed uterine involution, metritis and laminitis are serious sequeli of the condition. Attempts to manually remove the membranes should be commenced within 3–6 hours of parturition and a slow one hour intravenous infusion of oxytocin is the treatment of choice if this proves very difficult or impossible. Résumé On présente les résultats d'une étude prolongée sur les rétentions d'enveloppes foetales chez un grand nombre de juments. L'allantochorion situé dans la corne non gravide est la partie des enveloppes qui a le plus de chance d'etre retenue chez les juments. Et cela se produit notamment chez les juments qui ont avortè ou qui ont été soumises à des interventions obstétricales (dystocie, embryotomie ou opération césarienne). La cause la plus vraisemblable de rétention est une perturbation de l'activité utérine normale telle qu'elle se produit lors de la mise bas, avec involution utrine retardée: fourbure et metrite sont des conséquences graves de cette condition pathologique. L'extraction manuelle des enveloppes doit se faire dans les trois à six heures qui suivent la mise bas. Une lente perfusion d'ocytocyne intraveineuse—durant une demi heure—est le traitement de choix si la délivrance manuelle se révèle difficile ou impossible. Man trifft die Retentio häufiger nach Aborten, Schwergeburten, Embryotomie oder Kaiserschnitten. Die wahrscheinlichste Ursache der Retentio ist eine gestörte Uterustätigkeit unter der Geburt. Ernsthafte Folgen einer Retentio sind vor allem verzögerte Uterusinvolution, Metritis und Hufrehe. Die manuelle Loslösung der Placenta sollte innert 3–6 Stunden p.p. erfolgen. Falls dieser Versuch scheitert, wäre das Mittel der Wahl eine langsame Infusion (1 Stunde) von Oxytocin. Summary In 214 cases of severe dystocia in mares, of which 141 (66 per cent) were Draught horses, deviation of the head and neck, with or without torticollis, malformed head and limbs were found to be the cause of dystocia. No evidence of a genetic lethal factor was found and torticollis was often combined with scoliosis of the head and, frequently, with malformation of one or more limbs. This is considered evidence of a common aetiology and pathogenesis of the syndrome of malformation. The malformations were found to be associated with an increased incidence of caudal and, particularly, transverse presentations. These findings are presented as evidence to support the hypothesis that, during the second half of pregnancy, the narrow tip of the uterine horns provides only limited space for the developing cranial half of the foetus. The surprisingly rapid and complete anatomical and functional recovery of severe malformations of the bones of the neck, head and limbs of surviving foals, born by caesarian section, is taken as further evidence that these malformations have no genetic basis, but originate from unfavourable intrauterine conditions.
