SummaryIn the last decade the understanding of the histopathology, immunogenetics and clinical aspects of coeliac disease has changed significantly. Traditionally CD was known as a disease of childhood, managed by paediatricians and characterized by malabsorptions due to villous atrophy, with a fast response to gluten withdrawal from the diet. Looking more carefully to the biopsies we recognized the consecutive stages of intraepithelial lymphocytosis, crypthyperplasia and villous destruction. A specific definition of adulthood coeliac disease was not formulated. We are recognizing patients with malabsorption with only lymphocytic enteritis. Some of them are gluten sensitive. Gluten challenge in adults may be helpful. A large subgroup of adults with only crypthyperplasia and intraepithelial lymphocytosis has been diagnosed. Probably they belong to the gluten sensitive spectrum. Only 10-20% of them are positive for anti-endomysium antibodies. In patients with partial villous atrophy 40-50% of the adults are negative for coeliac antibodies. The threshold for biopsies must be related to case-finding of coeliacs and not to positive antibodies, otherwise milder forms of coeliacs will be missed. Since a few years a better diagnostic approach is available for refractory coeliac disease. Refractory patients can be divided in two categories: type I without aberrant T-cells and type II with aberrant T-cells. The majority of type II patients die within years due to enteropathy associated T-cel lymphomas. The present problems in adult coeliacs do suggest that pathologist must to be more alert for recognizing these non-classical conditions and we must train gastroenterologist to treat their coeliacs properly.Samenvatting Coeliakie komt veel meer voor dan over het algemeen werd aangenomen en screeningsstudies doen vermoeden dat slechts 10% van alle patie¨nten in Nederland als zodanig herkend wordt. Screening of laagdrempelige afname van dunnedarmbiopten bij patie¨nten behorend tot een risicogroep lijkt aangewezen. De presentatie is vooral bij volwassenen vaak mild. Dunnedarmbiopten zijn de hoeksteen van de diagnose en moeten nauwgezet beoordeeld worden. Serologische tests zijn hoog sensitief bij totale vlokatrofie, maar beduidend minder betrouwbaar bij mildere afwijkingen. Immunogenetisch onderzoek zal in de toekomst waarschijnlijk een belangrijke rol gaan spelen in de diagnostiek. De behandeling is helder maar belastend: een levenslang te volgen glutenvrij dieet. Het klinisch en histologisch herstel is meestal snel, maar een aanzienlijk deel van de volwassenen herstelt traag en/of onvolledig. Refractaire coeliakie is een zeldzame, maar bedreigende complicatie van coeliakie bij volwassenen, die onderverdeeld kan worden in twee typen, respectievelijk met en zonder aberrante T-cellen in het oppervlakte-epitheel. Type I lijkt goed behandelbaar met prednison/azathioprine, maar voor type II, waarbij maligne ontaarding tot intestinaal lymfoom frequent wordt gezien, is vooralsnog geen effectieve therapie voorhanden.
