stoma, il linfoangioma, la patologia espansiva della parotide o patologie correlate ai residui degli archi branchiali: in questi casi l'iter diagnostico si conclude con l'indagine ecografica e la TC o la RM vengono utilizzate solo per la definizione spaziale della lesione per la pianificazione dell'intervento chirurgico.Quando invece l'ecografia metta in evidenza una possibile lesione neoplastica della regione cervicale si attiva l'iter diagnostico completo con l'esecuzione del radiogramma diretto, della TC o della RM con riferimento alla patologia sospettata. Le metodiche di imaging devono fornire alcune informazioni fondamentali: un corretto bilancio spaziale, la documentazione di un'eventuale erosione vertebrale, la comunicazione della formazione espansiva con il canale rachideo, i rapporti della massa con il fascio nerveo vascolare del collo e con le vie aeree.Il comportamento clinico diagnostico è invece più articolato quando si studiano pazienti con esclusivo dolore in sede dorso-L'approccio clinico della diagnostica per immagini nella patologia neoplastica vertebrale e paravertebrale nel bambino è sostanzialmente diverso nel caso di interessamento del rachide cervicale (e quindi della regione del collo) rispetto a quello del rachide dorso-lombo-sacrale. Mentre nel primo caso il paziente giunge all'indagine radiologica per evidenza clinica di massa-adenopatia, segni di compressione delle vie aeree, dolore o rigidità del rachide, nella patologia della colonna dorso-lombare è il dolore il segno predominante (almeno nel 70% dei casi); più rare sono le situazioni in cui è il sintomo neurologico o la tumefazione palpabile che portano il bambino all'osservazione del pediatra e successivamente del radiologo pediatra.Nel caso di interessamento del collo è prassi utilizzare l'ecografia come innesco dell'iter diagnostico. Essa, infatti, è capace di distinguere lesioni tipiche che possono simulare vere e proprie masse paravertebrali, come ad esempio formazioni angiomatose, il lipoblalombo-sacrale. Il dolore rachideo è un'evenienza rara nel bambino e spesso è espressione di patologia rilevante 1 . È norma applicare quindi un atteggiamento aggressivo sia clinico (ispezione e palpazione accurata alla ricerca di scoliosi -cifosi -discromie -cutaneemasse -palpabili, corretta valutazione neurologica, ecc.) che dal punto di vista ematochimico (inquadramento reumatologico generale, ricerca di segni di flogosi ecc.). L'atteggiamento aggressivo è anche strumentale: è buona norma eseguire un radiogramma del rachide completo in stazione eretta in due proiezioni ed eventuali radiografie mirate a paziente supino, sull'area del dolore.Il responso del radiogramma del rachide può essere di due tipi: la lesione può non essere identificabile oppure può essere evidente una patologia ben definita.In quest'ultimo caso le patologie possono essere del tutto tipiche, come la malattia di Scheuerman, la spondilolisi-spondilolistesi, le calcificazioni discali idiopatiche ecc, per le quali l'esecuzione www.centauro.it
Several spaces are frequently involved at the same time due to the infiltration of adjacent compartments and the multifocal nature of certain diseases 6 .The diseases most frequently encountered in the visceral space affect the thyroid and parathyroid glands, oesophagus and lymph nodes; abscesses are not uncommon. Atypical branchial cysts and lymph node enlargement are the commonest lesions in the carotid space. The retropharyngeal infrahyoid space is the most common location of abscesses and direct or metastatic infiltration of malignant tumours; lipomas and haemangiomas are found more seldom. Lymphangioma is typically encountered in the posterior cervical space, the location of lipomas, haemangiomas, branchial cysts and lymph node enlargement caused by inflammation or metastases, and abscesses. Lipomas are the commonest lesions found in the anterior cervical space, while the prevertebral space is a typical location for vertebral and brachial plexus lesions 3,4 .Classifying neck masses according to the organ affected results in lesions affecting the soft tissues (SUMMARY -Neck lesions are not uncommon in children and typically present with localised swelling. The neuroradiologist's goal is the diagnosis of cervical lesions as a concomitant pathological finding, especially in cases with intraspinal spread. Neck lesions can be classified in terms of etiology as congenital, inflammatory or neoplastic, but this division is not the best suited to a radiological approach. A more useful diagnostic indicator is the location of the lesion with a further classification based on its radiological hallmarks. An assessment of age distribution is also important for a correct diagnosis.
